Degenerative disorders of the temporomandibular joint: etiology, diagnosis, and treatment.

Temporomandibular joint (TMJ) disorders have complex and sometimes controversial etiologies. Also, under similar circumstances, one person's TMJ may appear to deteriorate, while another's does not. However, once degenerative changes start in the TMJ, this pathology can be crippling, leading to a variety of morphological and functional deformities. Primarily, TMJ disorders have a non-inflammatory origin. The pathological process is characterized by deterioration and abrasion of articular cartilage and local thickening. These changes are accompanied by the superimposition of secondary inflammato

Differential expression of apoptosis-associated proteins in chondrocytes of the mandibular condyles of rabbits with anterior disk displacement.

Apoptosis is a potential contributor to anterior disk displacement (ADD) in temporomandibular joint disorders (TMD). The aim of this study was to investigate the expression of key protein regulators involved in apoptosis in the chondrocytes of mandibular condyles with induced ADD in experimental animals. ADD was surgically induced in the left temporomandibular joint (TMJ) of 15 rabbits without opening their bursas. A sham operation was performed on the right TMJ without displacing the disk forward. At the end of one, two, and four weeks after surgery, mandibular condyle samples were collected

Modified condylotomy versus conventional conservative treatment in painful reciprocal clicking--a preliminary prospective study in eight patients.

Modified condylotomy may be relevant in severe painful reciprocal clicking of the temporomandibular joint (TMJ) where conservative treatment is insufficient. The effect of the modified condylotomy was analyzed and compared with conventional nonsurgical treatment in a randomized pilot study of eight patients, 19-44 years of age, with severe painful reciprocal clicking. Before and after treatment, assessments were performed by subjective reports, clinical recordings, and blinded evaluations of radiography and magnetic resonance imaging (MRI). Based on the clinical evaluations before treatment, a

Magnetic resonance imaging in the diagnosis of intra-articular adhesions of the temporomandibular joint.

We aimed to evaluate the value of magnetic resonance imaging (MRI) for the diagnosis of intra-articular adhesions of the temporomandibular joint (TMJ). We diagnosed 27 consecutive patients with internal derangement of the TMJ (33 TMJs) between 1 December and 31 July 2003 in our department. All patients were examined by MRI and arthroscopy. The MRI findings were recorded as "positive", "suspicious", or "negative", and were compared with those of arthroscopy to obtain the numbers of true positive, true negative, false positive, and false negative results. Arthroscopy confirmed that 18 of the 33

Change in the number of occlusal contacts following splint therapy in patients with a temporomandibular disorder (TMD).

The mean number of occlusal contacts before and after occlusal appliance therapy were compared in three temporomandibular disorder groups and one control group. Only patients with a successful treatment outcome were considered in the statistical analysis. Statistically significant changes were seen in the disc displacement with reduction group, which was managed with anterior repositioning splint: the mean number of contacts increased 3 months after weaning off the splint. If the disc is out of position then the number of occlusal contacts appears to be reduced. It would therefore appear to be
