Who seeks surgical-orthodontic treatment: a current review.

Records for more than 2,000 individuals seen in the Dentofacial Clinic of the University of North Carolina were examined to evaluate trends in referral patterns for orthognathic surgery and acceptance of surgical treatment The vast majority of patients have been white and female from the beginning, and the female-male ratio remained constant at 2:1 throughout the 1990s. The proportion of nonwhite patients increased significantly after 1995, with the change resulting almost totally from more Hispanic and Asian patients. The proportion of African Americans, who represent 22% of the general popul

Two-year follow-up of distraction osteogenesis: its effect on mandibular ramus height in hemifacial microsomia.

Distraction osteogenesis has been used to lengthen the mandible in patients with hemifacial microsomia. Questions regarding soft tissue and skeletal growth after distraction osteogenesis have not been clearly elucidated in the literature. In this case report, a 2-year follow-up of distraction osteogenesis in a 7 year old boy is documented with lateral and posterior/anterior cephalometric analysis. The analysis was performed preoperatively and at specific postsurgical periods to evaluate the facial soft tissue and skeletal growth patterns. Objective analysis of this growing patient after distra

The genetics of human tooth agenesis: new discoveries for understanding dental anomalies.

The important role of genetics has been increasingly recognized in recent years with respect to the understanding of dental anomalies, such as tooth agenesis. The lack of any real insight into the cause of this condition has led us to use a human molecular genetics approach to identify the genes perturbing normal dental development. We are reporting a strategy that can be applied to investigate the underlying cause of human tooth agenesis. Starting with a single large family presenting a clearly recognizable and well-defined form of tooth agenesis, we have identified a defective gene that affe

Urinary catecholamine levels and incidence of dentofacial injuries in children: a 2-year prospective study.

Urinary catecholamines have been used to measure emotionally stressful states which may have an impact on dentofacial injuries. This prospective study investigated the hypothesis that urinary catecholamines such as epinephrine, norepinephrine and dopamine do not affect significantly the incidence of dentofacial injuries in children. As a test of this hypothesis 204 children, males and females, aged 8 to 10 years, were included in the study. After a first examination, each subject was re-examined, at one-year intervals, over a period of two years. Dentofacial injuries were recorded by a clinica

Osteogenesis imperfecta and orthognathic surgery: case report with long-term follow-up.

Osteogenesis imperfecta (OI) is the product of the abnormal synthesis and/or production of Type I collagen. Successful surgical management of extremity and spinal skeletal problems secondary to OI is documented in the orthopedic literature. Reports of successful facial skeletal surgery in all types of OI are encouraging. The purpose of this paper is to report on the long-term results of an orthognathic surgery patient successfully treated to correct a severe dentofacial deformity. The patient underwent an uncomplicated Le Fort I osteotomy with homologous interpositional bone grafts to advance

Tooth anomalies associated with failure of eruption of first and second permanent molars.

The occurrence of tooth anomalies in association with failure of the first and second molars to erupt was assessed in a sample of 1520 nonsyndromic subjects with uncrowded dental arches (mean age, 14 years 4 months) and compared with the prevalence rate calculated in a matched control group of 1000 subjects. The tooth anomalies examined included infraocclusion of deciduous molars, palatal displacement of maxillary canines, rotation of maxillary lateral incisors, aplasia of second premolars, and small size of maxillary lateral incisors. Associations among arrested eruption of first and second p

Antisense oligonucleotide blockade of connexin expression during embryonic bone formation: evidence of functional compensation within a multigene family.

Prior studies in our laboratory demonstrated the presence of gap junction proteins (connexins) throughout intramembranous bone formation [Minkoff et al. (1994) Anat Embryol 190:231-241]. In addition, two members of the connexin family of gap junction proteins, connexin 43 (Cx43; Gj alpha 1) and connexin 45 (Cx45; Gj alpha 6), were found by Civitelli et al. [1993; J Clin Invest 91:1888-1896] to be associated, specifically, with osteogenesis. Recently, however, a null mutation in the gene encoding Gj alpha 1 in mice has been produced by Reaume et al. [1995; Science 267:1831-1834]. Gj alpha 1 nul

Design and fabrication of a modified protraction headgear for Class III long face patients.

Treatment of children with long face and with Class III malocclusions is difficult because most of the available appliances treat one of the two problems at the cost of aggravating the other one. In this article, we present the steps of design and fabrication of a modified protraction headgear that can be useful for such a patient. A patient treated with this approach is presented.

The heritability of malocclusion: Part 1--Genetics, principles and terminology.

The relative contribution of genes and the environment to the aetiology of malocclusion has been a matter of controversy throughout the twentieth century. Genetic mechanisms are clearly predominant during embryonic craniofacial morphogenesis, but environment is also thought to influence dentofacial morphology postnatally, particularly during facial growth. Orthodontic and orthopaedic techniques are used in the treatment of malocclusion and other dentofacial deformities, but with limited effectiveness. The key to the determination of the aetiology of malocclusion, and its treatability lies in t

Case report of malocclusion with abnormal head posture and TMJ symptoms.

Abnormal cervical muscle function can cause abnormal head posture, adversely affecting the development and morphology of the cervical spine and maxillofacial skeleton, which in turn leads to facial asymmetry and occlusal abnormality. There can be morphologic abnormalities of the mandibular fossa, condyle, ramus, and disk accompanying the imbalance of the cervical and masticatory muscles activities. Two normally growing Japanese female patients with Class II Division 1 malocclusion presented with TMJ symptoms and poor head posture as a result of abnormal sternocleidomastoid and trapezius cervic

Prevalence of skeletal and dental anomalies and normal variants seen in cephalometric and other radiographs of orthodontic patients.

Pretreatment cephalometric radiographs may contain important incidental findings that require attention before orthodontic therapy. A review of the cephalometric and dental radiographs of 325 consecutive healthy orthodontic patients revealed 431 notable findings of the skull, cervical spine, and maxillofacial complex. Most of these findings were nonpathologic anomalies or normal variants. If recognized as such by the orthodontist, no further evaluation would be required, thus avoiding unnecessary costs and patient anxiety. However, there were 15 findings (3.5%) that required additional evaluat

The community and orthodontic care. Part II: Community-perceived importance of correcting various dentofacial anomalies. Part III: Community perception of the importance of orthodontic treatment.

Part II. A professionally-managed telephone survey was undertaken to assess the community-perceived importance of correcting various dentofacial anomalies. The sample included 505 respondents, aged eighteen and over, from metropolitan and non-metropolitan households across the state of Victoria. The sample distribution had a 95 per cent confidence limit with a 5 per cent margin of error and closely matched the known population distributions for age, sex and geographical location. This article forms part two of a series. It was found that the correction of functional problems such as "difficult

Eruption of the permanent upper canine: a radiologic study.

The early detection of eruptive anomalies of the upper canine requires an understanding of its normal eruptive pattern. We studied this pattern in terms of upper canine inclination and its relation to the lateral incisor, on the basis of the panoramic radiographic records of 305 children aged 4 to 12 years. The study sample comprised 554 maxillary canines in the oral pre-emergence phase of eruption. Subject age, sex, inclination of the canine (CI), its relation to the lateral incisor (RCLI), and development of the lateral incisor (DLI) were evaluated. The results show that the canine erupts, i

[Perioperative antibiotic prophylaxis in orthodontic bone operations of the facial skull].

Intraoral orthognathic surgical procedures are clean-contaminated operations because of the facultative pathogenic flora of the oral cavity. Without antibiotic prophylaxis in this kind of operation a postoperative wound infection can be expected in 20-31% of cases. Therefore, a retrospective analysis of the clinical course of 545 patients with various dentofacial deformities was performed to evaluate the importance of perioperative antibiotic prophylaxis. The total rate of wound infections was 2.8% and lower than rates published in other comparable studies without antibiotic prophylaxis. The r

The prevalence of facial asymmetry in the dentofacial deformities population at the University of North Carolina.

In a retrospective survey of 1,460 patients evaluated in the Dentofacial Clinic at the University of North Carolina, 495 (34%) were found to have clinically apparent facial asymmetry. When present, asymmetry affected the upper face in only 5% (n = 23), the midface (primarily the nose) in 36% (n = 178), and the chin in 74% (n = 365). The occlusal plane was canted, indicating vertical asymmetry, in 41% (n = 201). Patients with Class II problems, whether or not due to mandibular deficiency, had a 28% prevalence of asymmetry; those with other types of problems (e.g., Class III, long face, Class I)

Hearing, language, speech, vestibular, and dentofacial disorders in fetal alcohol syndrome.

Fetal alcohol syndrome (FAS) is characterized by congenital anomalies traditionally associated with hearing disorders. The present study sought to (a) evaluate possible central hearing loss; (b) verify and extend previous observations on sensorineural and conductive hearing losses; (c) evaluate possible vestibular disorders; (d) examine the relationships between hearing, speech, language, vestibular, and dentofacial disorders in FAS patients; and (e) evaluate the influence of patient age, race, and gender on the expression of these morbidities. A biracial group of 22 FAS patients (aged 3 to 26

Maxillomandibular deformity in association with Poland anomaly.

A case of jaw deformity in association with right-sided Poland anomaly in an 18-year-old male patient is presented. As there have been no cases previously reported of pectoralis major muscle hypoplasia combined with dentofacial deformity, it is hoped that other reports would emerge to describe a new syndrome.

The decision to extract: part II. Analysis of clinicians' stated reasons for extraction.

In a recently reported study, the pretreatment records of each subject in a randomized clinical trial of 148 patients with Class I and Class II malocclusions presenting for orthodontic treatment were evaluated independently by five experienced clinicians (drawn from a panel of 14). The clinicians displayed a higher incidence of agreement with each other than had been expected with respect to the decision as to whether extraction was indicated in each specific case. To improve our understanding of how clinicians made their decisions on whether to extract or not, the records of a subset of 72 su

Prenatal craniofacial development: new insights on normal and abnormal mechanisms.

Technical advances are radically altering our concepts of normal prenatal craniofacial development. These include concepts of germ layer formation, the establishment of the initial head plan in the neural plate, and the manner in which head segmentation is controlled by regulatory (homeobox) gene activity in neuromeres and their derived neural crest cells. There is also a much better appreciation of ways in which new cell associations are established. For example, the associations are achieved by neural crest cells primarily through cell migration and subsequent cell interactions that regulate
