Non-surgical treatment of severe anterior open bite, hyperdivergent pattern, and posterior crossbite in an adult with atrophy of the mandibular first molar extraction site: A case report.

Anterior open bite presents a significant challenge in orthodontic treatment due to its multifactorial aetiology. Managing such cases in adult patients is further complicated when additional factors, such as posterior crossbite, are present. Space closure in an atrophic edentulous ridge poses an added challenge due to reduced alveolar bone width and the risk of periodontal compromise. The diagnostic and treatment details of the present case were documented in accordance with the CARE guidelines for clinical case reporting. This case report describes the successful non-surgical management of an

Segmenting beyond the imaging data: creation of anatomically valid edentulous mandibular geometries for surgical planning using artificial intelligence.

BACKGROUND AND OBJECTIVES: Mandibular reconstruction following continuity resection due to tumor ablation or osteonecrosis remains a significant challenge in maxillofacial surgery. Virtual surgical planning (VSP) relies on accurate segmentation of the mandible, yet existing AI models typically include teeth, making them unsuitable for planning of autologous transplants dimensions aiming for reconstructing edentulous mandibles optimized for dental implant insertion. This study investigates the feasibility of using deep learning-based segmentation to generate anatomically valid, toothless mandib

Horizontal Bone Augmentation with Natural Collagen Porcine Pericardium Membranes: A Prospective Cohort Study.

Background and Objectives: Adequate buccal bone thickness is critical for long-term peri-implant health and stability. When residual alveolar bone volume is insufficient, guided bone regeneration (GBR) is a widely adopted technique. While non-resorbable membranes provide structural support, they carry a higher risk of complications and require secondary surgery. Resorbable collagen membranes, offer promising biological properties and easier clinical handling, yet clinical data remain limited. This prospective cohort study aimed to evaluate the clinical and radiographic outcomes of horizontal G

What We Have Learned in the Treatment of Seven Severely Atrophic Maxillary Full-Arch Immediate-Load Protocol Patient Cases Treated with a Digitally Planned and Produced Screw-Retained Subperiosteal Implant, a Case Series with Surgical and Initial Prosthetic Outcomes.

The volume and morphology of alveolar bone drives the rehabilitation of edentulous patients. Rehabilitation of the atrophic ridge remains a challenge, both surgically and prosthetically. Commonly, bone grafting procedures, in combination with endosseous dental implants, are necessary. Graftless options, using tilted and/or longer endosseous dental implants placed into pyriform rim, nasal, pterygoid, and zygomatic bone have been developed to treat these problem cases. These options are technique sensitive and can result in increased patient morbidity. Digitally planned, custom milled, and rigid

A digitally designed fixed subperiosteal implant solution for the treatment of the severely atrophic full-arch with an immediate-load protocol.

Alveolar bone morphology drives the edentulous patient's rehabilitation. In edentulism, maxillary resorption is often in three vectors: superiorly, medially, and posteriorly, leading to a prosthetically unfavorable pseudo-class III ridge relationship. Longstanding edentulism, especially in the context of ill-fitting dentures, can result in severe maxillary atrophy. Rehabilitation of the severely atrophic maxilla remains a challenge, frequently requiring extensive bone grafting procedures in combination with endosseous dental implants. Graftless options, using longer endosseous and/or tilted de

Alveolar bone heterogeneity during aging: a cone-beam CT based assessment of alveolar bone in tooth loss patients.

OBJECTIVE: This retrospective study aimed to evaluate differences in cortical and trabecular alveolar bone between normal dentition and edentulous sites across different age and sex groups. METHODS: A total of 191 partially edentulous patients who underwent preoperative cone-beam CT (CBCT) were retrospectively analyzed. Cortical bone thickness, cortical bone density, and trabecular bone density were measured at both normal dentition and edentulous sites across six anatomical regions in the maxilla and mandible. Comparisons were made across age groups and sexes, and between paired edentulous an

Horizontal Augmentation via Ridge Splitting and Expansion for Implant Placement in Atrophic Sites: A Prospective Non-Randomized Controlled Trial.

The alveolar ridge split and expansion (ARSE) technique is a predictable approach for horizontal ridge augmentation in atrophic edentulous sites. The objectives of the present study were to assess the impact of piezoelectric surgery and screw expanders on tissue and implant health, and also to comparatively evaluate the efficacy of piezoelectric surgery and screw expanders in edentulous ridge expansion compared to the conventional drilling technique. Thirty-six single or multiple edentulous sites were recruited into 3 groups based on ridge width measurements, that is, 12 sites/group. In group

Does orthodontic space opening in patients with congenitally missing maxillary lateral incisors also reduce the need for bone grafting during implant placement? A retrospective study.

BACKGROUND: Different studies have provided inconsistent results regarding the effectiveness of orthodontic tooth movement in establishing an adequate width and height of the edentulous ridge in patients with missing maxillary lateral incisors. This study aimed to compare the dimensions and density of the alveolar ridge after canine distalization for the preparation of implant placement and after no significant canine movement along the ridge. METHODS: Sixteen patients (30 sites) with congenitally missing teeth were included in this retrospective study. The patients were divided into two grou

Ramus Frame Dental Implant Functional Survival and Safety in Severely Atrophic Edentulous Human Mandibles.

Ramus frame dental implants were retrospectively studied in 360 adults with severely atrophic edentulous mandibles. Patient records up to 12 years post-treatment were independently reviewed after a single clinician surgically placed titanium long-arm ("Tatum") ramus frame implants and immediately loaded them with a mandibular overdenture. A total of 11 ramus frames were removed at 19 to 109 months post-treatment, mostly due to supramucosal bar fracture (N = 6) or mobility (N = 3). Kaplan-Meier product-limit analysis revealed the post-treatment survival probability for functional ramus frame im

Less Is More: A Case Report on All-on-4 Prosthesis.

Severe alveolar ridge atrophy following tooth removal poses a common and worsening issue in edentulous jaws, affecting patient comfort and quality of life. The All-on-4 approach involves axially positioning two anterior implants and distally orienting two posterior implants, reducing cantilever length and enabling prostheses with 12 teeth. This less invasive option, utilizing distal tilting of implants at 30°, offers biomechanical advantages and has shown short-term success, though long-term research is limited. Immediate-loading treatments are gaining popularity for their high survival rates.

Rehabilitation of Edentulous Arch Using All-on-Four Treatment Protocol: A Case Report.

In edentulous arches, alveolar ridge atrophy after tooth extraction is a common problem that affects patient comfort and quality of life. Implant-supported fixed restorations are a well-proven treatment option for edentulism. The concepts of implant dentistry have developed over time to produce better aesthetics and functional results. To reduce cantilever length and enable prostheses with 12 teeth, the all-on-four technique entails inserting two anterior implants axially and distally orienting two posterior implants. Compared to conventional loading, immediate loading offers various benefits

The Evolution of Custom Subperiosteal Implants for Treatment of Partial or Complete Edentulism in Patients with Severe Alveolar Ridge Atrophy.

Dental implants have always played an important role in dentistry and have been used to replace missing teeth since around 600 AD. They can be classified into three groups: endosteal, subperiosteal, and transosteal. Over time, different materials have been used to manufacture dental implants and these, in turn, can be divided into three groups: metals, ceramics, and polymers. Today, the most commonly used treatment for edentulism is the use of endosteal implants. However, such an approach cannot be used in patients with severe alveolar ridge atrophy and, in such cases, custom subperiosteal imp
