A contemporary approach for treatment planning of horizontally resorbed alveolar ridge: Ridge split technique with simultaneous implant placement using platelet rich fibrin membrane application in mandibular anterior region.

Treatment of edentulous sites with horizontal atrophy represents a clinical situation in which the positioning of endosseous implants might be complex or sometimes impossible without a staged regenerative approach. This case report presents management of horizontally deficient mandibular anterior ridge with a contemporary approach to treatment planning and application of platelet-rich fibrin membrane for ridge split technique and simultaneous implant placement. Implants in anterior mandibular area are considered to be most predictable, stable, with high success rate and patients' satisfaction

Tissue Engineering Therapy for Atrophic Maxilla Using Minimally Manipulated CD90 and CD105 Bone Marrow Stem Cells: A Case Report.

Bone deficiencies in the jaws of edentulous patients limit the possibility of successful placement and osseointegration of endosseous implants. Bone resorption is defined as a vertical and horizontal loss of quality and quantity in the residual bone ridges; lack of bone structure requires bone grafting procedures to enable implant placement and to improve the function, predictability, and longevity of implants. The objective of this case study was to demonstrate the feasibility of a novel cell therapy treatment for localized jaw bone defects with tissue repair cells composed of a mixture of bo

Segmental sandwich osteotomy and tunnel technique for three-dimensional reconstruction of the jaw atrophy: a case report.

BACKGROUND: A three-dimensionally favourable mandibular bone crest is desirable to be able to successfully implant placement to meet the aesthetic and functional criteria in the implant-prosthetic rehabilitation. Several surgical procedures have been advocated for bone augmentation of the atrophic mandible, and the sandwich osteotomy is one of these techniques. The aim of the present case report was to assess the suitability of segmental mandibular sandwich osteotomy combined with a tunnel technique of soft tissue. Based on our knowledge, nobody described before the sandwich osteotomy with tun

Management of Severely Atrophic Mandibular Ridge using Hollow Denture with an Analytical Discussion: A Clinical Case Report.

A severely resorbed edentulous mandibular ridge, although fairly common in occurrence, its rehabilitation poses a challenging clinical situation for the prosthodontist. This has implored prosthodontists to seek for ingenious modifications in the conventional prosthetic forms so as to gratify patient's expectations who have difficulty in wearing the mandibular complete dentures. Opting for the most suitable treatment modality mandates equilibrating the retention, stability and support triad to the fullest. Extreme resorption of the denture bearing tissue bed may lead to problems with achieving

Histological and histomorphometric evaluation of the synthetic biomaterial Natix® in horizontal reconstruction of alveolar ridge.

BACKGROUND: Following loss of teeth, atrophy of alveolar ridge of the jaws is a substantial problem and unintended outcome that compels clinicians to perform bone reconstruction ahead of implant placement. Although autogenous bone is recommended as the gold standard in bone reconstruction, aninvasive second surgery harvestinga limited volume of bone (from intraoral source) has led a significant approachingthe use of synthetic bone substitute materials. The aim of this study was to evaluate the histologic and histomorphometric properties of porous titanium granules (Natix®) used in horizontal r

Soft-Tissue Enhancement Combined With Biologically Oriented Preparation Technique to Correct Volumetric Bone Defects: A Clinical Case Report.

The aim of the present case report was to illustrate how to obtain an ideal alveolar ridge contour by means of peri-implant soft-tissue management combined with the prosthetic approach of the biologically oriented preparation technique (BOPT). A patient presenting a moderate vertical and horizontal bone loss in the posterior maxilla was treated with sinus floor elevation and simultaneous implant placement. During the reentry procedure, the horizontal defect was corrected with an apically repositioned flap combined with a connective tissue graft. To increase the volume of the interimplant papil

Interpositional Augmentation Technique in the Treatment of Posterior Mandibular Atrophies: A Retrospective Study Comparing 129 Autogenous and Heterologous Bone Blocks with 2 to 7 Years Follow-Up.

The aim of this retrospective study was to compare clinical and radiographic outcomes of the interpositional (inlay) augmentation technique in atrophic posterior partially edentulous mandibles using three different types of block bone grafts: autogenous bone block harvested from the iliac crest, deproteinized bovine bone mineral block, and collagenated equine bone block. A total of 115 patients were included with a 4.2-year mean after-loading follow-up. Data seem to suggest that heterologous bone blocks are similar in results to autogenous bone blocks, so they might be considered preferable as

Alveolar ridge atrophy related to facial morphology in edentulous patients.

OBJECTIVES: The morphology of the alveolar process determines the retention and stability of prosthetic restorations, thereby determining the result of the therapy. Considering that the edentulous jaws may be affected by the atrophy process, it was hypothesized that the morphology of the alveolar process of the maxilla may be dependent on the anterior facial height and anatomy of the mandible. SUBJECTS AND METHODS: Twenty-five healthy edentulous Caucasian individuals were randomly chosen. Each subject underwent a lateral cephalogram before and after prosthetic rehabilitation. During expositio

Classification of the level of mandibular atrophy - A computer-assisted study based on 500 CT scans.

OBJECTIVE: For the optimal treatment of patients with highly atrophic mandibles, it is required to assess and quantify the extent of atrophy. The classification schemes that are well established today are known to be limited with respect to objectivity and reproducibility. Thus, the aim of the study was to generate a computer-aided method of classification, investigate its applicability in comparison with the established methods, and apply it to a large set of data. MATERIALS AND METHODS: Mandibular geometries were segmented from 500 Multislice (MSCT) datasets of atrophic and non-atrophic man

Rehabilitation of Atrophied Low-Density Posterior Maxilla by Implant-Supported Prosthesis.

The rehabilitation of edentulous posterior maxilla is a challenging task that could be complicated by the availability of low-density and reduced height of alveolar bone. Patients were enrolled in this study upon the presence of type IV bone and residual bone height <5 mm in posterior maxilla. Novel procedure for transalveolar sinus floor elevation was described to insert 10 short implants in 4 patients. Neither implant failure nor prosthetic failure was observed during a follow-up of 44 ± 3 months. Mesial bone loss of 0.5 ± 0.6 mm and a distal bone loss 1.3 ± 0.9 mm were measured after 35 ± 4

Histological Features and Biocompatibility of Bone and Soft Tissue Substitutes in the Atrophic Alveolar Ridge Reconstruction.

The reconstruction of the atrophic alveolar ridges for implant placement is today a common procedure in dentistry daily practice. The surgical reconstruction provides for the optimization of the supporting bone for the implants and a restoration of the amount of keratinized gingiva for esthetic and functional reasons. In the past, tissue regeneration has been performed with autogenous bone and free gingival or connective tissue grafts. Nowadays, bone substitutes and specific collagen matrix allow for a complete restoration of the atrophic ridge without invasive harvesting procedures. A maxilla

Le Fort 1 osteotomy and calvarial bone grafting for severely resorbed maxillae.

INTRODUCTION: Long standing maxillary edentulism leads to alveolar ridge resorption which prevent implant placement and causes prosthetic malocclusion. The aim of the study was to assess vertical and transversal bone increase following Le Fort 1 osteotomy associated with calvarial bone grafting. MATERIALS AND METHODS: 66 patients who presented severely atrophic maxillae were treated with Le Fort 1 osteotomy with bone grafting from 2003 to 2014. Vertical and transversal bone level was measured preoperatively and 6 months post-operatively to calculate the alveolar ridge augmentation. Follow up

Maxillary Sinus Augmentation.

The clinician contemplating reconstruction of the posterior maxilla with osteointegrated implants must overcome a number of anatomic challenges. Buccolingual and/or apico-occlusal atrophy of the edentulous ridge and pneumatization of the maxillary sinus often limit the volume of bone available for implant placement. Many clinicians advocate augmentation of the maxillary sinus to increase the dimension of available bone for implant placement. A female patient aged 38-year old was undergone maxillary sinus augmentation (sinus lifting) after improper implant placing in upper right molar area. Car
