Identifying incident medication-related osteonecrosis of the jaw and antiresorptive drug holidays using natural language processing on clinical notes in men and women prescribed bisphosphonates or denosumab for fracture prevention.

OBJECTIVE: Medication-related osteonecrosis of the jaw (MRONJ) is a rare complication of antiresorptive therapy for osteoporosis, with risk potentially influenced by "drug holidays" around dentoalveolar procedures. MRONJ risk is higher in rheumatic diseases. Epidemiologic studies are limited by poor administrative code performance for MRONJ identification and lack of structured pharmacy data capturing drug holidays. We developed and validated a natural language processing (NLP) algorithm to classify MRONJ and detect drug holidays from free-text clinical notes in electronic health records (EHR)

INfORM/IADR Key Points for Good Clinical TMD Practice-A Critical Comment.

PURPOSE: To critically evaluate the recently published INfORM/IADR consensus paper containing 10 key points for good clinical practice in the management of patients with temporomandibular disorders (TMDs). MATERIALS AND METHODS: After perusing the text of the document, followed by intensive discussion, the 10 key points were carefully analyzed and annotated. RESULTS: Two key points could be replaced by other (clinically relevant) statements. In addition to terminologically and semantically imprecise formulations, some verbal descriptions remain superficial and vague. While no distinction was

Viral infections are associated with apical periodontitis: A meta-analysis of prevalence, clinical symptoms, and lesion sizes across 31 clinical studies.

OBJECTIVE: Bacteria and viruses are components of the oral microbiome and are linked to various oral diseases. Clinical observations indicate a higher prevalence of apical periodontitis (AP) during viral epidemics. However, research on this association is limited. This meta-analysis aimed to explore the relationship between viral infections and AP. METHODS: This study adhered to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Relevant studies were identified through systematic database searches, and data were extracted for eligible studies. Three validated q

Follow-up of periapical inflammation after root canal retreatment and surgical endodontic treatment using dental-dedicated magnetic resonance imaging: a case report.

The aim is to present a dental-dedicated magnetic resonance imaging (ddMRI)-based diagnosis and follow-up in a clinical case of apical periodontitis (AP) on a root canal treated (RCT) tooth. Persistent AP in RCT teeth can be difficult to distinguish from scar tissue or incomplete healing. In the absence of symptoms, no currently known imaging method can verify the presence of inflammation. ddMRI is a novel imaging modality proposed for dentistry, that has the potential to reveal inflammation. A 50-year-old male presented with mild symptoms from a previously RCT 36. ddMRI suggested mild inflamm

Modified T-Shaped Closure of Rostral Maxillary Defects in Dogs Following Rostral Maxillectomy.

Rostral maxillary defects following curative intent surgery for maxillary neoplasms can be difficult to close. This paper describes a modified T-shaped closure following rostral maxillectomy that permits the primary mucosal closure of larger rostral maxillary defects that may not be amenable to standard closure techniques. The technique facilitates decreased incision site tension, results in a more cosmetic closure (decreased concave deformity of the muzzle), maintains normal vestibule size, preserves critical vasculature, and maintains the greatest amount of available tissue for repair of def

Dental-Dedicated MRI in the Assessment of Pulp Vitality and Apical Periodontitis: A Feasibility Study.

INTRODUCTION: This feasibility study presents the diagnostic accuracy of a novel dental-dedicated magnetic resonance imaging (ddMRI) system for assessing pulp vitality and diagnosing apical periodontitis (AP), as an adjunct to clinical assessment and cone beam CT (CBCT) imaging. METHODS: Ten consecutive patients with possible endodontic problems were screened, and ultimately 18 teeth in nine patients (10 healthy teeth, five necrotic teeth with suspected AP and three necrotic or root canal treated teeth with no suspicion of AP) with a recent CBCT were included. The teeth were tested for vitali

The efficacy of freehand, pilot drilled and fully guided implant surgery in partially edentulous patients: A randomized control trial.

BACKGROUND: Partial edentulism poses challenges to oral function, aesthetics, and quality of life. Implant placement techniques-freehand, pilot-drilled, and fully guided-differ in accuracy, surgical time, and outcomes. In this study, only one predefined index implant per patient was analyzed to avoid confounding from multi-implant cases, and template fabrication for the pilot-drilled group was performed using diagnostic wax-up and thermoplastic material. This study evaluated these techniques in partially edentulous patients. METHODS: Ninety patients were randomly assigned to three groups: fre

Reconstruction of the atrophic maxilla combining Le Fort I osteotomy, sinus augmentation and titanium mesh guided bone regeneration: a technical note.

Severe atrophy of the edentulous maxilla poses a significant surgical challenge. This technical note presents a novel approach combining Le Fort I osteotomy, bilateral sinus augmentation, and titanium mesh guided bone regeneration. Preoperative prosthetic planning allows accurate maxillary repositioning to restore a skeletal Class I relationship and enable an implant-supported prosthesis without prosthetic gingiva. The titanium mesh provides stable, three-dimensional support for uniform ridge reconstruction, enabling ideal implant placement. This technique offers a reproducible and less invasi

Computer-Guided Biopsy of Osteosclerotic Jaw Lesion Using 3D-Printed Surgical Guides: A Fully Digital Workflow.

Osteosclerotic jaw lesions, often incidentally detected on routine radiographs, are rarely biopsied due to their benign appearance and proximity to delicate anatomic structures. This case report presents a fully digital workflow for guided biopsy of a deep mandibular osteosclerotic lesion, integrating artificial intelligence-based segmentation, intraoral scanning, CAD design, and 3D printing of surgical templates. A 3D-printed guide with depth-control stops was used to safely and precisely position trephine burs between the roots of tooth 4.6 and the mandibular canal. The approach allowed accu
