Lingual thyroid: a new surgical approach--a case report.

Ectopic lingual thyroid is a rare developmental anomaly, presenting in the adult with obstructive symptoms during respiration, deglutition and speech. The main therapeutic issue after exact diagnosis is the restoration of thyroid function, and surgical intervention, when necessary, has to deal with problems related to the site and function of the ectopic gland. A case of a 25-year-old female with dyspnoea and dysphagia due to a 3 x 4 cm ectopic lingual thyroid, excised by an original peroral approach via a mandibular midline osteotomy, is reported. Our surgical technique is described and the d

Advances in maxillofacial imaging.

The maxillofacial region, extending from the base of the skull to the hyoid bone, is one of the most anatomically complex regions of the body. This area contains elements and organs belonging to a number of different systems that can be affected by a variety of local and systemic pathologic processes. Diagnostic imaging has assumed a central role in the evaluation of this region. In the following monograph, we present the reader with an overview of contemporary maxillofacial imaging, focusing on three areas: recent technical advances that impact on the diagnosis of the maxillofacial region, th

Marden-Walker syndrome: a case report and a critical review of the literature.

We present a patient with blepharophimosis, joint contractures, immobile facies, decreased muscular bulk, postnatal growth retardation, developmental delay, micrognathia, cleft palate, camptodactyly, arachnodactyly, pectus, kyphoscoliosis, hypospadias, and absent deep tendon reflexes. These findings are consistent with Marden-Walker syndrome (MWS). Twenty-two additional cases in the literature are reviewed. Diagnostic criteria are proposed, and the spectrum of variability is discussed. Evidence for autosomal recessive inheritance is reviewed as is the differential diagnosis. Possible pathogene

[Surgical and orthodontic treatment approach for cleidocranial dysostosis].

Developmental deformities of the permanent dentition are of primary concern in the treatment of patients suffering from cleidocranial dysostosis. The therapeutic approach should be based on interdisciplinary cooperation between orthodontists as well as oral and maxillofacial surgeons. Therapy may include surgical exposure of impacted teeth in combination with removal of supernumerary teeth. Already during the surgical procedure orthodontic bands and brackets may be attached. The next step in treatment should consist of orthodontic alignment of the teeth and correction of the jaw relations. In

The effect of mouth closure on drooling and speech.

The efficacy of a feeding program to decrease drooling and increase vocalizations by promoting mouth closure was explored in two boys with cerebral palsy through the use of oral facilitation techniques. A single-subject ABA design was repeated across subjects. Baseline 1 consisted of 5 (Subject 1) and 10 (Subject 2) half-hour sessions during which the subjects were fed applesauce and apple juice without treatment. A 1-hr play session followed, during which saliva was collected on an absorbent bib and weighted. Bilabial vocalizations produced by the subjects were recorded and counted. A 12-sess

A child with signs of developmental apraxia of speech with whom a palatal lift prosthesis was used to manage palatal dysfunction.

A client is described who exhibited a large number of characteristics consistent with developmental apraxia of speech (DAS). The exhibited symptoms included excessive nasal resonance and nasal emission of air due to velopharyngeal port dysfunction, for which successful management was achieved by use of a palatal lift prosthesis. The results of the client's use of the lift, in conjunction with her speech and language remedial programming, is presented.

[Developmental disorders of the permanent dentition following fractures of the visceral cranium during the growth period. Incidence and morphological findings].

The retrospective examination of 70 patients who had suffered fractures in tooth-bearing segments of the maxilla and the mandible when they were between one and twelve years old revealed a high incidence of late pathologic changes (in 39 out of 70). Disorders in odontogeny, eruption of the teeth and occlusion were observed. Primary and secondary loss of teeth did also occur. Our results show the necessity to inform both the young patients and their parents about the high incidence of late changes after jaw fractures in childhood. An effective recall-system is required in order to diagnose and

The 'happy puppet' syndrome of Angelman: review of the clinical features.

Thirty six children with typical features of Angelman's syndrome, including global developmental delay, ataxia, episodes of paroxysmal laughter, seizures, and microcephaly were studied. The series included three sibships of three affected sisters, two affected brothers, and two affected sisters, respectively. The facial appearance is characterised by a prominent jaw, a wide mouth, and a pointed chin. Tongue thrusting is common. The movement disorder consists of a wide based, ataxic gait with frequent jerky limb movements and flapping of the hands. Tone is variable in the limbs with normal refl

[Two cases of temporomandibular arthrosis of young children].

Two cases of temporomandibular arthrosis were reported. Case 1 was a 6 years 4 months old female with pain in her left mandibular region in moving her jaw. Opening of the mouth by herself was slightly limited. Clicking noise was seen and felt from both of her temporomandibular joints. She was in the dental developmental stage IIA by Hellman, and had deep overbite (5mm). Roentogenographic examination revealed a bony defect on her left condylar head. Treatment was performed with an activator. Pain was not felt and the mandibular opening recovered after 2 weeks of treatment. Roetogenographs showe

Fragile X syndrome: recognition in young children.

In recent years, a number of articles have appeared in the literature concerning the fragile X syndrome; however, in few cases was the diagnosis of the syndrome in young children discussed. A review of 20 children younger than 7 1/2 years of age who had the fragile X syndrome seen at the Cincinnati Center of Developmental Disorders was undertaken in an attempt to establish guidelines that would aid the practicing physician in determining which children should have a chromosomal analysis. All children were developmentally delayed; 95% had speech delays. Short attention span with hyperactivity,

An investigation of motor equivalence in the speech of children and adults.

Although the concept of motor equivalence (i.e., articulatory intercoordination) is generally accepted as functioning in normal speech production, few studies have experimentally demonstrated its existence. One purpose of the present study was thus to obtain additional data concerning this phenomenon. Because motor equivalence is often assumed to represent a rather sophisticated ability in speakers, another purpose of the study was to determine whether trends could be observed that might demonstrate a developmental progression toward more frequent occurrence of articulatory intercoordination w

Computerized tomography as a guide in the diagnosis of temporomandibular joint disease.

As described, CT is a noninvasive method for providing detailed views of both the soft and hard tissues of the TMJ region. Because internal derangements of the joint involve both of these structures, CT can provide an invaluable tool in the study, diagnosis, and treatment of various disorders of this nature. It is extremely important, however, to create a close working relationship with the radiologist who will be performing these studies. Only with adequate clinical study and information communicated to the radiologist can true benefits be obtained. As CT scans are available nationwide, this

Diagnosis of radiolucent lesions of the jaw by fine needle aspiration biopsy.

Radiolucent lesions of the jaw, representing a variety of metabolic, inflammatory, developmental, neoplastic and other disorders, may be quite variable in appearance. Fine needle aspiration biopsy, performed on 57 patients who presented with oral abnormalities, was used in an attempt to differentiate the pathologic processes in the 23 patients with radiolucent changes resulting from bony involvement. Most aspirations were performed on an outpatient basis, and the cytologic findings were correlated with the clinical course and subsequent surgical findings. The entities encountered included amel

Far-advanced primary hyperparathyroidism in an 18-year-old young man.

Advanced debilitating primary hyperparathyroidism has become a clinical rarity as the condition is diagnosed at an asymptomatic stage in more patients. An 18-year-old young man is described who was referred because of an expanding facial mass. Evaluation revealed the classic musculoskeletal findings of severe primary hyperparathyroidism, as originally reported in the early part of the century.

[Laterofacial malformations (maxillomandibular neurocristopathies) associated with anomalies of the brain stem and cranial nerves].

Neurological and cerebral computed tomography examinations were conducted in 25 infants and young children with uni- or bilateral maxillomandibular neurocristopathies (Goldenhar's, Franceschetti's first arch syndromes or transient forms). Taking all lesions into account, 18 children (82%) presented morphological and motor anomalies of the brain stem (mesencephalus, metencephalus) and its corresponding cranial nerves: four had morphological anomalies of the cerebellum and pons; four presented disorders of bulbopontine control (heart, lungs, deglutition); fifteen demonstrated sensory, sensorial,

[A plea for early arthroplasty of temporomandibular ankylosis in children].

The well-meaning wait-and-see policy when confronted with uni-or bilateral temporomandibular ankylosis in children is now out-of-date. An early conservative arthroplasty with temporal aponeuroplasty should be performed for several reasons. --Recent advances have increased our knowledge of the biology and development of fetal and infant temporomandibular joints. --Anaesthesia by fibroscopy during endotracheal intubation of the child with permanent jaw constriction is now a safe technique. --The developmental anatomy of the temporomandibular joints allows, employing an appropriate approach, the

Surgical treatment of mandibular prognathism--a review of 25 cases.

Mandibular prognathism is a common developmental jaw deformity with functional disabilities and facial disfigurement, characterised by a prominent mandible. Very satisfying cosmetic and functional results can be achieved by surgery and the literature is replete with surgical techniques for correcting this deformity. This paper discusses the surgical treatment of mandibular prognathism and reviews a series of 25 consecutive patients treated by an extra-oral technique known as Subsigmoid Oblique Osteotomy (Vertical Subcondylar Osteotomy) during a seven-year period, (1969-76).

Focal odontodysgenesis of the maxillary second premolars in a child.

A few months before her sixth birthday, a Caucasian girl suffered progressively worse episodes of painful, nontender, erythematous swellings of the palate lingual to apparently healthy primary second molars. Neither of the developing maxillary second premolars was discerned clearly in radiographs, but, upon surgical exploration, incomplete crowns were found in both follicles. All the other premolars appeared to be developing normally. Histologic, histochemical, and chemical analyses showed that the enamel was hypocalcified, apparently as a result of hypomaturation; the dentin was essentially n
