Charting the course of Sjögren's disease: A historical analysis of scientific progress.

Sjögren's disease (SjD) bears the name of the Swedish ophthalmologist Henrik Sjögren. His 1933 doctoral thesis on keratoconjunctivitis sicca ignited interest, yet much remains under debate. SjD is an autoimmune disorder that primarily affects the exocrine glands, particularly the salivary and lacrimal glands, leading to a dryness of the eyes and mouth. Diagnostic criteria have evolved significantly, with over 10 classification criteria proposed since 1965. From its inception, SjD was often overshadowed by other rheumatic diseases, struggling for recognition as a distinct autoimmune entity. Ear

Sjögren's disease and depression.

Depression is a major and under-recognized comorbidity in Sjögren's disease, contributing to disease burden and impaired quality of life. This narrative review summarizes current evidence regarding the prevalence, pathophysiological mechanisms, clinical impact, assessment, and treatment of depression in patients with Sjögren's disease. Reported prevalence rates range widely from 3.2% to 78.6%, largely owing to methodological heterogeneity, although meta-analytic data demonstrate a significantly increased risk of depression in patients with Sjögren's disease than in healthy controls. Etiology i

Triple immunotherapy containing telitacicept for systemic lupus erythematosus complicated by multiple autoimmune diseases: a case report.

SLE may occur simultaneously with or subsequently to other autoimmune diseases; such cases are called overlap syndromes. Cases of SLE overlapping with three or more autoimmune diseases are extremely rare, and there is no unified standard or consensus for treatment. We report a clinical case of a patient with SLE, immune-mediated necrotizing myopathy, Sjögren's syndrome, and psoriasis. After treatment with a triple immunosuppressive regimen containing telitacicept, the patient achieved a satisfactory therapeutic response. As the first dual-pathway inhibitor targeting BLyS/APRIL, telitacicept de

Efficacy and safety of pharmacological treatment for Sjögren's disease: a network meta-analysis.

BACKGROUND: Sjögren's disease (SjD) is an autoimmune disease characterized by symptoms such as dry mouth and dry eyes, with potential involvement of multiple organ systems. We aimed to evaluate the efficacy and safety of oral pharmacotherapy for SjD through a network meta-analysis (NMA). METHODS: PubMed, Embase, Cochrane Library, and Web of Science were searched for randomized controlled trials (RCTs) up to 9 January 2025. The Cochrane Risk of Bias Tool 2.0 was used to estimate the quality of the studies. Bayesian NMA was implemented utilizing the GEMTC package in R (version 4.5.0) to compare

Differential associations between treatment delay and survival across head and neck cancer subsites: a national hospital-based framework for risk-stratified prioritization.

PURPOSE: Delays in time to treatment initiation (TTI) have been associated with inferior outcomes in head and neck cancer, yet prior studies have typically evaluated tumors collectively, assuming uniform time sensitivity across diseases. This study quantified subsite- and stage-specific associations between TTI and overall survival (OS) and derived groups of tumors with similar TTI-mortality associations. METHODS: We conducted a retrospective cohort study using the National Cancer Database (2010-2017), including adults with head and neck malignancies treated with curative intent. Multivariabl

Translational PBPK Modeling and Exposure-Response Analysis of AT-RvD1 for Treating Sjögren's Disease.

Sjögren's disease (SD) is an autoimmune condition characterized by chronic inflammation and impaired secretory function of the salivary glands (SG). The proresolving lipid mediator AT-RvD1 restores SG function in SD-like mice; however, its tissue pharmacokinetics, exposure-response relationships, and human dose predictions remain poorly understood. Therefore, this exploratory study characterizes AT-RvD1 tissue pharmacokinetics, develops an SG-focused physiologically based pharmacokinetic (PBPK) model, explores the relationship between SG exposure and secretory response, and predicts human expo

Histone lactylation: a metabolic-epigenetic bridge linking glycolysis to salivary gland fibrosis in Sjögren's disease.

Sjögren's disease (SjD) is a chronic, systemic autoimmune disorder characterized by lymphocytic infiltration and progressive loss of exocrine gland function. Despite advances in understanding its immunopathogenesis, the molecular mechanisms governing the transition from inflammation to irreversible salivary gland fibrosis remain obscure. Emerging evidence suggests that the crosstalk between metabolic reprogramming and epigenetic modifications profoundly shapes the local inflammatory microenvironment and accelerates disease progression. Histone lactylation, a recently discovered lactate-derived

Deep learning prediction of radiation-induced xerostomia and personalized outcome-driven treatment planning in head and neck cancer.

Objective. Radiation therapy (RT) is critical in head and neck cancer (HNC) treatment but often causes radiation-induced toxicities, such as xerostomia (RIX). While deep learning (DL)-based models show promise in predicting these toxicities, their black-box nature hinders clinical applications. This study aims to develop a robust DL model to predict RIX 12 months after RT and to leverage model interpretability techniques, specifically class activation maps (CAMs) and voxel-wise dose gradient maps (GMs), to guide personalized treatment plan optimization.Approach. A 3D ResNet-based model was tra

Eosinophilic Sialodochitis Identified in Duct Remnant After Submandibular Gland Resection.

Eosinophilic sialodochitis (ES) is an increasingly recognized cause of recurrent sialadenitis but likely remains underdiagnosed as it is commonly considered to be nonspecific chronic sialadenitis. ES is likely increasing in prevalence in association with the growing number of allergic diseases and should be considered in patients presenting with multiple episodes of salivary gland pain and swelling refractory to conventional treatment. We highlight this difficulty in diagnosis with a report of ES established following Wharton duct remnant swelling after submandibular gland (SMG) resection for

Thrombomodulin Shapes Thrombo-Inflammatory Interactions in Lupus Nephritis.

Lupus nephritis (LN) confers a disproportionate thrombotic risk in systemic lupus erythematosus (SLE) that is not fully explained by conventional cardiovascular risk factors. The underlying molecular mechanisms remain largely unknown. We aimed to characterize the thrombo-inflammatory profiles in LN compared with SLE without nephritis (SLE-NonLN), and contextualized these findings to primary antiphospholipid syndrome (APS), primary Sjögren's disease (SjD), and healthy controls. In a cross-sectional design, we included 325 patients with SLE from two Danish cohorts, 10 with primary APS, 45 with p

The obesity paradox in primary Sjögren's disease: immunometabolic associations of chemerin, platelet count, and statin use.

INTRODUCTION: Sjögren's disease is characterized by autoimmune destruction of exocrine glands. Obesity generally worsens the course of many diseases; however, earlier observations in our cohort suggested a paradoxically lower disease activity (ESSDAI) in overweight/obese patients. This cross-sectional study aimed to investigate potential mechanisms underlying these findings. METHODS: Chemerin and oxidized low-density lipoprotein (oxLDL) levels were measured in a subset of regularly followed patients (n=75). Patients were stratified by body mass index (BMI): BMI <25 kg/m² ("non-obese", n=27) a

IFN-γ-Associated Signaling Networks in Interstitial Lung Disease Associated With Sjögren's Disease.

BACKGROUND: Sjögren's disease (SD) is a chronic systemic autoimmune disorder that can affect multiple organ systems. Interstitial lung disease (ILD) is a clinically significant pulmonary manifestation of SD and is associated with impaired quality of life and increased mortality. Interferon-γ (IFN-γ), a major effector cytokine of Th1 immunity, has been implicated in immune dysregulation and chronic inflammation; however, its contribution to SD-associated ILD (SD-ILD) and its relationship with fibrotic remodeling remain incompletely understood. MATERIALS AND METHODS: This pathway-oriented narra

Diagnostic Utility of the Milan System for Reporting Salivary Gland Cytopathology With Cytohistological Correlation.

INTRODUCTION: Fine needle aspiration cytology (FNAC) is an affordable diagnostic method for assessing salivary gland lesions. However, interpreting cytology results of salivary gland FNACs can be difficult due to the wide variety of lesion morphologies and overlapping features among different tumour subtypes. To address this challenge, the "Milan System for Reporting Salivary Gland Cytopathology" (MSRSGC) was introduced to aid in diagnosis, management, and to determine the risk of malignancy (ROM) across different categories. Our study aims to assess the cytological spectrum of salivary gland

Age-specific reference values for unstimulated whole saliva flow rate in primary Sjögren's syndrome: a cross-sectional study.

BACKGROUND: The clinical phenotype of primary Sjögren's syndrome (pSS) varies with age at onset, yet evidence on age and unstimulated whole saliva (UWS) flow rate in pSS is limited and conflicting. This study describes the age-specific distribution of UWS flow rate in pSS patients and provides reference values for clinical use. METHODS: This retrospective cross-sectional study included pSS patients diagnosed per 2016 ACR/EULAR criteria, stratified by age at diagnosis: early-onset (≤ 35 years), middle-onset (35-65 years), and elderly-onset (≥ 65 years). UWS flow rate was compared across groups
