[Indications of intraoperative ultrasound in head and neck surgery].

BACKGROUND: A possible use of B-scan sonography arises from the difficulty in transferring information by means of imaging to the intraoperative situation, which is now possible with navigation systems in complicated surgical procedures in the field of otolaryngology. A solution to this problem offers the intraoperative use of ultrasonography for orientation in soft tissue surgery. PATIENTS AND METHOD: A prospective study involving 115 patients in total entailed scanning with a small part linear and fingertip probe with either 10 and 7.5 MHZ. An ultrasound endoscope featuring a 7.5 MHZ convex

Parotidectomy: a 17-year institutional experience at a rural academic medical center.

OBJECTIVES: We describe the parotidectomy experience at a single rural institution over 17 years. METHODS: We retrieved the records of all patients who underwent parotidectomy for any nontrauma indication (current procedural terminology codes 42410, 42415, 42420, 42425, and 42426) from January 1990 to June 2007 at Dartmouth-Hitchcock Medical Center, a rural medical center that provides both primary and tertiary care to a local population of 14,000, with a catchment area of 600,000 people. Information was collected on initial patient presentation, use of fine-needle aspiration, extent of surge

Diagnostic difficulties of plunging ranula: case series.

OBJECTIVES: To evaluate common pitfalls in diagnosing complicated plunging ranula, either due to misidentification of plunging ranula or alternative pathology (i.e. false negatives or false positives, respectively). METHODS: A review of cases of plunging ranula seen in Middlemore Hospital, New Zealand, was performed. Diagnostically uncertain cases were identified and reviewed, taking particular note of clinical, radiological and surgical findings. RESULTS: From our database, 12 cases were found to have had a complicated diagnosis of plunging ranula. Ten cases were false negatives: four were

Dental infection simulating skin lesion.

Orocutaneous fistulas or cutaneous sinus, a tract of dental origin, is an uncommon but well-documented condition that usually requires emergency treatment. Such condition may be misdiagnosed by physicians and dentists and may sometimes be confused with bone and skin tumor, osteomyelitis, congenital fistula, salivary gland fistula, pyogenic granuloma, infected cyst, deep mycotic infection, and other pathologies. A case of facial sinus tract that was initially misdiagnosed by a physician as a nonodontogenic lesion is presented. Nonsurgical endodontic therapy was the treatment of choice for this

Diagnosis and treatment of branchial cleft anomalies in UKMMC: a 10-year retrospective study.

Branchial cleft anomalies result from abnormal persistence of branchial apparatus, which is located at the lateral part of the neck. These occur due to failure of obliteration of the branchial apparatus during embryonic development. Differential diagnoses of lateral neck mass are salivary gland or neurogenic neoplasms, paragangliomas, adenopathies, cystic hygroma or cystic metastasis from squamous cell carcinoma or thyroid papillary carcinoma. Clinically, a branchial cyst is smooth, round, fluctuant and non-tender, and usually occurs over the upper part of the neck, anterior to the sternocleid

Parotid gland abscess caused by Haemophilus paraphrophilus: a case report.

Human diseases caused by Haemophilus paraphrophilus (H.paraphrophilus) are unusual. The following case report describes a 67-year-old man who presented with pain and swelling of the right side of the face. Fine needle aspiration suggested a parotid gland abscess. Microbiological studies identified H.paraphrophilus. This is the first time a parotid abscess has been found to be caused by this organism.

Imaging of acute head and neck infections.

Tonsillar infection is the most common cause of infections of the neck in children and young adults whereas odontogenic infection is the most common cause in older population groups. Other sources of neck infection include the salivary glands, nasal sinuses, middle ear and mastoids, cervical lymph nodes, and trauma. Computed tomography and magnetic resonance imaging have excellent sensitivity for the recognition of deep infections, particularly for the identification of abscess formation and its precise location and extension of disease. A careful assessment of potential severe complications,

Oral infections and antibiotic therapy.

Oral infections commonly originate from an odontogenic source in adults and from tonsil and lymphatic sources in children. Odontogenic infections arise from advanced dental caries or periodontal disease. Oral trauma, radiation injury, chemotherapy mucositis, salivary gland infection, lymph node abscess, and postoperative infection are potential nonodontogenic sources of infections that could potentially be life threatening. This article reviews the serious nature and potential danger that exists from oral infection and the antibiotics available to treat them are reviewed. Successful treatment

Suppurative dacroadenitis causing ocular sicca syndrome in classic Wegener's granulomatosis.

Wegener's granulomatosis (WG) is a multisystem vasculitic disorder which can commonly afflict various components of the eye. Here we describe some unusual ocular manifestations of the disease in one patient. A young male with history of upper respiratory tract symptoms including epistaxis, nasal stuffiness and maxillary sinus pain presented with bilateral lacrimal gland abscess and ptosis. Lacrimal gland biopsy revealed granulomatous vasculitis. Lung cavities, positive cytoplasmic-antineutrophil cytoplasmic antibodies and high titers of serine proteinase-3 antibodies confirmed the diagnosis of

Bilateral parotid abscesses as the initial presentation of strongyloidiasis in the immunocompetent host.

BACKGROUND: Most suppurative parotid abscesses are unilateral and result from bacterial infection via Stensen's duct. Formation of bilateral parotid abscesses rarely occur, and cases arising in the setting of parasite infection have not been described to date. METHODS AND RESULTS: A 41-year-old man presented with bilateral parotid abscesses and underwent incision and drainage of right-sided swelling because of poor response to the aspiration drainage and intravenous antibiotic therapy. A nematode larva, morphologically consistent with Strongyloides stercoralis (S. stercoralis), was found in t

Metastatic breast carcinoma in the mandible presenting as a periodontal abscess: a case report.

INTRODUCTION: Tumors can metastasize to the oral cavity and affect the jaws, soft tissue and salivary glands. Oral cavity metastases are considered rare and represent approximately 1% of all oral malignancies. Because of their rarity and atypical clinical and radiographic appearance, metastatic lesions are considered a diagnostic challenge. The purpose of this report is to present a rare case of a metastatic breast carcinoma mimicking a periodontal abscess in the mandible. CASE PRESENTATION: A 55-year-old Caucasian woman was referred to our clinic for evaluation of bisphosphonate-induced jaw

A case of coexisting Warthin tumor and langerhans cell histiocytosis associated with necrosis, eosinophilic abscesses and a granulomatous reaction in intraparotid lymph nodes.

We present a patient (50-year-old male) with coexisting Warthin tumor and involvement of two intraparotid lymph nodes by Langerhans cell histiocytosis associated with necrosis, eosinophilic abscesses and a granulomatous reaction. This is the second documented case of this unusual combination of histological changes in nodal Langerhans cell histiocytosis and the first case involving intraparotid lymph nodes occurring together with an ipsilateral Warthin tumor.

Role of high resolution ultrasound in parotid lesions in children.

PURPOSE: A pictorial review of the spectrum of sonographic abnormalities of the parotid gland in children is presented. METHODS: Two pediatric radiologists performed retrospective review of medical records and imaging findings of all parotid ultrasounds performed in 298 children in the age range of 24 days to 16 years, over a five-year period. RESULTS: The lesions varied from diffuse glandular abnormalities to discrete solid and cystic lesions. Of the 298 studies reviewed, 148 (49.6%) were normal results, while 150 (50.4%) had abnormalities of parotid gland identified at ultrasonography. The

Sinus histiocytosis with massive lymphadenopathy Rosai-Dorfman disease: a unique case presentation.

Rosai and Dorfman first described sinus histiocytosis with massive lymphadenopathy (SHML) in 1969 with an article detailing 4 cases in which they differentiated this disease entity from the grouping of diseases categorized as histiocytosis X, where it was previously classified. Also known as Rosai-Dorfman disease (RDD), it is clinically characterized as massive, painless, bilateral, symmetric cervical lymphadenopathy, with fever and leukocytosis. An 11-year-old African American boy was referred to our clinic for extraction of a severely decayed tooth #30 and evaluation of a large right-sided n

The reliability of high resolution ultrasound in the identification of pus collections in head and neck swellings.

Head and neck swellings often present to oral and maxillofacial surgeons. It is often unclear whether a collection of pus is present. This information is essential for decisions regarding surgical intervention. Although ultrasound scanning (USS) is widely used to investigate the presence of a collection, the reliability and validity of this investigation in this context is uncertain. A retrospective review of 4000 consecutive head and neck USS reports over a 4 year period in the Cardiff University Health Board produced 43 cases in which ultrasound had been used to look for evidence of pus coll

Fine needle aspiration of salivary gland masses in HIV-infected patients.

Salivary gland disease is an important manifestation of HIV-infection. The aim of this study was to evaluate the cytologic findings of salivary gland fine needle aspiration (FNA) in South African human immunodeficiency virus (HIV)-infected patients. A retrospective review was performed on confirmed HIV-positive patients who underwent FNA of various body sites, including salivary glands, over a 5-year period. There were 495 (14.1%) salivary gland FNAs out of a total of 3,501 HIV-positive patients. This included 260 (52.5%) parotid, 226 (45.7%) submandibular, 2 (0.4%) sublingual, and 7 (1.4%) sp

[Ultrasound of foreign bodies in the head and neck. Cases compared with an in vitro model].

BACKGROUND: High resolution ultrasound is an accepted method in identification and classification of various head and neck alterations. Foreign bodies are a seldom encountered entity and are supposed to have a characteristic echogenicity. In literature there are numerous publications describing diagnosing foreign bodies in other specialities like gynaecology or surgery with ultrasound. The aim of the study was to examine these findings and to reproduce them in the head and neck area. MATERIALS AND METHODS: The study includes the retrospective analysis of 6 cases. 3 cases have been reconstruct
