Robot-Assisted Gasless Trans-Hairline Excision of Submandibular Gland Tumors.

Benign submandibular gland tumors are usually resected through a transcervical incision, which provides direct exposure but leaves visible cervical scarring. Remote-access endoscopic techniques improve scar concealment but can be limited by two-dimensional visualization, assistant-dependent camera control, and rigid instrument collision in the narrow submandibular gland space. This article presents a standardized video protocol for robot-assisted submandibular gland excision via a gasless trans-hairline approach for benign tumors. The procedure begins with postauricular trans-hairline incision

Salivary Cutaneous Fistula Caused by Submandibular Sialolithiasis in an Adolescent.

Pediatric submandibular sialolithiasis with a salivary cutaneous fistula is exceptionally rare. A 15-year-old boy presented with a 4-month history of a persistent right submandibular inflammatory lesion with a central opening and hypergranulation despite previous incision and drainage. Contrast-enhanced computed tomography demonstrated a 10.8 mm distal Wharton duct stone, a 34 mm inflammatory lesion extending toward the skin, and marked atrophy of the right submandibular gland. Because of persistent inflammation, fistula formation, and severe glandular atrophy, the fistulous tract and affected

The efficacy of modified sternocleidomastoid muscle flap in preventing sialocele and salivary fistula following partial superficial parotidectomy.

BACKGROUND: Sialocele and salivary fistula are well-established early postoperative complications following partial superficial parotidectomy (PSP). AIMS: This study aims to evaluate the effectiveness of the modified sternocleidomastoid muscle (MSCM) flap performed prophylactically following PSP to prevent sialocele and salivary fistula formation. METHODS: This retrospective cohort study included 230 patients who underwent PSP for benign parotid neoplasia at a tertiary referral center between 2012 and 2024. The primary independent variable was the surgical management (MSCM flap vs. no flap f

Seroma formation after parotidectomy: incidence, risk factors, and clinical implication.

BACKGROUND: Postoperative seroma is an underrecognized but clinically relevant complication after parotidectomy, leading to repeated aspirations and patient discomfort. Limited evidence exists on its incidence, risk factors, and clinical course. METHODS: We retrospectively reviewed 527 patients who underwent parotidectomy between 2020 and 2025. Demographics, tumor characteristics, surgical details, and postoperative variables were analyzed. Seroma was defined as fluid accumulation requiring aspiration without evidence of salivary fistula. Persistent seroma was defined as requiring multiple as

Sialendoscopy as treatment of face aesthetic surgery complications: technical note.

OBJECTIVE: The demand for cosmetic surgery on the face and body has increased worldwide. Brazil is currently the world leader in open plastic surgery and second on facelift, buccal fat removal, injectables (fillers), and thread lifts. The incidence of injuries to salivary glands, ducts, and facial nerves related to cosmetic procedures is uncommon but devastating. Treatment includes dressings, botulinum toxin, anticholinergics, drainages and salivary gland resection. Sialendoscopy has been successfully described for the treatment of ductal diseases with gland preservation. We present five cases

Endoscopic versus conventional open parotid gland surgery: a systematic review and meta-analysis.

Endoscopically-assisted parotid gland surgery has been increasingly adopted as a minimally invasive alternative to conventional open parotidectomy, aiming to improve perioperative outcomes and cosmetic results while maintaining surgical safety. This systematic review and meta-analysis synthesized contemporary comparative evidence on endoscopic versus conventional parotid gland surgery. A comprehensive search of PubMed/MEDLINE, Scopus, Web of Science, CENTRAL, and Google Scholar was conducted from inception to December 14, 2025. Observational and interventional studies comparing endoscopically-

Combined BTX-A and Collagen Membrane in Benign Parotid Enucleation: A Comparative Cohort Study.

Benign parotid tumors are increasingly treated with parenchyma-sparing extracapsular enucleation, yet postoperative salivary collections and Frey syndrome can still generate clinically relevant morbidity; we evaluated whether a standardized intraoperative bundle combining intraparotid botulinum toxin A (BTX-A) and bovine collagen membrane interposition is associated with fewer complications than standard enucleation alone. In this retrospective comparative cohort at a tertiary Head and Neck Surgery Unit, consecutive adults undergoing extracapsular enucleation for pleomorphic adenoma or Warthin

Prophylactic and therapeutic use of botulinum toxin in post-parotidectomy fistulas and sialocele: a scoping review.

INTRODUCTION: Sialocele and parotid fistula are complications following parotidectomy. Botulinum toxin type A has emerged as a less invasive alternative to prevent or treat them. This scoping review aimed to map the available evidence regarding the preventive and therapeutic use of botulinum toxin type A. METHODS: Studies involving patients with salivary gland fistulas or sialocele prevented or treated were included. A comprehensive search was conducted in PubMed, Scopus and Google Scholar. RESULTS: Thirty-five studies, including 681 patients, were analysed. Most were case reports or case se

Postoperative dietary management after parotid gland surgery in the absence of high-level evidence: consensus-based guidelines and a stepwise protocol for diet advancement.

BACKGROUND: Despite the ubiquity of dietary advice after parotid surgery, no structured or evidence graded guideline has previously addressed its rationale, timing, or impact on salivary complications. The assumption that "some form of diet is always needed" has persisted largely unchallenged, with heterogeneous habits replacing data driven recommendations. METHODS: An expert panel conducted a structured consensus process informed by systematic reviews of sialocele and salivary fistula prevention, contemporary parotidectomy series, and ERAS based nutritional frameworks for head and neck surge

An Odontogenic Cutaneous Sinus Tract Mimicking a Mandibular Neoplasm: Diagnostic Challenges and Radiologic Correlation.

Odontogenic cutaneous sinus tracts (OCSTs) are an uncommon consequence of chronic dental infection in which inflammatory drainage extends from a periapical source through bone and soft tissues to the skin surface. Although odontogenic infections typically present with intraoral symptoms such as pain, swelling, or vestibular drainage, chronic lesions may occasionally manifest as extraoral nodules, cysts, or draining sinuses. Because many patients experience little or no dental discomfort, these lesions are frequently misdiagnosed as dermatologic, infectious, or neoplastic conditions. The report

Iatrogenic Obstructive Sialadenitis of the Parotid and Submandibular Glands Following Facial Rejuvenation.

OBJECTIVE: During facial rejuvenation procedures, the salivary glands and their ducts are susceptible to injury. Given the paucity of data on obstructive salivary gland complications after facial rejuvenation procedures, this study aimed to characterize their clinical features and assess the outcomes of endoscopic management. METHODS: A tertiary salivary gland clinic database was retrospectively reviewed for patients who presented with obstructive sialadenitis involving the parotid and submandibular glands following facial rejuvenation procedures and were treated with sialendoscopy. [Pediatric Mumps Parotid Abscess with Fistula Formation: A Case Report].

INTRODUCTION: Mumps infection primarily leads to parotitis in children. While complications such as orchitis and encephalitis are uncommonly reported, the development of a parotid abscess due to mumps is extremely rare. We present the first described case of parotid abscess secondary to mumps, complicated by a persistent salivary fistula. BACKGROUND: A 21-month-old unvaccinated, healthy boy presented with right-sided cheek swelling lasting 15 days. Prior treatment at home with amoxicillin-clavulanate did not yield improvement. Sonographic evaluation revealed an enlarged right parotid gland wi

Diagnostic features and therapeutic management of accessory parotid gland tumours: a retrospective cohort analysis.

The accessory parotid gland (APG) is a common anatomical variant, situated adjacent to Stensen's duct and anatomically separate from the main body of the parotid gland. While often healthy, this tissue can give rise to neoplasms. This retrospective cohort analysis investigated the diagnostic features and therapeutic management of these rare tumours. The records of 647 patients with parotid gland disease treated at a single Maxillofacial Surgery Unit from 1988 to 2023 were analysed, selecting 17 patients with confirmed APG tumours for detailed analysis. The mean age of the included patients was

Lyell´s syndrome associated with the use of topical agents: case report.

UNLABELLED: Pseudomonas aeruginosa AG1 (PaeAG1) is a Costa Rican strain that was isolated in 2010 in a major Hospital. This strain has resistance to multiple antibiotics such as β-lactams (including carbapenems), aminoglycosides, and fluoroquinolones. PaeAG1 is considered critical (Priority 1) due to its resistance to carbapenems, and it was the first report of a P. aeruginosa isolate carrying both VIM-2 and IMP-18 genes encoding for metallo-β-lactamases (MBL) enzymes (both with carbapenemase activity). Owing to these traits, we have studied this model for 10 years using diverse approaches inc

Update: Prevention and Treatment of Parotid/Submandibular Gland Fistula/Sialocele Following Rhytidectomy With Botulinum Toxin A.

Dissecting deep to the superficial musculoaponeurotic system (SMAS), a popular option for facelifts, has the potential for injury to the parotid gland leading to postoperative sialoceles and fistulas. Similarly, deep plane procedures in the neck that include partial submandibular gland resection may lead to salivary gland leaks. We previously described the management of sialocele following rhytidectomy. Since then, administration of botulinum toxin A has emerged as the primary treatment and prevention of this rare complication. The technique of administration and dosage of botulinum toxin in t

Bilateral Orocutaneous Fistula Secondary to Pericoronal Infection of Mandibular Third Molars: A Rare Case Report.

Odontogenic cutaneous fistulas are abnormal connections between the oral cavity and skin, often mistaken for skin infections. They typically result from dental infections but may also arise from salivary gland issues, tumors, or congenital anomalies. Accurate diagnosis is essential to prevent complications like infection, osteomyelitis, and discomfort. A 26-year-old woman presented with bilateral facial lesions and itching for the past year. Her symptoms began 5 years earlier with cheek pain and swelling, initially treated with antibiotics. Examination revealed pericoronitis of the mandibular

Which is the fastest and safest approach? A comparison of time and complications of transparotid and anteroparotid retromandibular approach in management of mandibular subcondylar fractures: A prospective randomized controlled trial.

INTRODUCTION: Mandibular condylar fractures are among the most common facial fractures and its incidence has been rising since the past few decades. Although various approaches for management of mandibular condylar fractures have been described in literature there is a gap in the published literature when it comes to comparison of these approaches. There have only been a handful of studies which have compared use of Retromandibular Transparotid against Retromandibular anteroparotid approach in management of mandibular condylar fractures with conflicting results. The aim of this study was to ai
