Pectoralis major myocutaneous flap in head and neck reconstruction: An experience in 100 consecutive cases.

BACKGROUND: The pectoralis major myocutaneous (PMMC) flap has been used as a versatile and reliable flap since its first description by Ariyan in 1979. In India head and neck cancer patients usually present in the advanced stage making PMMC flap a viable option for reconstruction. Although free flap using microvascular technique is the standard of care, its use is limited by the availability of expertise and resources in developing world. The aim of this study is to identify the outcomes associated with PMMC flap reconstruction. PATIENTS AND METHODS: After ethical approval we retrospectively

Mycobacterium ulcerans disease in the middle belt of Ghana: An eight-year review from six endemic districts.

BACKGROUND: Mycobacterium ulcerans (MU) produces mycolactone toxin when infected with a plasmid. Toxin is cytotoxic and immunosuppressive, causing extensive destruction of tissues, leading to large ulcers on exposed parts of the body. Spontaneous healing by secondary intention leads to contractures, subluxation of joints, disuse atrophy, distal lymphedema and other complications. The disease is endemic in some communities within the middle belt of Ghana. OBJECTIVE: To document the clinical and epidemiological features of MU disease in the middle belt of Ghana and the outcome of treatment. PA

Large tonsillolith associated with the accessory duct of the ipsilateral submandibular gland: support for saliva stasis hypothesis.

Tonsillolith is a calcified mass in the tonsil and/or its surrounding tissue, which is considered to be caused by chronic tonsillitis. However, here we hypothesized that a tonsillolith can also be formed by chronic saliva stasis in the tonsillar tissue, without any signs of chronic inflammation. We present the case of a 32-year-old male patient with a large tonsillolith. We reviewed his medical files, pre-operative imaging and intraoperative findings. During a standard tonsillectomy, we encountered a large tonsillolith measuring 3.1 × 2.6 cm. Additionally, a careful dissection of the lower pol

Sialolithiasis of an accessory parotid gland: an unusual case.

We report a rare case of sialolithiasis of an accessory parotid gland, which was located anteromedial to the masseter muscle and isolated from the main parotid gland. The calculus developed from this accessory gland, and the main gland was free of lithiasis and inflammation. To our knowledge, there is no reported case of 14 stones in an accessory parotid salivary gland. The calculus was removed through a standard incision without injury to the facial nerve or a salivary fistula.

Submandibular Duct Fistula Caused by a Large Sialolith: Incidental Finding in a Barium Swallow Study.

BACKGROUND: This case report details an incidental finding of a submandibular duct fistula to a patient's posterior mouth floor found on a barium swallow examination that was performed to rule out gastrological causes of substernal chest pain. The radiologist was unable to determine the cause of the filling defect at the time of the study. The patient's history revealed that a large calculus of unknown size had been passed spontaneously through the floor of the mouth, rupturing the proximal gland duct and creating the fistula. DISCUSSION: Sialolithiasis is the formation of calculi in the sali

Bilateral spontaneous salivary otorrhoea: Case report and a review of the literature.

Spontaneous salivary otorrhoea is an extremely rare clinical entity. Most of the times, salivary otorrhoea results from various forms of trauma. It has also been attributed to the patent foramen of Huschke, and fissures of Santorini. Here, we present a rare case of an 8 year old child presenting with salivary discharge from both the ears. The diagnosis was established on the basis of biochemical and radiological investigations. The patient was managed by surgical exploration and ligation of the fistulous tract.

Does early oral feeding increase the likelihood of salivary fistula after total laryngectomy?

OBJECTIVE: This study compared the incidence of salivary fistula between groups with an early or late reintroduction of oral feeding, and identified the predictive factors for salivary fistula. METHODS: A randomised trial was performed using 89 patients with larynx or hypopharynx cancer, assigned to 2 groups (early or late). In the early group, oral feeding was started 24 hours after total laryngectomy or total pharyngolaryngectomy, and in the late group, it was started from post-operative day 7 onwards. The occurrence of salivary fistula was evaluated in relation to the following variables:

A technical note about flap fixation technique to prevent salivary fistulas in reconstructive oral cavity surgery.

Salivary fistulas are frequent complications in oral oncological reconstructive surgery by means of pedicled or free flaps. The most common risk factors are malnutrition, advanced disease, and healing alterations due to radiation therapy or infections. However, they can be observed also in healthy patients where the flap suture breakdown is the only cause. During the reconstructive phase, flaps are anchored to the remnant tongue, hyoid bone, and residual gingival mucosa; the last structure often does not offer suitable margins for a strong suture. The aim of this study was to propose a transma

Imaging of penetrating injuries of the head and neck:current practice at a level I trauma center in the United States.

Penetrating neck injuries are commonly related to stab wounds and gunshot wounds in the United States. The injuries are classified by penetration site in terms of the three anatomical zones of the neck. Based on this zonal classification system, penetrating injuries to the head and neck have traditionally been evaluated by conventional angiography and/or surgical exploration. In recent years, multidetector-row computed tomography (CT) angiography has significantly improved detectability of vascular injuries and extravascular injuries in the setting of penetrating injuries. CT angiography is a

Congenital parotid fistula.

Parotid fistula is a cause of great distress and embarrassment to the patient. Parotid fistula is most commonly a post-traumatic situation. Congenital parotid salivary fistulas are unusual entities that can arise from accessory parotid glands or even more infrequently, from normal parotid glands through an aberrant Stensen's duct. The treatment of fistulous tract is usually surgical and can be successfully excised after making a skin incision along the skin tension line around the fistula opening. This report describes a case of right accessory parotid gland fistula of a 4-year-old boy with di

[The primary use of pectoralis myofascial flap in salvage laryngectomy].

BACKGROUND: The salvage laryngectomy (SLE) is very often the only curative option in recurrent laryngeal or hypopharyngeal carcinomas. But the SLE is associated with an increased risk of complications such as the formation of salivary fistulas. To reduce the rate of fistulas a simultaneous elevation of the myofascial pectoralis major flap (PMML) is described. The aim of this study was to compare the SLE with and without the use of the PMML for prophylaxis of salivary fistulas. PATIENTS AND METHOD: 9 patients were included, suffering from a T4a larynx or hypopharynx carcinoma recurrence after

Delayed treatment of parotid sialocele: a functional approach and review.

The present report focused on 2 cases treated successfully with a delayed minimally invasive surgical approach and reviewed the available studies in which this method was used and their outcomes. The current data available suggest that the diversion of salivary flow into the oral cavity by cannulation of the distal portion of the parotid gland duct from the sialocele into the mouth, through the ostium or using a new controlled internal fistula creation, is a reliable, relatively simple, and effective procedure. It is available to the clinician when delayed treatment is needed, with low complic
