Epstein-Barr virus and oral health: epidemiology, pathogenesis, clinical aspects, and diagnostic insights: a narrative review.

Epstein-Barr virus (EBV), a widespread human herpesvirus, establishes lifelong latency and is linked to a range of systemic and oral diseases. Although primarily associated with infectious mononucleosis and lymphoproliferative disorders, its oral health implications are often overlooked. This review summarizes current insights into the epidemiology, pathogenesis, clinical aspects, and diagnostic methods of EBV in oral health. A comprehensive literature search using PubMed and Scopus databases identified studies focusing on EBV and its oral presentations, including keywords like "Epstein-Barr V

A scientometric study on research characteristics of HIV/AIDS involving the oral cavity with an implication for public health.

BACKGROUND/PURPOSE: Oral manifestations are often the first signs of human immunodeficiency virus (HIV) infection. The purpose of this study was to analyze the scientometric characteristics and research trends of HIV/AIDS involving the oral cavity. MATERIALS AND METHODS: All the papers on oral involvement of HIV/AIDS were comprehensively retrieved from the Scopus database. The years of publication were divided into before 2006 and 2006-2024 in the analysis of research trends. RESULTS: There were 1770 relevant papers on HIV/AIDS involving the oral cavity, with total citations of 26,307 and th

Prevalence of Oral Soft Tissue Lesions and Their Associations with Oral Hygiene Practices, Addictive Habits, and CD4 Counts Among Lesbian, Gay, Bisexual, Transgender, and Queer Individuals Living with Human Immunodeficiency Virus.

OBJECTIVES: Oral manifestations often reflect underlying immune suppression and disease progression. These lesions can profoundly affect the quality of life, leading to pain, difficulty in eating, and social stigma. In the lesbian, gay, bisexual, transgender, and queer (LGBTQ) population living with human immunodeficiency virus (HIV), additional sociobehavioral factors, such as discrimination, mental health challenges, and barriers to healthcare access, may exacerbate oral health issues. MATERIAL AND METHODS: This cross-sectional study aimed to evaluate the prevalence of oral soft tissue lesi

Clinical and behavioral determinants of oral lesions among people living with HIV/AIDS in West Java, Indonesia: a cross-sectional study.

BACKGROUND: Oral lesions remain common among people living with human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) and may serve as important clinical indicators of immune status and disease progression, particularly in resource-limited settings. However, data from Indonesia remains limited. This hospital-based cross-sectional study aimed to characterise the spectrum of oral lesions and identify clinical and behavioral determinants associated with their occurrence among people living with HIV/AIDS (PLWHA) attending a tertiary referral hospital in West Java, Indonesia.

Mucocutaneous manifestations as early indicators of systemic disease: a narrative review.

BACKGROUND: Mucocutaneous findings may provide important early clues to systemic disease. Because the skin, oral mucosa, eyes, and other visible mucosal surfaces are readily accessible during clinical examination, their careful assessment can contribute to earlier recognition of autoimmune, infectious, metabolic, endocrine, inflammatory, and malignant conditions. OBJECTIVE: This narrative review aims to summarize clinically relevant mucocutaneous manifestations that may act as early indicators of systemic disease, with emphasis on their diagnostic significance and practical relevance for clin

Iturin A disrupts Cryptococcus neoformans membrane integrity, attenuates virulence, and synergizes with amphotericin B.

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

A rare case of hairy leukoplakia in a young immunocompetent patient.

We present a case of oral hairy leukoplakia (OHL) in a male, 21-year-old immunocompetent patient. The patient had white, asymptomatic plaques located bilaterally on the tongue margin. The patient noticed the signs days after burning his tongue with hot food, which triggered the investigation. The case was treated initially as candidiasis and frictional keratosis. Exfoliative cytology indicated the presence of actinomycetes in the lesions, and a blood count suggested mild leukopenia (3,910/mm³; reference value 4,500 - 11,000/mm³). Photodynamic therapy and antiseptic mouthwash were used, with no

A cross-sectional study of oral health and disease prevalence in HIV-positive patients in Tabriz, Iran (2024).

HIV-positive patients often experience increased oral and dental health challenges due to compromised immunity, long-term antiretroviral therapy (ART), stigma, and discrimination. These factors can lead to reduced access to oral care, delayed treatment, and exacerbation of oral diseases, which may further contribute to disease transmission among close contacts and healthcare providers. This study aimed to assess the prevalence of oral health issues, evaluate the decayed, missing, and filled teeth (DMFT) index, and identify factors associated with poor oral health outcomes among HIV-positive pa

Oral Hairy Leukoplakia: A Rare Involvement in a Patient with Polycythemia Vera.

Although there have been some reports of oral hairy leukoplakia (OHL) in patients with hematologic neoplasms, to the best of our knowledge, this study is the first to report this lesion affecting a patient with polycythemia vera. A 54-year-old male patient diagnosed with polycythemia vera presented with non-removable white patches with a rough surface on the bilateral border of the tongue. According to clinical, histopathological and in situ hybridization features, OHL was established. Two months after diagnosis, the patient developed splenomegaly and initiated ruxolitinib. Bone marrow biopsy

Refractory Dermatophytosis and Oral Hairy Leukoplakia as Sentinel Manifestations of Advanced HIV Infection.

Dermatophytosis is usually a benign superficial fungal infection, but in immunocompromised individuals, particularly those with advanced HIV infection, it may present with extensive, recurrent, and treatment-resistant lesions. We report the case of a 28-year-old woman with widespread dermatophytosis involving the trunk, extremities, and genital region, accompanied by oral hairy leukoplakia (OHL) on the lateral tongue. The lesions were refractory to repeated topical antifungal therapies, leading to diagnostic delay. Mycological analysis identified Trichophyton mentagrophytes. Further evaluation

Chronic Hyperplastic Candidiasis-An Adverse Event of Secukinumab in the Oral Cavity: A Case Report and Literature Review.

Secukinumab (SEC) is a recombinant, fully human monoclonal antibody that is selective for interleukin-17A (IL-17A). SEC may increase the risk of developing infections such as oral herpes and oral candidiasis. The aim of this case report and literature review was to describe chronic hyperplastic candidiasis (CHC) in a patient with psoriasis (PsO) and psoriatic arthritis (PsA) treated with SEC. CHC is a rare and atypical clinical entity. A definitive diagnosis requires biopsy of the oral mucosa for histopathological diagnosis (PHD). The differential diagnosis includes hairy tongue, hairy leukopl

Case Series of HIV-Associated Oral Lesions Across Different Clinical Stages in People Living with HIV.

INTRODUCTION: Human immunodeficiency virus (HIV) impairs immune function leading to oral mucosal lesions. While highly active antiretroviral therapy (HAART) has reduced the incidence of HIV-associated oral lesions (HIV-OLs), these lesions can still manifest across all HIV stages due to various patient-related factors. PURPOSE: To evaluate the occurrence of HIV-OLs and clinical characteristics across all HIV stages in people living with HIV (PLWH). PATIENTS AND METHODS: Five patients aged 7 to 60 with canker sores visited the Oral Medicine Clinic. One newly diagnosed patient with stage II HIV

Prevalence of HIV in people with potential HIV-indicator conditions in Germany: an analysis of data from statutory health insurances.

BACKGROUND: In care of people living with human immunodeficiency virus (HIV), early diagnosis of infection is one of the greatest challenges remaining. A promising approach to increase early diagnosis could be optimized HIV testing in persons with indicator conditions (ICs). ICs are conditions which are AIDS-defining in people living with HIV, conditions that may have significant adverse consequences for the individual's clinical management if the presence of HIV infection is not detected, and conditions with an (undiagnosed) HIV prevalence of ≥0.1%. METHODS: In this cohort study, anonymous r

Common Tongue Conditions in Primary Care.

Tongue conditions occur in 15.5% of the US population. The most common tongue conditions are geographic tongue, fissured tongue, and black hairy tongue; these conditions do not require treatment. Median rhomboid glossitis can be associated with a candidal infection; symptomatic lesions usually improve with use of antifungals. Atrophic glossitis is often linked to a nutritional deficiency and resolves with treatment of the underlying condition. Oral hairy leukoplakia is caused by Epstein-Barr virus and most often presents in patients with severely compromised immunity; it can be treated with or

Oral hairy leukoplakia: The role of Epstein-Barr virus, the occurrence in immunocompetent patients, and the malignant transformation potential.

Oral hairy leukoplakia (OHL) was first reported by Greenspan et al., in 1984 and it was observed in a group of male homosexuals in the San Francisco. These lesions are predominantly found on the lateral borders of the tongue and show a corrugated or "hairy" surface. The Epstein-Barr virus (EBV) was then detected in these OHL lesions and proved to be involved in the development of the OHL. Besides, the OHL is also seen in people with immunosuppressive states. In this study, we focused on three different aspects of the OHL: (1) the necessity of presence of the EBV in the OHL lesions; (2) the OHL
