[Oral submucous fibrosis. Review of the literature apropos of a case].

The authors report a case of oral submucous fibrosis and present a review of the literature available on this subject. Oral submucous fibrosis is rare outside South-East Asia where it affects from 0.2 to 1 p. 100 of the population. A few cases have been described among Europeans. The disease seems to be specific to the oral and upper respiratory mucosae. It is characterized by the progressive development of subepithelial fibrosis and the subsequent occurrence of leukoplakia which may undergo transformation into epidermoid carcinoma. It initially presents as a non-specific stomatitis, sometimes

Oral submucous fibrosis--treatment with hyalase.

Oral Submucous Fibrosis (OSMF) is a disease of unknown etiology. A total of 96 patients with Oral Submucous Fibrosis have received four regimens of treatment--local dexamethasone, local hyaluronidase, local combination of dexamethasone and hyaluronidase, and local placental extract. The patients were followed up for a period varying from 3 months to 2 years. The group of patients receiving hyaluronidase alone showed quicker improvement in symptoms although its combination with dexamethasone gave somewhat better longer-term results. A new regimen for the treatment of submucous fibrosis is recom

Oral submucous fibrosis in a 4-year-old girl.

A rare case of oral submucous fibrosis in a 4-year-old Indian girl is presented. The condition is defined, and the clinical and laboratory features are discussed. Different treatment modalities and current theories of etiology are presented. Finally, the importance of regarding this condition as a precancerous lesion is supported.

The black layer on the teeth of betel chewers: a light microscopic, microradiographic and electronmicroscopic study.

Ten teeth of betel chewing P'wo Karen, a hilltribe group of northern Thailand, were available for this morphologic study. The structural composition of the black layer on these teeth was studied by light microscopy, microradiography and by electronmicroscopy. Principally, the black layer on the teeth of betel chewers was identical with subgingival calculus which had been colored by specific polyphenols contained in the betel nut. However, a pellicle-like layer forming initially on enamel crystals was also observed. From the morphologic aspect of the black layer a specific anticariogenic proper

HLA-typing in oral submucous fibrosis.

Oral submucous fibrosis (OSF) is a disease of the mouth and oropharynx characterised by progressive deposition of collagen leading to severe limitation of movement of the jaw in advanced cases. It is almost completely confined to inhabitants of, or migrants from India who chew 'betel nut'. The histopathological and clinical features suggest that autoimmune mechanisms may be involved. Because all chronic autoimmune diseases show disturbance in the frequencies of HLA antigens, we have HLA typed 50 OSF patients and a similar number of healthy subjects of the same ethnic origin. Raised frequencies

Ultrastructural findings of muscle degeneration in oral submucous fibrosis.

Muscle changes were investigated ultrastructurally in 2 groups of patients having oral submucous fibrosis. One group was from patients with no evidence of restricted mouth opening and the other was from patients with restricted mouth opening. Electronmicroscopically, the majority of muscle fibres taken from the first group appeared normal with only occasional muscle fibres showing accumulation of homogeneous material and compression of the sarcomeres closest to this material. In contrast, the tissues from patients with restricted mouth opening showed severe changes and necrosis in a high propo

Malignant transformation rate in oral submucous fibrosis over a 17-year period.

Sixty-six patients with oral submucous fibrosis were followed-up for a period of 17 yr (median observation 10 yr) in Ernakulam District, Kerala, India. Oral cancer developed in five (7.6%) patients. The malignant transformation rate in the same sample was 4.5% over a 15-yr observation period (median 8 yr). These findings impart a high degree of malignant potential to this condition.

Estimation of major immunoglobulin profile in oral submucous fibrosis by radial immunodiffusion.

Oral submucous fibrosis is defined as a collagen disease. The major immunoglobulin profile of 10 patients of oral submucous fibrosis (OSF) is estimated by radial immunodiffusion technique. The possible existence of autoimmune phenomena is expressed. It was observed in the present study that the severity of oral submucous fibrosis was directly proportional to the estimated elevated levels of major immunoglobulins. This may be taken as an indicator to know the gravity of this oral condition and also to evaluate the various related gammopathies and their possible management. It is anticipated tha

Oral submucous fibrosis: two cases of malignant transformation in Asian immigrants to the United Kingdom.

Three cases of oral submucous fibrosis are reported, two of which were associated with oral carcinoma. The disease is described and aspects of its clinical presentation, aetiology and treatment are discussed. It is argued that the condition is not as rare amongst Asian immigrants as the small number of reported cases might suggest and that early detection and follow up are important because of the risk of neoplasia.

Ultrastructural findings in the oral mucosa of betel chewers.

Eighteen biopsies of the oral mucosa of northern Thai hilltribe betel chewers were studied histologically and by transmission and scanning electron microscopy (TEM, SEM). Clinically, varying stages of epithelial atrophy and one case of submucous fibrosis were observed. Histologically, epithelial atrophy with marked reduction of the rete pegs, hyperortho- and/or parakeratosis, and subepithelial edema and inflammatory changes were the prominent findings. On the ultrastructural level, cytoplasmic projections of the basal cells into the subepithelial stroma were seen. The basal membrane frequently

Hypercoagulation and fibrinolysis in oral sub-mucous fibrosis.

In seven out of seven patients suffering from oral submucous fibrosis (OSMF), we detected circulating molecules that are immunologically similar to fibrinogen (MISFI), as suggested by the hemagglutination inhibition studies using the FDP Kit (Wellcome) and by paracoagulation tests such as serial dilution protamine tests. The ethanol gelation test was positive in three of the seven patients. Cryofibrinogen developed in six of the seven patients within three to six days of incubation of plasma samples at 4 degrees C. Plasma kept at 37 degrees C did not develop any fibrous or amorphous precipitat

Pilot survey of oral mucosa in areca (betel) nut chewers on Hainan Island of the People's Republic of China.

100 people on Hainan Island were studied for their smoking and chewing habits and the condition of their oral mucosa. Ninety-five percent of the study population chewed areca (betel) nut. In men this habit was supplemented with either smoking cigarettes or water pipe. In two men a small commissural leukoplakia was found. In three women clinical and histologic changes pointed towards oral submucous fibrosis.

Oral submucous fibrosis as a precancerous condition.

An evaluation of the malignant potential of submucous fibrosis, based on observations of 89 patients with the disease in Ernakulam District, Kerala, India was done. Out of the 89 patients, 12 were referred patients and 77 were diagnosed in population based studies. At the time of first examination oral cancer was found to be coexistent in nine (10%) cases. For 66 patients follow-up observations ranging from 4 to 15 yr (median 8 yr) were available. During this period malignant transformation was observed in three patients (4.5%). Considering malignant transformation and coexistence together, or

The aetiology of oral submucous fibrosis: the stimulation of collagen synthesis by extracts of areca nut.

Oral submucous fibrosis is a chronic disabling disease developing in up to 0.5% of the estimated 500 million habitual chewers of the "betel" quid. The quid, or chew, usually comprises a leaf of the Piper betel vine in which is wrapped fragments of the nut of Areca catechu, together with slaked lime and varied additives, including tobacco. The precise aetiology of oral submucous fibrosis (OSF) remains obscure, but epidemiological and animal studies have pointed to a close association with the prolonged usage of A. catechu nuts. Epithelial atypia and epidermoid carcinoma have been reported in 15
