Global expert consensus on oral submucous fibrosis: Standardizing care, surveillance, and prevention strategies.

BACKGROUND: Oral submucous fibrosis (OSF) is a chronic, progressive, and irreversible fibrotic disorder of the oral mucosa that is strongly associated with areca nut consumption. It is classified as an oral potentially malignant disorder (OPMD) with one of the highest malignant transformation rates (4%-7%). Despite extensive research across Asia and globally, significant heterogeneity persists in diagnostic criteria, grading systems, and management strategies. This hinders effective clinical practice, surveillance, and public health policymaking. By integrating translational evidence on areca

The multistep progression of areca nut-induced oral cancer: a mechanistic roadmap from pathogenesis to precision therapy.

Areca nut is classified as a Group 1 carcinogen by the International Agency for Research on Cancer. It is a widely consumed psychoactive substance with profound cultural roots in regions including Hunan, Hainan, and Taiwan of China. Its key bioactive components include alkaloids (e.g. arecoline and arecaidine) and areca nut-specific nitrosamines, that induce DNA damage, reactive oxygen species bursts, and chronic inflammation in oral tissues. Coupled with mechanical trauma from chewing, these insults drive the malignant progression of oral submucous fibrosis to oral cavity carcinomas. This rev

A review on angio-fibrotic pathways driving cancer potential in oral submucous fibrosis.

Oral Submucous Fibrosis (OSF) is a chronic, progressive, potentially malignant disorder associated with areca-nut exposure, characterized by persistent inflammation, extracellular matrix accumulation, and fibrotic restriction of oral function. This review highlights how fibrosis-driven vascular compromise and sustained hypoxia generate a tumor-promoting microenvironment that underlies malignant transformation. Dysregulated fibroblast activation, abnormal matrix remodeling, impaired angiogenesis, and chronic inflammatory signaling collectively promote stromal stiffness, oxidative stress, epithe

Comparative evaluation of diode laser fibrotomy versus intralesional dexamethasone plus hyaluronidase for moderate oral submucous fibrosis.

Oral submucous fibrosis (OSMF) represents a chronic, progressive, potentially malignant disorder lacking consensus on optimal management strategies for moderate clinical stages. Therefore, it is of interest to compare diode laser fibrotomy (Group A) versus intralesional dexamethasone sodium phosphate plus hyaluronidase (Group B) in 30 patients with moderate (Group II-III) OSMF. Mouth opening and cheek flexibility measurements at baseline, 1, 3 and 6 months revealed statistically significant intragroup improvements in both groups (p < 0.001). Intergroup analysis demonstrated significantly great

A Novel Fibrosis-Integrated Nomogram for Predicting Survival in Oral Squamous Cell Carcinoma Patients with Oral Submucous Fibrosis: A Retrospective Cohort Study.

BACKGROUND: Oral squamous cell carcinoma (OSCC) with oral submucous fibrosis (OSF) represents a distinct clinicopathological entity with unique prognostic factors not captured by conventional TNM staging. We aimed to develop a fibrosis-integrated nomogram (FIN) for individualized survival prediction in this population. METHODS: We conducted a retrospective cohort study of 314 consecutive OSCC patients with coexisting OSF treated between 2015 and 2022. Patients were randomly divided into training (n=220) and validation (n=94) sets. Independent prognostic factors were identified using Cox regre

A systematic review of staging systems for oral submucous fibrosis: evolution, variations, and clinical implications.

INTRODUCTION: Multiple clinical, functional, and histopathological staging frameworks for OSMF have been described in the literature. Staging helps clinicians assess prognosis, tailor interventions, and monitor progression. The aim of the study is to collect and analyze the various staging systems for OSMF reported in the literature to date and to identify how these systems have evolved over time. MATERIALS AND METHODS: A comprehensive search using multiple electronic databases and gray literature was undertaken to synthesize evidence on existing staging systems. Two reviewers independently p

A clinical study on the comparative effectiveness of micronutrient supplementation and physiotherapy in oral sub-mucous fibrosis.

Oral Sub-mucous Fibrosis (OSMF) is a chronic, progressive, potentially malignant disorder linked to areca nut chewing. Multiple therapies have been tried, but no consensus exists on the most effective approach. Therefore, it is of interest to compare the effectiveness of micronutrient supplementation and physiotherapy in managing OSMF. Patients with OSMF were randomly assigned to receive either micronutrient supplementation (Group A) or structured physiotherapy (Group B). Outcomes assessed included mouth opening and burning sensation at baseline and follow-up. Both groups showed symptomatic im

The silent trajectory: unaddressed adolescent arecanut chewing and early onset of oral submucous fibrosis.

INTRODUCTION: Arecanut chewing is common in many parts of South and Southeast Asia, but its effects on young people are often ignored. Many adolescents start this habit early, which is concerning because it is linked to early development of Oral Submucous Fibrosis-a condition that can cause serious problems and may progress to cancer. CONTENT: A structured review was done using PubMed/MEDLINE, Scopus, and Google Scholar, including English studies from 2000 to 2025. Studies were included if they reported arecanut use in adolescents, focusing on age of initiation and oral fibrotic changes. Stud

Delineating novel diagnostic biomarkers and therapeutic targets for oral submucosal fibrosis: an integrative multi-omics and machine learning approach.

BACKGROUND: Oral submucosal fibrosis (OSF) is a chronic and progressive disorder, caused by chewing areca nuts, affecting the oral cavity and oropharynx. OSF is characterized by severe symptoms like severe burning sensation, restricted mouth opening, etc. Given the multifactorial and poorly understood molecular basis of the disease, there is a need for novel biomarkers and therapeutic targets. METHOD: We downloaded 3 RNA-seq, two microarray, one epigenomic, and one single-cell RNA-seq datasets from the gene expression omnibus database. Differentially expressed genes (DEGs) were characterized

Targeting Pyroptosis in Oral and Maxillofacial Diseases.

The oral and maxillofacial region is a complex microenvironment continuously exposed to microbial challenge, mechanical stress, and oncogenic stimuli. Accumulating human evidence firmly implicates pyroptosis in the pathogenesis of major oral and maxillofacial diseases, with cleaved gasdermin D (GSDMD)/gasdermin E (GSDME) detected in clinical specimens of periodontitis, peri-implantitis, apical periodontitis, oral lichen planus, and oral submucous fibrosis. The central mechanism involves inflammasome assembly (eg, NLRP3), leading to caspase activation, gasdermin cleavage (eg, GSDMD, GSDME), and

Advanced therapeutic strategies and drug delivery approaches for management of oral submucosal fibrosis: a comprehensive review.

Oral Submucous Fibrosis (OSF) is a chronic, progressive, and potentially malignant oral mucosal disorder that leads to development of trismus, burning sensations, mucosal blanching, and impaired oral function. Traditional treatments of OSF, including corticosteroids, hyaluronidase, antioxidants, and surgery, are limited due to poor drug delivery, systemic toxicity, and patient non-compliance. In this current scenario, the development of an advanced drug delivery system can be a potential strategy for OSF therapy, and emerging drug delivery strategies offer promising solutions by enabling local
