Treatment of Epulis Fissuratum Using an In Situ Harvested Free Gingival Graft: A Case Series and Literature Review.

Epulis fissuratum (EF) is a reactive fibrous overgrowth in the oral vestibule caused by chronic denture irritation. We describe two patients (aged 58 and 42 years) in whom EF was excised and vestibuloplasty performed using a free gingival graft (FGG) harvested from the excised lesion itself. In Case 1, a hyperplastic fold of the maxillary anterior alveolar mucosa was removed, the incision apically repositioned, and the thinned, deepithelialized graft immobilized over the periosteum. In Case 2, peri-implant hyperplastic tissue was excised and the graft, prepared using the accordion technique, w

Topical Oxygen Therapy and a Zinc-Containing Palatal Stent for Post-surgical Healing: A Case Report of a Large Irritation Fibroma of the Hard Palate.

Irritation fibroma is a common benign reactive lesion of the oral mucosa, typically resulting from chronic mechanical irritation. While generally small and asymptomatic, rare cases may exhibit atypical growth. This report presents a 37-year-old male patient with a 2.7 cm fibroma located on the hard palate. Complete surgical excision was performed, and histopathological examination confirmed the diagnosis of irritation fibroma with no evidence of dysplasia or malignancy. Postoperative management included the use of a chairside-fabricated zinc-infused palatal stent, combined with topical oxygen-

Unusual denture-associated oral squamous papilloma with koilocytosis: a case report and literature review.

BACKGROUND: Oral squamous papilloma (OSP) is a benign epithelial proliferation commonly linked to human papillomavirus (HPV) and usually affects the palate, tongue and lips. Anterior mandibular alveolar mucosa involvement is uncommon, and overlap between koilocytic HPV-suggestive change and chronic denture-related trauma is rarely documented. A review of the literature indicates limited documentation of such overlapping clinicopathological features at this anatomical site. CASE PRESENTATION: A 49-year-old non-smoking male with absence of alcohol consumption history, presented with a 12-month

Reactionary Bone Changes in Long-Standing Pyogenic Granuloma: A Case Report.

Pyogenic granuloma is a reactive lesion that is commonly seen in the skin and oral cavity. Though it is an unfortunate misnomer, being neither pyogenic nor a true granuloma, the name has been used for years. In the oral cavity, it presents as a growth mostly situated on the gingiva, but may also occur on the tongue, buccal and labial mucosa, and palate, and may even be seen in relation to dental implants. The lesion is usually bright or purple red in color, soft in consistency, relatively painless, and appears highly vascularized. Local etiologic factors are usually poor oral hygiene or chroni

A Large Irritation Fibroma of the Hard Palate: A Rare Clinical Entity.

An irritative fibroma of the oral cavity can be defined as a benign tumor of connective tissue. They usually occur in the oral cavity, with the most common sites being the buccal mucosa and tongue. However, reported cases over the hard palate are few. Irritant or reactive fibromas are brought upon by recurrent, mildly intense stimulation of the oral mucosa. This can be because of repeated tobacco chewing, ill-fitted dentures, intentional or unintentional biting, sharp teeth, and so on. Because, clinically, fibromas resemble the features of other benign or reactive tumors, histological examinat

Multifocal epithelial hyperplasia: an understudied infectious disease affecting ethnic groups. A mini review.

Focal Epithelial Hyperplasia or Multifocal Epithelial Hyperplasia (MEH), also known as Heck's disease, is considered a rare pathology of the oral mucosa associated with human papillomavirus types 13 and 32. For reasons not fully understood, MEH disproportionally affects specific populations of indigenous groups around the world. After the first reports in Native Americans, the epidemiology of the disease has been described in different geographical regions mainly related to particular indigenous populations, the majority of the studies are clinical case reports, but the biological determinants

Periodontal changes induced by fixed orthodontic therapy.

BACKGROUND: Orthodontic therapy is a complex process involving a series of specialists in the dental field: the oral-maxillo-facial surgeon, the periodontologist, sometimes even the prosthetist, implantologist, or general practitioners. The injuries of the oral mucosa induced by orthodontic therapy include gingival overgrowths, traumatic lesions of the oral mucosa, different degrees of periodontal damage manifested by gingival retraction, alveolar bone resorption. METHODS: From a total of 327 subjects who came to the Dental Clinic in Craiova 74 subjects were selected, who presented with gingi

LONEStar: The Lunar Flashlight Optical Navigation Experiment.

Grapefruit juice can alter oral drug pharmacokinetics by different mechanisms. Irreversible inactivation of intestinal cytochrome P450 (CYP) 3A4 is produced by commercial grapefruit juice given as a single normal amount (e.g. 200-300 mL) or by whole fresh fruit segments. As a result, presystemic metabolism is reduced and oral drug bioavailability increased. Enhanced oral drug bioavailability can occur 24 hours after juice consumption. Inhibition of P-glycoprotein (P-gp) is a possible mechanism that increases oral drug bioavailability by reducing intestinal and/or hepatic efflux transport. Rece

Conventional Scalpel and Diode Laser Approach for the Management of Traumatic Fibroma.

An oral fibroma is a benign scar-like reaction frequently resulting from chronic mouth irritation. It is also called an oral polyp, fibrous nodule, localised intraoral fibrous hyperplasia, and traumatic fibroma. Chronic irritation from things like biting one's lips or cheek, orthodontic treatments, rubbing against a hard tooth, or wearing dentures or other dental prostheses is frequently the cause. It is often the same colour as the surrounding mouth lining, but occasionally, it can be paler or appear darker if it has bled. Trauma can cause the surface to become rough and scaly or ulcerated. I

Dental MRI of Oral Soft-Tissue Tumors-Optimized Use of Black Bone MRI Sequences and a 15-Channel Mandibular Coil.

Soft-tissue lesions in the oral cavity, one of the most common sites for tumors and tumor-like lesions, can be challenging to diagnose and treat due to the wide spectrum from benign indolent to invasive malignant lesions. We report an abnormally large, rapidly growing hyperplastic lesion originating from the buccal mucosa in a 28-year-old male patient. Clinical examination revealed a well-circumscribed, smooth-surfaced, pinkish nodular lesion measuring 2.3 × 2 cm, which suggested the differential diagnosis of irritation fibroma, pyogenic granuloma, oral lipoma, and other benign or malignant ne

Oral fibroepithelial polyps ("chewing granulomas") in 21 dogs: Histomorphology, immunohistochemical characterization, and clinical outcome.

Oral fibroepithelial polyps (FEPs) are common, benign, nonneoplastic lesions in humans that often develop slowly in sites of local irritation or trauma. This study analyzed 23 oral fibroepithelial polypoid lesions retrieved from 21 dogs (2014-2021). All lesions were pedunculated with usually an irregular/cauliflower-like or rarely smooth surface. FEPs most commonly arose under or lateral to the tongue; other sites included the labial and gingival mucosa, soft palate, and hard palate. All the lesions were characterized by a thick fibrovascular stalk consisting of bundles of fibrocytes and fibro

Quantification of inflammatory, angiogenic, and fibrous components of reactive oral lesions with an insight into the pathogenesis.

BACKGROUND: Reactive oral lesions pose diagnostic difficulties as they mimic each other clinically. A definitive diagnosis is made based on the histopathological presentation of this group of lesion. Stromal microenvironment is the key to the sequence of the stages of these lesions. Stringent quantification of each component of the stroma is important to understand the pathogenesis. The aim is to evaluate inflammation, angiogenesis, and fibrosis in the reactive group of lesions through quantitative analysis. MATERIALS AND METHODS: Blocks of irritation fibroma, inflammatory fibrous hyperplasia

Electro-surgical management of a traumatic fibroma with clinical assessment of re-epithelialization using methylene blue assay: A clinico-histopathological case report.

This article addresses the clinical presentation, diagnosis, histological features, and management of a case of fibroma. Fibroma is proliferative fibrotic lesions of the gingiva and oral mucosa, which may cause aesthetic and functional problems. Fibrous hyperplasia and fibroepithelial hyperplasia are histological alternates of these nonneoplastic lesions. The case demonstrated an overgrowth in the lower back tooth region. The growth was pale in color, sessile, painless, and firm in consistency. The lesion caused discomfort when chewing food, the lesion was excised using dental electrocautery a

Large irritation fibroma of hard palate: a case report of a rare clinical entity.

Fibromas are benign tumors of connective tissue common in the oral cavity but rare on hard palate. This paper reports on an asymptomatic, slowly growing mass on the hard palate of a 90-year-old lady, with a reported use of denture for two decades. The patient presented with a 2.2cm, smooth-surfaced, well-circumscribed nodule attached with a stalk to the palatal mucosa. After excision, the histopathological examination revealed a mass of fibrous connective tissue, covered by stratified squamous epithelium with focal low-medium grade hyperplasia and hyperkeratosis. These findings were consistent

Laser-Assisted Management of a Rare Presentation of Peripheral Ossifying Fibroma in an Infant.

Reactive hyperplasias are a group of lesions often seen in the oral mucosa, especially on the gingiva, in association with local irritation or trauma. Peripheral ossifying fibroma (POF) is a common reactive lesion, almost always affecting the tooth-bearing areas of the oral cavity. It is most often encountered in young adults but extremely rare in patients below 10 years of age. Here, we report a unique presentation of peripheral ossifying fibroma affecting the anterior mandible in a three-month-old infant. We also highlight the role of laser in the management of such lesions.

Relative frequency of oral focal reactive overgrowths: An institutional retrospective study.

CONTEXT: Focal fibrous hyperplasia, peripheral ossifying fibroma, pyogenic granuloma, peripheral giant cell granuloma, giant cell fibroma and focal reactive overgrowth (FROGs) are one of the foremost numerous benign soft-tissue growths in the oral cavity. Chronic irritation or trauma is with identified as the causative aspect. It may develop up to few centimeters in diameter, pedunculated or sessile and may arise on the gingiva or buccal mucosa. Treatment involves surgical excision, and recurrences are infrequent. AIMS: The aim of this study was to assess the prevalence of FROGs of oral mucos

Oral epithelial reactive atypia/dysplasia: An underestimated true atypia/dysplasia?

Several clinical and histological features, usually associated with chronic inflammation could complexify the diagnosis of oral epithelial dysplasia (OED). These changes in response to inflammatory stimuli, or re-epithelialization events, are described as reactive epithelial atypia or dysplasia (REA/D). Within a REA/D scenario (for example in the edges of chronic traumatic ulcers), the diagnosis of OED could be challenging for oral pathologists due to an unfeasibility to accurate the true nature of that changes. Due to a reactive profile, REA/D suggests an evolutionary pattern of reversibility

Oral submucous fibrosis: An enigmatic morpho-insight.

Oral submucous fibrosis (OSMF) is a chronic progressive, scarring disease affecting oral, oropharyngeal, and sometimes the esophageal mucosa. It is characterized by the progressive fibrosis of the submucosal tissue. The pathogenesis of OSMF has been directly related to the habit of chewing areca nut and its commercial preparation, which is widespread in Indian subcontinent and Southeast Asia. The areca nut has been classified as a "group one human carcinogen." Oral squamous cell carcinoma in the background of OSMF is one of the most common malignancies in South and Southeast Asian countries. M

A Clinical Report of Solitary Gingival Overgrowth in a Young Female Patient.

Pyogenic granuloma is a type of inflammatory hyperplasia. The term "inflammatory hyperplasia" is used to describe nodular growths of oral mucosa that histologically contain inflamed fibrous and granulation tissues. It is nonneoplastic. It is predominant in second decade of life in young adult females. Oral pyogenic granuloma is the most common gingival tumor with a striking predilection to gingiva. About one-third of the lesions occur following trauma. Poor oral hygiene may be a precipitating factor as 75% of all cases show calculus or foreign material in gingival sulcus. Lesions are more comm
