DCNN models with post-hoc interpretability for the automated detection of glossitis and OSCC on the tongue.

This study aimed to develop and evaluate deep convolutional neural network (DCNN) models with Grad-CAM visualization for the automated classification with interpretability of tongue conditions-specifically glossitis and oral squamous cell carcinoma (OSCC)-using clinical tongue photographs, with a focus on their potential for early detection and telemedicine-based diagnostics. A total of 652 tongue images were categorized into normal control (n = 294), glossitis (n = 340), and OSCC (n = 17). Four pretrained DCNN architectures (VGG16, VGG19, ResNet50, ResNet152) were fine-tuned using transfer le

Subacute Combined Degeneration From Isolated Folate Deficiency Secondary to Nutrition and Co-Trimoxazole Use in a Patient With IgG4-Related Disease.

Subacute combined degeneration (SCD) is characterized by demyelination primarily affecting the dorsal column and lateral corticospinal tracts. It typically presents with paresthesia in the feet and progresses to involve the upper extremities. SCD is commonly associated with nutritional deficiencies, particularly deficiencies of vitamin B12, folate, or copper. Neurological disorders solely caused by isolated folate deficiency are uncommon. Classic presentations of folate deficiency include fatigue, lethargy, glossitis, diarrhea, and the gradual development of megaloblastic anemia. Early SCD dia

Questionnaire Survey on Enterococcus faecalis-2001 Supplementation in 137 Patients With Oral Diseases at Two Facilities.

OBJECTIVE: Lactic acid bacteria are gaining attention not only for their role in regulating the intestinal flora but also for their immunostimulatory effects. Recent clinical trials and in vitro studies have demonstrated the efficacy of heat-killed Enterococcus faecalis 2001 (EF-2001) in treating oral candidiasis, suggesting its potential to improve oral microbial flora. However, its effectiveness in other oral conditions remains unclear. This study aimed to evaluate the clinical utility of EF-2001 in a broader range of oral diseases. METHODS: A prospective observational questionnaire-based s

Investigating geographic tongue severity: a cross-sectional study on symptoms and potential risk factors.

BACKGROUND: Geographic tongue (GT) is a benign, recurrent inflammatory condition marked by atrophic and erythematous lesions, often asymptomatic but sometimes causing discomfort. This study evaluated some potential etiological and risk factors influencing Geographic Tongue Area and Severity Index (GTASI) and the relationship between GTASI scores and symptoms. METHODS: A total of 163 GT patients (100 females [61.35%], 63 males [38.65%]) were assessed using the GTASI, which incorporates erythema, white halo, and fissured tongue (FT) scores. Statistical analyses investigated relationships betwee

Buccodental Toxicities Induced by Tyrosine Kinase Inhibitors in Patients Diagnosed with Renal Cell Carcinoma-A Literature Review.

Tyrosine kinase inhibitors (TKIs), either as single agents or in combination with other drugs, have become a gold standard in many oncological pathologies. The identification, analysis, and clinical management of their multiple and various systemic adverse events are a clear requirement and represent a true challenge in daily practice. For this narrative review, registration clinical trials that have led to the approval of certain TKI protocols in the setting of renal cell carcinoma (RCC) were identified via the latest version of the National Comprehensive Cancer Network (NCCN) kidney cancer g

From Image to Diagnosis: Convolutional Neural Networks in Tongue Lesions.

Clinical examination of the tongue is essential for diagnosing systemic and local diseases. However, traditional diagnostic methods rely on subjective evaluation. Artificial intelligence, particularly convolutional neural networks, has shown promise in enhancing diagnostic accuracy in medical imaging. This study aimed to classify common tongue lesions using convolutional neural networks, improving diagnostic precision in routine dental examinations. A dataset of 1038 tongue images was analyzed, categorized into six classes: healthy, coated, fissured, hairy, geographic, and median rhomboid glos

Disproportionality analysis comparing safety profiles of sonidegib and vismodegib based on the FAERS database.

This study aims to comprehensively evaluate the long-term safety profiles of hedgehog pathway inhibitors (HPIs), sonidegib and vismodegib, which have been approved for basal cell carcinoma but lack extensive real-world safety data. Using the FDA Adverse Event Reporting System (FAERS) data from 2012 to 2024, we analyzed adverse events (AEs) associated with these drugs. Notably, most reports were from North America and Europe, which may result in limited generalizability to other populations. Four disproportionality analysis algorithms (ROR, PRR, BCPNN, MGPS) were employed to identify significan

Impact of gastrointestinal tract diseases on the oral cavity manifestations.

Gastrointestinal diseases often manifest in the oral cavity, sometimes preceding systemic symptoms and serving as early indicators of underlying conditions. This review explores the oral manifestations associated with various gastrointestinal disorders, emphasizing their diagnostic significance and the importance of interdisciplinary collaboration. Diseases such as Crohn's disease, ulcerative colitis, celiac disease, gastritis, peptic ulcer disease, gastroesophageal reflux disease (GERD), and rare syndromes like Peutz-Jeghers, Gardner's, Cowden, and Plummer-Vinson are discussed in terms of the

Oral lesions in patients with systemic diseases: A hospital-based observational study.

Systemic diseases frequently manifest through characteristic oral lesions, making the oral cavity an important indicator of underlying medical conditions. Therefore, it is of interest to assess the prevalence and patterns of oral lesions among patients with confirmed systemic diseases in a hospital-based population. Hence, a cross-sectional observational analysis of 500 adults was conducted, documenting demographic data, systemic diagnoses and clinically verified oral findings, which were evaluated using WHO criteria and analyzed statistically. The results demonstrated high frequencies of peri

When Hashimoto's Thyroiditis Masks Biermer's Disease: A Revealing Case of Autoimmune Polyendocrinopathy.

Biermer's disease is an autoimmune disorder characterized by atrophic gastritis leading to vitamin B12 deficiency, often revealed late due to non-specific symptoms. It may be associated with other autoimmune pathologies, notably Hashimoto's thyroiditis, as part of a type 3b autoimmune polyendocrinopathy (AIP). We report the case of a 44-year-old female patient, followed for autoimmune hypothyroidism, in whom vitamin B12 deficiency was detected. The patient showed no suggestive signs such as glossitis, paresthesia, or other neurological abnormalities. Investigations confirmed Biermer's disease,

Pellagra in Complex Clinical Settings: A Case Involving Bariatric Surgery, Whipple Procedure, and Alcohol Use Disorder.

Pellagra is a disease of niacin (vitamin B3) deficiency and is classically characterized by the triad of diarrhea, dermatitis, and dementia, and has the potential to cause death. Pellagra can be caused by either insufficient dietary intake or dysfunctional utilization of niacin. Because niacin can be found in nearly every food group (meat, dairy, vegetables, grains, etc.), deficiency is rarely seen in resource-rich countries where food scarcity is less prevalent. In this case, a 50-year-old woman presented to the emergency department with abdominal pain, watery diarrhea, altered mental status,

Mannheimia granulomatis-Infected Pyogranulomatous Oral Lesions with Concurrent Amyloidosis in Roe Deer (Capreolus capreolus) in France.

Mannheimia granulomatis-associated lesions have been poorly described compared with those associated with other members of the Pasteurellaceae family; they include pneumonia, panniculitis, and keratoconjunctivitis in domestic and wild ruminants and hares. We describe 15 cases of roe deer (Capreolus capreolus) showing M. granulomatis-infected oral lesions that were detected in France between 2016 and 2023. Comprehensive necropsies were performed, with additional analyses based on findings. All deer were >15 mo of age on the basis of tooth eruption. They were thin (1/15), cachectic (13/15), or o
