Advances in tuberculosis diagnostics and biomarkers, 2020-26: scientific evidence, recommendations, gaps, and developmental priorities.

Major advances in tuberculosis diagnostics and biomarker research from 2020 to 2025 have informed updated WHO policy guidance, consolidating recommendations for the detection of tuberculosis disease and drug resistance within a single framework. These recommendations align screening, diagnosis, and drug-resistance testing across populations and specimen types. Digital chest radiography, increasingly supported by computer-aided detection, is now a core WHO-recommended screening and triage tool, improving case detection and referral for confirmatory testing at scale. Decentralised molecular diag

Diagnostic testing for pulmonary tuberculosis in low and middle-income countries: innovations to achieve universal access-a narrative review.

UNLABELLED: Though rapid molecular diagnostics for tuberculosis (TB) have substantially improved diagnostic accuracy and turnaround times, implementation and impact has been constrained by cost, infrastructure requirements, and continued reliance on sputum. This review examines advances in diagnostic technologies and sampling strategies that could overcome these barriers and accelerate universal access to TB diagnostics. We conducted a narrative review of studies published between Jan 1, 2020, and April 30, 2026, focusing on near-point-of-care molecular (NPOC) platforms, next-generation lipoar

Differential associations between treatment delay and survival across head and neck cancer subsites: a national hospital-based framework for risk-stratified prioritization.

PURPOSE: Delays in time to treatment initiation (TTI) have been associated with inferior outcomes in head and neck cancer, yet prior studies have typically evaluated tumors collectively, assuming uniform time sensitivity across diseases. This study quantified subsite- and stage-specific associations between TTI and overall survival (OS) and derived groups of tumors with similar TTI-mortality associations. METHODS: We conducted a retrospective cohort study using the National Cancer Database (2010-2017), including adults with head and neck malignancies treated with curative intent. Multivariabl

Basaloid Squamous Cell Carcinoma of the Midline Dorsal Tongue: Case Report and Review of the Literature.

BACKGROUND: Squamous cell carcinoma (SCC) of the dorsal tongue accounts for only 2%-5% of tongue cancers; midline dorsal tongue cancers account for less than 1% of these tumors. Basaloid SCC (BSCC) is usually associated with more aggressive features than conventional SCC. There are fewer than 30 reported cases of lingual BSCC. METHODS: A 76-year-old woman presented with irregular, exophytic, rugated, and furrowed changes of the midline dorsal tongue, which was initially misdiagnosed as mucoepidermoid carcinoma on an outside institution biopsy. Other than mild linear enhancement of the orophar

Lamellodiscus louiseuzeti n. sp. (Monopisthocotyla: Diplectanidae: Lamellodiscinae): a parasite of Chaetodon hoefleri (Teleostei, Chaetodontidae) with distinct attachment organ morphology.

During a study on representatives of Diplectanidae (Monopisthocotyla) infecting Chaetodon hoefleri, a marine teleost from Senegal, we collected individuals belonging to a species of Lamellodiscinae, which showed a unique arrangement of the haptoral lamellodiscs, so far unreported in the literature, with the first lamella of the ventral lamellodisc open anteriorly, unlike the dorsal lamellodisc, which is closed. Due to this arrangement, we describe the species Lamellodiscus louiseuzeti n. sp.. Apart from the morphology of the lamellodiscs, the new species differs from its congeners by having a

Proteomic investigation of signaling dynamics: from static maps to network rewiring.

PURPOSE: This study focused on the mechanism underlying the therapeutic effect of curcumin against tongue cancer (TC). METHODS: Target genes of TC and curcumin were identified, respectively. Three datasets of TC from Gene Expression Omnibus were included, and then the differentially expressed genes were collected. After combing the data from The Cancer Genome Atlas, bioinformatics analyses were performed to investigate hub genes in terms of the functions and correlations. The proliferation and migration of TC cells were evaluated with CCK-8 assay and scratch wound healing assay, respectively.

Mutant SOD1 expressed by oligodendrocytes aggregates in myelinic nanochannels and accelerates disease progression in familial ALS mice.

UNLABELLED: Amyotrophic lateral sclerosis (ALS) is a highly debilitating and fatal disease characterized by the progressive loss of motor neurons. Reduced oligodendroglial support has been implicated in ALS progression but remains mechanistically unexplained. Here, using a mutant superoxide dismutase 1 (SOD1-G37R) mouse model of familial ALS, Cre-mediated excision of the mutant SOD1 gene within the oligodendrocyte lineage prior to myelin compaction is shown to slow disease onset, improve motor performance, and prolong survival. In contrast, silencing mutant SOD1 expression within oligodendrocy

Navigating the healthcare systems for primary caregivers of children with developmental disabilities: experiences from a rural setting in Kenya.

INTRODUCTION: Families with children who have developmental disabilities in Sub-Saharan Africa often face significant challenges in accessing healthcare services and are more than three times more likely to encounter difficulties in obtaining necessary care. Despite global advancements in disability-related policies, caregivers in low-resource settings continue to face obstacles when navigating the healthcare system. This article examines how primary caregivers of children with developmental disabilities navigate healthcare systems in rural Kenya. METHODS: A phenomenological qualitative appro

Sulfur metabolism and immune-microbial networks across oral niches in periodontal disease.

Volatile sulfur compounds (VSCs) integrate microbial metabolism with local inflammation in periodontal disease. We profiled five oral niches (saliva, tongue, subgingival, supragingival, and interdental plaque) across clinical health, gingivitis, and periodontitis by combining direct VSC measurements (subgingival and oral headspace H2S/CH3SH), functional cysteine/methionine degradation assays, 16S rRNA profiling, gingival crevicular fluid cytokines, and targeted qPCR. Subgingival H2S concentrations were significantly elevated in periodontitis after adjusting for age, sex, plaque index, and subg

The Prognostic Significance of Elective Level 4 Neck Dissection in Oral Tongue Cancer.

INTRODUCTION: Elective neck dissection (END) for oral tongue squamous cell carcinoma (OTSCC) typically involves level 4 due to potential metastases that may bypass levels 1-2. Our study challenges this notion and investigates the necessity of level 4 inclusion in END for OTSCC. METHODS: A retrospective cohort study performed in a tertiary-care university affiliated medical center and included all OTSCC patients treated with END from 2000 to 2020, with a minimum 2-year follow-up. The study compared patients with END levels 1-3 to those with levels 1-4 regarding regional recurrence, disease-spe

Bronchogenic Cyst of the Tongue: Congenital Tongue Masses.

Bronchogenic cysts, a subgroup of foregut duplication cysts, are rare congenital malformations of ventral foregut development. These cysts can be found ectopically along the entire developmental pathway of the foregut. Less than 1% of bronchogenic cysts are found in the oral cavity, with fewer than 10 cases previously reported in the literature. These lesions typically present asymptomatically shortly after birth but have the potential to cause respiratory and/or feeding difficulties. Treatment is surgical excision, and histopathology is ultimately required to reach a definitive diagnosis and

Lingual metastasis from primary pulmonary adenoid cystic carcinoma: A rare case report.

Adenoid cystic carcinoma (AdCC) is a malignancy that most frequently originates in the salivary glands of the head and neck, though it may also rarely occur in other sites such as the trachea or breast. It is characterized by its slow-growing nature in the early stages, along with distinctive features of perineural invasion and a tendency for late metastasis, particularly to the lungs. Recurrence or metastasis can occur even years after the initial treatment. Currently, no standardized clinical protocol exists for long-term follow-up. The disease can recur or metastasize even years after initi

[Motor neuron diseases from a radiological perspective : Focus on amyotrophic lateral sclerosis].

BACKGROUND: Motor neuron diseases (MND) affect the upper and/or lower motor neurons. Radiological diagnostics primarily serve to systematically exclude treatable mimics and support the clinical and electrophysiological diagnosis. The focus is on amyotrophic lateral sclerosis (ALS); supplementary progressive muscular atrophy (PMA, purely lower motor neuron, LMN disease) and spinal muscular atrophy (SMA). OBJECTIVE: Which imaging signs support the diagnosis of ALS, how do electromyography/magnetic resonance imaging (EMG/MRI) fit into the Gold Coast criteria and which other motor neuron diseases

Gastrointestinal manifestations and enzyme replacement therapy in late-onset Pompe disease: insights from a cross-sectional analysis.

BACKGROUND: Late-onset Pompe disease (LOPD) is an autosomal-recessive disorder caused by acid α-glucosidase (GAA) deficiency, typically presenting after the first year of life and resulting in systemic glycogen accumulation. Beyond motor and respiratory manifestations, gastrointestinal (GI) symptoms have been increasingly reported. Although enzyme replacement therapy (ERT) improves skeletal and respiratory function, its impact on GI symptoms remains unclear. METHODS: There were 124 LOPD patients recruited to complete the Gastrointestinal Symptom Rating Scale (GSRS), together with questionnaire

Rating Scales for Obstructive Sleep Apnea Syndrome: The Importance of a Comprehensive Assessment.

Obstructive sleep apnea syndrome (OSAS) is characterized by cyclical apnea states during sleep, caused by alterations in the dilatory muscles of the pharynx and tongue. It is an underestimated and underappreciated disorder, which can affect a person's health and quality of life, as well as trigger the onset of several other systemic diseases. Patients diagnosed with OSAS present with clinical pictures that are not always easy to distinguish and may be associated with multiple comorbidities. This heterogeneity makes early diagnosis difficult and complicates not only the resolution of sleep apne

Changing patterns in tongue, oral cavity, laryngeal and hypopharyngeal squamous cell carcinomas in New Zealand: Incidence, trends and survival from 2006 to 2022.

BACKGROUND: Cancers of the oral cavity, larynx, and hypopharynx have traditionally been associated with common risk factors such as tobacco and alcohol use. With changes in smoking and drinking patterns, the incidence of these cancers is expected to change. Although the incidence of oral cavity cancer has been reported recently, there is limited evidence available for laryngeal and hypopharyngeal cancers in New Zealand (NZ). Furthermore, while growing evidence suggests increasing incidence of tongue cancer among females and young individuals, this trend has not been investigated in NZ. This st

Diagnostic challenge of high vitamin B12 levels: a case report and review of the literature.

BACKGROUND: Vitamin B12 (cobalamin) deficiency is a well-documented clinical condition, with several causes and important consequences. High serum B12 level (hypercobalaminemia) is less common and not easily explained, being caused by different conditions, such as blood disorders, autoimmune diseases, and solid tumors. Moreover, cases of increased B12 levels due to the presence of polymers formed by vitamin B12 and other proteins have been described to show falsely elevated serum levels when measured by conventional methods. CASE PRESENTATION: A 47-year-old white Italian man presented to an i

Association between oral microbiota and inflammatory bowel disease: A 2-sample Mendelian randomization study in East Asian populations.

Inflammatory bowel disease (IBD), including ulcerative colitis (UC) and Crohn's disease (CD), is increasingly prevalent in East Asia. Its pathogenesis is linked to microbiota dysbiosis via the oral-gut axis, but population-specific causal evidence remains scarce. This study aimed to clarify the causal associations between oral microbiota and UC/CD in East Asian populations using Mendelian randomization (MR), providing evidence for IBD etiology and precise prevention/treatment. A 2-sample MR approach was adopted, using genome-wide association study data of East Asian populations. Single nucleot

Distribution of adverse pathological features and prognosis across tongue, buccal, gum, and other oral cancer subsites: A nationwide study.

BACKGROUND: Staging of oral squamous cell carcinoma (OSCC) does not account for anatomical subsites. However, these subsites demonstrate considerable pathological heterogeneity and survival differences. Most prior studies have not systematically examined the influence of distinct pathological factors across subsites. Therefore, this study aimed to evaluate the clinical and pathological characteristics, as well as survival outcomes of OSCC across major oral subsites, using a nationwide cancer registry. METHODS: A total of 17,118 patients with surgically treated OSCC diagnosed between 2018 and
