Successful management of near-infrared photoimmunotherapy-induced skin injury using pedicled flap transfer.

Near-infrared photoimmunotherapy (NIR-PIT) is a recently developed cancer treatment which employs the combination of an antibody-photoabsorbor conjugate and NIR light. Currently, NIR-PIT is approved only in Japan for the treatment of unresectable, locally recurrent head and neck squamous cell carcinoma, using the anti-epidermal growth factor receptor antibody cetuximab and the photoabsorber IR700. However, there is no consensus on the optimal timing and management of NIR-PIT-induced skin injuries from both functional and oncological perspectives. Here, we report the case of a 71-year-old male

Beyond SGCE: expanding the clinical and molecular spectrum of KCTD17- and KCNN2-related myoclonus-dystonia.

Myoclonus-dystonia syndrome (MDS) is a clinically and genetically heterogeneous movement disorder characterized by myoclonus and dystonia as its core features. While mutations in the epsilon-sarcoglycan gene (SGCE) account for most familial cases, heterozygous pathogenic variants in KCTD17 and KCNN2 have recently been described as novel genetic causes of MDS. We describe three patients from two Polish families presenting with progressive movement disorder combined with other features. Exome sequencing (ES) identified a novel heterozygous KCTD17 variant c.461 T > A, p.(Met154Lys) in a five-year

Immunoglobulin light-chain amyloidosis in the setting of multiple myeloma diagnosed from oral biopsy.

BACKGROUND: Immunoglobulin light-chain (AL) amyloidosis, the most common clinical form of systemic amyloidosis, is a serious condition that may affect the oral mucosa. Although macroglossia is the most common manifestation, other oral mucosal sites may be involved. This case report describes AL amyloidosis diagnosed by oral biopsy in a patient who was subsequently found to have multiple myeloma. CASE DESCRIPTION: A 58-year-old woman sought treatment because of multifocal oral nodules, fatigue, weight loss, shoulder and low back pain, congestive heart failure, chronic anemia, cough, episodes o

Betel leaf (paan) chewing: an unusual cause of refractory hypothyroidism in levothyroxine-treated patient.

The habit of chewing betel leaf, or "paan", is prevalent in different parts of India. Betel leaves, areca nut, and lime are common ingredients of paan. Preclinical studies have demonstrated that betel leaf and areca nut affect thyroid function. Lime, due to its high calcium content, adsorbs levothyroxine (LT4) tablets. It is worthwhile to consider paan consumption as a possible etiology of "refractory hypothyroidism" in individuals being treated with a standard LT4 replacement dose in India. Chronic paan chewing causes reddish discoloration of the tongue. Encountering such a finding on examina

Targeted nanoemulsion Blocks MTFR2-HIF-1α-EZH2/FoxM1 axis to suppress tongue squamous cell carcinoma invasion and metastasis.

OBJECTIVE: Tongue squamous cell carcinoma (TSCC) is characterized by high invasiveness and early lymph node metastasis, leading to a poor prognosis.Current surgical and chemoradiotherapy regimens often cause functional impairment, drug resistance, and other drawbacks. Thus, developing highly effective, lowtoxicity treatments is a key research priority. Traditional Chinese medicine (TCM) offers unique advantages via multitarget synergistic effects, enabling innovative therapeutic approaches. METHOD: In this study, we developed Astragaloside IV-Brucea javanica oil nanoemulsion (AS/BJO-NEs) and

Lymphatic Vessel Density or Area Are Not Reliable Prognostic Factors in Oral Cancer.

BACKGROUND: Oral tongue squamous cell carcinomas (OTSCC) frequently metastasise via lymphatic vessels, thus lymphangiogenesis may enhance tumour cell dissemination by increasing tumour-lymphatic interactions. The prognostic significance of lymphatic vessel density (LVD) and lymphatic vessel area (LVA) in OTSCC remains inconclusive. This study aimed to evaluate the association of tumour-associated LVD and LVA with clinical outcomes and pathological characteristics in a homogenous OTSCC cohort. METHODS: We analysed tumour samples from 121 OTSCC patients using D2-40 immunohistochemical staining

Triple-Mutated HSV-1 Expressing Soluble B7-1 Plus CTLA-4 Blockade Suppresses Lymph Node Metastasis in Tongue Cancer.

Cervical lymph node metastasis is a major predictor of poor prognosis in patients with oral squamous cell carcinoma (OSCC), underscoring the need for therapies that effectively target metastatic spread. We previously reported that G47Δ, a third-generation oncolytic herpes simplex virus type 1 (HSV-1), rapidly traffics from tongue tumors to cervical lymph nodes and suppresses metastasis in murine models. Based on this lymphotropic property and the immunological significance of CD28-B7-1 interactions for T-cell activation, we pursue the potential of T-mB7-1, a new G47Δ-derived oncolytic HSV-1 ex

Case series: Joubert syndrome and eosinophilic esophagitis.

Joubert syndrome (JS) is a rare genetic disorder characterized by developmental abnormalities, particularly in the brainstem and cerebellar vermis, alongside multisystem manifestations such as kidney and liver anomalies, polydactyly, cleft lip or palate, and tongue defects. The underlying ciliopathy causing JS may also contribute to gastrointestinal symptoms and immune dysregulation via Wnt signaling and impaired epithelial maintenance. Dysmotility including Hirschsprung disease has been documented at increased rates in JS and in other ciliopathies. Our case series highlights how JS patients f

CEMs-SELEX: DNAzyme-powered de novo biomarker discovery and noninvasive cancer diagnosis.

The ability to interrogate complex biological environments without prior target knowledge is a key challenge in chemical sensing. Conventional methods are limited in discovering previously unidentified biomarkers. To overcome this, we developed CEMs-SELEX (cell extracellular mixtures-systematic evolution of ligands by exponential enrichment), a de novo selection platform that screens for functional DNAzymes responsive to disease-specific CEMs. Applied to tongue squamous cell carcinoma (TSCC), we identified C502, a DNAzyme activated by tumor secretions with high affinity (Kd = 24.49 nanomolar)

Barrier-to-depth ratio as a potential predictor of cervical lymph node metastasis and prognosis in oral tongue squamous cell carcinoma.

Anatomical barriers, such as the sublingual and paralingual spaces, may affect tumor spread, but reliable imaging-based predictors of cervical lymph node metastasis (LNM) in oral tongue squamous cell carcinoma (OTSCC) are limited. This study aimed to evaluate the association of the magnetic resonance imaging (MRI)-derived barrier-to-depth ratio (BDR) with cervical LNM and survival in OTSCC, and to integrate BDR into a prognostic nomogram. A retrospective cohort study was performed at Beijing Stomatological Hospital, Capital Medical University, including patients with OTSCC who underwent preope

Bitter taste TAS2R14 and TAS2R46 receptors bound to G proteins: comparison of cryo-EM, AlphaFold, and molecular dynamics structures.

Bitter taste type 2 G protein-coupled receptors (TAS2R GPCRs) detect bitter substances on the tongue, but they are also prevalent in tissues throughout the human body that play a role in cancer and other diseases, making them targets for pharmacological research. However, structural information on G protein (GP)-bound TAS2R complexes resolved by cryogenic electron microscopy (cryo-EM) is available for only 4 of the 25 TAS2Rs. Herein, we show that AlphaFold (AF) predictions of GP-bound TAS2R14 and TAS2R46 agree well with cryo-EM structure determinations, thus suggesting that the AF2 predicted s

Preoperative MRI-based tumor volume or tumor thickness for tongue cancer: Which is more important?

BACKGROUND: Multiple studies have shown that tumor thickness (TT) or tumor volume (TV) play crucial roles in the diagnosis and treatment of tongue squamous cell carcinoma (TSCC). This study aimed to compare the application value of three-dimensional (3D) TV and TT based on preoperative magnetic resonance imaging (MRI) in forecasting cervical lymph node (CLN) metastases and the prognosis of TSCC. METHODS: A cohort of 81 patients was included in this study. ITK-SNAP and 3D Slicer software were used to determine 3D TV and TT. Xpert Ultra on nasopharyngeal swabs detects one-third of pulmonary tuberculosis: a nested case-control study in South African primary care.

BACKGROUND: Sputum-free alternatives for tuberculosis (TB) diagnosis are a public health priority. Nasopharyngeal swabs (NPSs), a specimen routinely used for viral testing especially at-scale in outbreak investigations, may have promise but has not been evaluated in people investigated for TB. METHODS: We evaluated Xpert MTB/RIF Ultra (Ultra; Cepheid, Sunnyvale, California, USA) on stored NPSs (NPSU) from adults with presumptive TB consecutively presenting to primary care in Cape Town, South Africa. Using a nested case-control design, all participants with TB had a NPS tested (n = 148) and pe

Exploratory study on the association between oral functions and nailfold capillary parameters.

Extending life expectancy is a major public health priority. Oral frailty is closely associated with systemic health. However, the association between oral functions and systemic health, particularly nailfold capillary parameters, remains unclear. We analyzed data from 311 participants (mean age: 55.66 ± 12.54 years) to examine the associations between oral function parameters (number of remaining teeth, tongue pressure, and oral mucosal moisture) and nailfold capillary parameters (number, length, width, and turbidity). The results showed that the number of remaining teeth was significantly as

Sarcoglycanopathy with absent expression of all sarcoglycan proteins in a young cat with clinical features of feline hypertrophic muscular dystrophy.

CASE SUMMARY: A 1-year-old, male castrated domestic shorthair cat presented for chronic mobility decline and muscle stiffness, first noted at 3 months of age, and persistently elevated serum creatine kinase levels (>19,000-53,000 IU/l). Clinical examination revealed macroglossia and selective hypertrophy of cervical and appendicular muscles. Neurologic evaluation indicated a myopathic gait without pain or neurologic deficits. Muscle biopsy was supportive of muscular dystrophy (MD) and immunohistochemistry demonstrated loss of the sarcoglycan complex with complete absence of all tested sarcogly
