A Tale of Two Infections: A Rare Synchronous Infection of Herpes Simplex Virus Oesophagitis and Clostridium difficile in an Immunocompetent Host.

Herpes simplex virus (HSV) is a common cause of infectious oesophagitis, predominantly affecting immunocompromised populations. Clostridium difficile (C. difficile), while prevalent in this population, is also a frequently encountered cause of nosocomial diarrhoea associated with antibiotic exposure, prolonged hospitalisation, or residence in long-term care facilities. A simultaneous HSV oesophagitis and C. difficile infection (CDI), despite the absence of the typical risk factors, is rare but possible, as illustrated by the following case.  A 95-year-old female patient presented with acute co

New-onset IgE mediated reactions in eosinophilic esophagitis associated elimination diets (NIMREEDs).

BACKGROUND: Empiric elimination diets remain a cornerstone of treatment for eosinophilic esophagitis (EoE). Immunoglobulin E (IgE) mediated allergic reactions have been sparsely reported during the food reintroduction phase, raising concerns both for patients and physicians. We aimed to assess the physician reported rates of new-onset IgE mediated reactions in EoE associated elimination diets (NIMREEDs), physician awareness of NIMREEDs, and how awareness impacts physician and patient treatment selection. METHODS: An international REDCap based questionnaire was distributed via interest groups,

Research tools and protocols: readability of pediatric and adult patient-reported outcome measures in eosinophilic esophagitis.

Patient-reported outcome measures (PROs) in eosinophilic esophagitis (EoE) have been utilized as research tools to assess outcomes in clinical trials. To our knowledge, adult and pediatric EoE PROs have not previously been analyzed from a health literacy perspective. We aimed to evaluate the readability of the most utilized EoE PROs for adult and pediatric populations and assess whether these PROs met national health literacy recommendations of readability at or below the sixth-grade level. We conducted a readability analysis of thirteen EoE PROs using four readability measures: Flesch-Kincaid

Review article: Seeing Is Believing-The Eosinophilic Oesophagitis Endoscopic Reference Score Can Serve as a Valuable Clinical Trial Endpoint in Eosinophilic Oesophagitis.

BACKGROUND: Eosinophilic oesophagitis (EoE) is a major contributor to upper gastrointestinal morbidity. Although an increasing number of treatment modalities have been developed, most are assessed by symptoms and the histologic metric of peak eosinophil count (PEC). AIM: To review current evidence regarding limitations of reliance on PEC as a co-primary endpoint while making a case for endoscopic assessment as a "trial-ready" outcome to supplement or replace it. METHODS: Directed literature review, including a summary of EoE clinical trials and outcome metrics, combined with expert discussio

Oesophageal Epithelial Cell-Intrinsic MHCII Regulates Food Antigen-Dependent Eosinophilic Esophagitis in an IFNγ-Dependent Manner.

BACKGROUND: Eosinophilic oesophagitis (EoE) is a chronic food allergy that causes oesophageal inflammation and dysfunction. Recent work demonstrates IFNγ-dependent gene signatures in inflamed EoE biopsies. IFNγ has been implicated in the promotion of MHCII expression on oesophageal epithelial cells (EECs). However, the regulation of EEC-MHCII expression in vivo, and its contribution to EoE, is unknown. OBJECTIVE: The objective of this study was to determine the regulation and role of EEC-intrinsic MHCII expression in EoE. METHODS: We examined the expression of HLA II-pathway transcripts in h

Mobile endoscopy in rural South Africa: outcomes of a decentralised outreach programme to improve gastrointestinal healthcare access.

INTRODUCTION: Gastrointestinal (GI) endoscopy is a valuable diagnostic tool for identifying upper and lower GI pathologies yet is rarely available in resource-limited settings. This project evaluates the impact of a mobile endoscopy outreach effort implemented across five district hospitals in the Western Cape of South Africa. METHODS: A mobile endoscopy outreach programme was created and implemented at five district hospitals within the Western Cape of South Africa. Health records of patients who presented for endoscopy across these five hospitals were then retrospectively reviewed. Descript

Hiatal Hernia Size and Reflux Parameters in Gastro-Oesophageal Reflux Disease: Evidence From a Retrospective Cohort.

Background Hiatal hernia is commonly associated with gastro-oesophageal reflux disease (GORD). However, the relationship between hernia size and reflux severity remains unclear. This study aimed to assess the impact of the hiatal hernia size on DeMeester score, symptom correlation, and presence of pathological reflux on pH studies. The study also looked at the impact of various characteristics, including age, smoking, alcohol consumption, body mass index (BMI), and presence of oesophagitis and/or dysmotility, on DeMeester score, symptom correlation, and presence of pathological reflux on pH st

Trachealisation of the Oesophagus in Lymphocytic Oesophagitis: An Uncommon Endoscopic Presentation.

Lymphocytic oesophagitis (LyE) is a rare histopathological diagnosis with variable and often non-specific endoscopic features. Oesophageal rings and trachealisation are typically associated with eosinophilic oesophagitis but are uncommonly described in LyE. We describe a 79-year-old woman who presented with progressive dysphagia and unintentional weight loss. Endoscopic examination demonstrated trachealisation of the oesophagus, while histopathological evaluation confirmed florid lymphocytic oesophagitis with apoptotic keratinocytes. The patient exhibited only partial symptom relief with proto

MiR-155 and MiR-1275 relation with graft-versus-host disease and hepatitis B in hematopoietic stem cell transplant recipients.

Mepolizumab is an anti-interleukin-5 monoclonal antibody that is in clinical trials with GlaxoSmithKline (GSK) for the treatment of severe asthma, nasal polyposis and hypereosinophilic syndrome and eosinophilic oesophagitis (the latter two indications are classed as eosinophilia in the phase table). Interleukin-5 stimulates the production, activation and maturation of eosinophils. Since mepolizumab inhibits interleukin-5 and has a long terminal half-life, treatment with mepolizumab causes a sustained reduction in the numbers of circulating eosinophils. Thus, mepolizumab may be a useful therape

Targeted Axillary Dissection for Clinically Node-Positive Breast Cancer Cases that Turn Negative After Neoadjuvant Systemic Therapy Using Charcoal Tattooing.

The magnitude of the problem of sickle cell disease (SCD) in India is very large and diverse. For people living with SCD there are several unmet health care needs making it pertinent to focus on recognizing the important public health burden of SCD in the country. In a bid to achieve this goal, the Indian College of Hematology (ICH) of the Indian Society of Hematology and Blood Transfusion (ISHBT) in collaboration with Indian Council of Medical Research (ICMR) has formulated a Guideline on Management and Control of Sickle Cell Disease in India. This was achieved by a national taskforce compris

Italian guidelines for the diagnosis and management of gastro-esophageal reflux disease: Joint consensus from the Italian Societies of: Gastroenterology and Endoscopy (SIGE), Neurogastroenterology and Motility (SINGEM), Hospital Gastroenterologists and Endoscopists (AIGO), Digestive Endoscopy (SIED), and General Medicine (SIMG).

Gastroesophageal reflux disease (GERD) is one of the most common conditions encountered in outpatient general medicine and gastroenterology clinics. However, uncertainties remain, particularly concerning the optimal diagnostic work-up and the most effective management. To address this issue, experts from 5 Italian Societies conducted a Delphi consensus process, which included a review of the current literature and voting process on 27 key statements. Recommendations and quality of evidence were evaluated using the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) crit

Variable topography of black oesophagus (acute necrotising oesophagitis).

Black oesophagus, or acute necrotising oesophagitis, is characterised by diffuse circumferential black discolouration of the distal oesophagus associated with mucosal necrosis. A consistent feature is a very sharp line of demarcation at the gastro-oesophageal junction. Two cases are reported to demonstrate differences in the lower margin. Case 1: a 63-year-old man who died of gastrointestinal haemorrhage complicating black oesophagus was noted to have an irregular lower border located approximately 5 cm from the gastro-oesophageal junction. This was associated with columnar mucosa typical of B

Before the Potty Party: Preparing Practitioners for Toilet Training.

Air pollution due to the presence of small particles and gases in the atmosphere is a major cause of health problems. In urban areas, where most of the population is concentrated, traffic is a major source of air pollutants (such as nitrogen oxides or NO x and carbon monoxide or CO). Therefore, for smart cities, carrying out an adequate traffic monitoring is a key issue, since it can help citizens to make better decisions and public administrations to define appropriate policies. Thus, citizens could use these data to make appropriate mobility decisions. In the same way, a city council can ex

A New Classification System for Anatomic Variations in Axial Blood Supply of Pectoralis Major Muscle Flap: Do They Have an Impact on Flap Outcomes?

BACKGROUND: Although long-term outcomes following laparoscopic fundoplication for gastro-oesophageal disease have now been reported as very satisfactory, a small, but important, minority of patients are unhappy with the outcome, often due to recurrent reflux symptoms or new-onset dysphagia. In this study, we sought to establish whether various parameters that can be determined before surgery, can predict the long-term outcome of surgery. METHODS: Data collected prospectively were evaluated to determine factors that were associated with outcome at 5 years following laparoscopic fundoplication.

GZMK-expressing CD8+ T cells promote recurrent airway inflammatory diseases.

Inflammatory diseases are often chronic and recurrent, and current treatments do not typically remove underlying disease drivers1. T cells participate in a wide range of inflammatory diseases such as psoriasis2, Crohn's disease3, oesophagitis4 and multiple sclerosis5,6, and clonally expanded antigen-specific T cells may contribute to disease chronicity and recurrence, in part by forming persistent pathogenic memory. Chronic rhinosinusitis and asthma are inflammatory airway diseases that often present as comorbidities7. Chronic rhinosinusitis affects more than 10% of the general population8. Am

Real World Outcomes with Treatment Free Remission in Chronic Myeloid Leukemia-Experience from a Tertiary Care Cancer Centre.

Chronic myeloid leukaemia (CML) is caused by balanced translocation t(9::22)(q34;q11) resulting in formation of pathogenic BCR-ABL fusion gene. Tyrosine kinase inhibitors (TKI) have revolutionised the treatment of CML. Ongoing treatment with TKI leads to side effects and has financial impact. Teratogenic potential of TKI and growth disturbance also represent an important challenge. Thus, TKI discontinuation in form of treatment free remission (TFR) has emerged as a new and important therapeutic goal. In this retrospective study, we reviewed CML patients who were kept on TFR. Inclusion criteria
