Electronic Health Record Clinical Decision Support Tool Improves Ambulatory Reflux Testing Practices.

INTRODUCTION: Performing reflux testing on acid suppression without a confirmed diagnosis of gastroesophageal reflux disease conflicts with best practice recommendations and reduces diagnostic utility. We assessed whether clinical decision support (CDS) improves ordering practices. METHODS: Quality improvement methods were used to develop and test the effectiveness of a novel CDS tool. RESULTS: The testing rate on acid suppression decreased (30.9% vs 8.4%; P < 0.01), best practice concordance increased (16.4% vs 60.0%; P < 0.001), and more clinical management changes occurred (23.1% vs 60.0%

Preoperative Hill's classification as a predictor of postoperative gastroesophageal reflux disease following laparoscopic sleeve gastrectomy: a prospective multicenter study from Egypt.

BACKGROUND: Laparoscopic sleeve gastrectomy (LSG) is the most widely used bariatric procedure, but its influence on gastroesophageal reflux disease (GERD), whether inducing, aggravating, or improving it, remains a subject of ongoing debate. This study evaluated whether preoperative Hill’s classification can predict postoperative GERD and identified other endoscopic factors related to this outcome in an Egyptian multicenter cohort. METHODS: In this prospective multicenter cohort, 106 consecutive morbidly obese patients undergoing primary LSG (January 2023–June 2024) were evaluated. All underwen

Effects of bariatric surgery on obesity associated gastroesophageal reflux disease: Insights from a systematic review and network meta-analysis.

BACKGROUND: Obesity is a global epidemic frequently associated with gastroesophageal reflux disease (GERD). Metabolic and bariatric surgery (MBS) is the most effective treatment for weight reduction in patients with obesity, with the most commonly performed procedures being sleeve gastrectomy (SG), Roux-en-Y gastric bypass (RYGB), and one-anastomosis gastric bypass (OAGB). Many recent studies focused on determining the best procedure for patients with obesity and GERD; however, results from these studies vary, and the optimal procedure remains uncertain. AIM: To compare the effects of MBSs -

Improvement of nighttime gastroesophageal reflux symptoms with sleep positional therapy using a smartwatch app.

Nighttime gastroesophageal reflux symptoms affect sleep quality and daytime functioning. The aim of this study is to investigate if sleep positional therapy, using a smartwatch app, can reduce nighttime gastroesophageal reflux symptoms by training patients to avoid sleeping in right lateral decubitus position. Patients with nighttime gastroesophageal reflux symptoms were included. During a two-week treatment period a smartwatch app was used that was designed to vibrate when the subject is lying in right lateral decubitus position, prompting patients to change their sleeping position. Symptoms

The Combined Utility of the CARS Endoscopic Score and Functional Lumen Imaging Probe (FLIP) Based on Real-Time Assessments.

BACKGROUND: During endoscopy, which often is the initial diagnostic test performed for esophageal symptoms, a CARS endoscopic score can be calculated and functional lumen imaging probe (FLIP) panometry can be performed. Recently, the combination of a CARS score > 4 calculated from endoscopic videos and non-spastic obstruction on FLIP noted from offline analysis has been found to be highly predictive of disorders of EGJ outflow obstruction (i.e., achalasia and conclusive EGJOO). We aimed to validate these findings based on detailed assessments performed in real time during endoscopy and FLIP pa

Acid bile salt induces esophageal mucosal barrier dysfunction through miR-146a-5p-KYNU pathway in gastroesophageal reflux disease.

BACKGROUND: Gastroesophageal reflux disease (GERD) is a common digestive disease characterized by impaired esophageal mucosal barrier integrity. Growing evidence indicated that mucosal barrier dysfunction of GERD might be attributed to esophageal inflammation, rather than direct chemical damage of acidic refluxates. However, the potential mechanisms underlying the activation of esophageal inflammation in GERD remain unclear. METHODS: Patients with GERD symptoms (gastroesophageal reflux disease [GERD] and functional esophageal disorders [FED]) and healthy volunteers were prospectively enrolled

Expanding the Contemporary Paradigm of Sleeve Gastrectomy: Innovation, Prevention, and Physiological Preservation.

Over the past two decades, sleeve gastrectomy has become the most widely performed bariatric operation, yet growing long-term evidence has highlighted limitations such as weight recurrence, gastroesophageal reflux, and declining metabolic durability, prompting a shift toward refining rather than abandoning sleeve-based strategies. The articles in this special issue collectively demonstrate how thoughtful innovations-ranging from metabolic enhancements and reflux-preserving solutions to reversible procedures and preventive diagnostic measures-can expand the therapeutic scope of sleeve gastrecto

Combining Transoral Incisionless Fundoplication and Endoscopic Sleeve Gastroplasty (F-ESG): An Endoscopic Approach to Treat Pathologic Gastroesophageal Reflux in Obesity.

BACKGROUND AND AIMS: Obesity-related Gastroesophageal Reflux Disease (GERD) presents a significant clinical challenge, limited by the effectiveness of current treatments. Excess weight impairs the repair of the anti-reflux barrier, and conventional obesity treatments may adversely affect anti-reflux physiology. This study systematically evaluates a modular endoscopic technique that combines Transoral Incisionless Fundoplication (TIF) with Endoscopic Sleeve Gastroplasty (ESG) simultaneously (F-ESG), offering a synergistic solution to both GERD and obesity. METHODS: This single-center pilot stu

Dynamic magnetic resonance imaging evaluation of stomach geometry modifications after endoscopic sleeve gastroplasty.

INTRODUCTION: Sleeve gastrectomy (SG) induces weight loss through tubulization of the stomach. Up to 35% of SG patients develop Gastroesophageal Reflux Disease (GERD). The pathophysiological mechanism for GERD development has been associated to changes in the geometry and mechanics of the Esophago-Gastric junction (EGJ). Endoscopic sleeve gastroplasty (ESG) is an endoscopic bariatric procedure leading to weight loss and obesity-related comorbidities resolution through gastric restriction and shortening. Unlike SG, ESG preserves the gastric fundus, potentially resulting in lesser impact on EGJ

Genetic evidence links alcohol, coffee, cheese, and anxiety to GERD risk: A Mendelian randomization study.

In previous observational studies, an association has been found between the frequency of alcohol consumption, coffee intake, cheese consumption, and anxiety and the risk of gastroesophageal reflux disease (GERD). However, conflicting conclusions exist among these studies, and the causal relationship between these exposure factors and GERD remains unclear. Independent genetic variants associated with alcohol consumption frequency, coffee intake, cheese consumption, and anxiety at the genome-wide significance level were selected as instrumental variables. Summary-level data for GERD were derive

Gas Bloat Syndrome after Nissen Fundoplication: Association with Anatomical Failure and Revisional Operation.

BACKGROUND: Gas bloat syndrome (GBS) is a recognized complication of Nissen fundoplication (NF), characterized by impaired gastric venting and progressive distention. Although its negative impact on quality of life is well established, the relationship between GBS, anatomical failure, and revisional surgery remains unclear. This study evaluated the association between GBS, clinical outcomes, anatomical integrity, and reoperation after NF. STUDY DESIGN: We analyzed 554 patients who underwent primary NF at our institution and completed postoperative GERD-Health-Related Quality of Life (GERD-HRQ

Measuring the long diameter of the esophageal hiatus using axial CT images.

OBJECTIVES: The absence of normative anatomical benchmarks often leads to suboptimal crural closure during anti-reflux surgery. The hiatal long diameter represents a favorable anatomical parameter for measurement. This study therefore aimed to: 1) Validate a straightforward method for measuring it using axial CT images; 2) Establish normative thresholds for this diameter in an asymptomatic population; and 3) Evaluate its predictive value for key gastroesophageal reflux disease (GERD) related parameters. MATERIALS AND METHODS: We prospectively enrolled asymptomatic volunteers and patients who

Validation of Dominant Symptom Intensity as a Patient-Reported Outcome Measure in the Evaluation of Esophageal Symptoms.

BACKGROUND: Since gastroesophageal reflux disease (GERD) manifests typical and atypical symptoms of varying frequency and severity, the dominant symptom needs identification and quantification. GOALS: We aimed to compare 5-point Likert scales assessing symptom burden to validated GERD questionnaires and outcomes following GERD management. STUDY: We retrospectively analyzed pooled data from patients undergoing pH-impedance testing for reflux symptoms from 2 centers. Preprocedure questionnaires assessed symptom severity and frequency on 5-point Likert scales (0=not severe, infrequent; 4=extrem

Five-year comparative outcomes of Nissen sleeve gastrectomy versus standard sleeve gastrectomy in patients with type 2 diabetes.

BACKGROUND: Patients with type 2 diabetes (T2D) and obesity are prone to gastroesophageal reflux disease (GERD) due not only to obesity itself but also to gastroparesis, digestive neuropathy, and antidiabetic treatment. Nissen sleeve gastrectomy (NSG) achieves satisfactory long-term weight loss and significantly reduces GERD incidence. However, its results in this population remain to be compared with those of standard sleeve gastrectomy (SG) given the potential impact of the Nissen wrap on GERD outcomes. OBJECTIVES: To compare the 5-year results of NSG on patients with T2D and obesity in ter

POEM vs laparoscopic Heller myotomy with fundoplication for achalasia: systematic review and meta-analysis of randomized and prospective studies.

BACKGROUND: Achalasia is definitively treated with peroral endoscopic myotomy (POEM) or laparoscopic Heller myotomy with fundoplication (LHM + PF), but reflux after POEM remains a key concern. We compared the safety and GERD outcomes of POEM and LHM + PF using randomized controlled trials and prospective comparative studies in treatment-naïve adults. METHODS: We searched the MEDLINE, Embase, CENTRAL, Web of Science, and Scopus databases on October 18, 2025. Eligible studies directly compared POEM and LHM + PF in adults. The primary outcome was adverse events, and the secondary outcomes were e

Long-term outcomes of peroral endoscopic myotomy for achalasia: a systematic review and meta-analysis with median follow-up ≥ 5 years.

BACKGROUND: Peroral Endoscopic Myotomy (POEM) has become the first-line minimally invasive treatment for achalasia. Its short-term and mid-term (1-5 years) efficacy has been consistently demonstrated, but long-term follow-up data (≥ 5 years) remain limited. This systematic review and meta-analysis aims to comprehensively evaluate the long-term (median follow-up ≥ 5 years) clinical efficacy and safety of POEM. METHODS: Using medical literature databases, we retrieved randomized controlled trials (RCTs) and non-randomized comparative studies from the inception of the databases through September
