Why a positive rheumatoid factor test does not always mean rheumatoid arthritis.

Rheumatoid factors (RFs) are autoantibodies directed against the tail region of IgG antibodies and are present in a majority of people with rheumatoid arthritis (RA). RF production is commonly assessed to help diagnose RA, but its performance is limited due to low specificity and sensitivity for reasons that have remained unclear. In this issue of the JCI, Hocaoğlu and Sawalha shed light on this matter using extensive phenome-, genome-, transcriptome-, and proteome-wide association analyses of RF-seropositive and -seronegative individuals. They report that RF production was controlled by a gen

Ovesco StentFix for the Fixation of Fully Covered Self-Expanding Metal Stents and Lumen-Apposing Metal Stents Across Diverse Gastrointestinal Indications: A Single-Centre Pilot Case Series.

Background and aim Stent migration is a recognised complication of fully covered self-expandable metal stents (FCSEMS). Over-the-scope (OTS) clip-based fixation devices have emerged to address this limitation, yet real-world evidence across heterogeneous clinical indications, including lumen-apposing metal stents (LAMS) and endoscopic ultrasound-directed transgastric endoscopic retrograde cholangiopancreatography (EDGE) procedures, remains limited. We report our single-centre experience with the Ovesco StentFix system (Ovesco Endoscopy AG, Tübingen, Germany) across a broad range of gastrointes

Evolution of endoluminal treatment of Zenker's diverticulum and complete pharyngo-oesophageal obstruction.

PURPOSE OF REVIEW: Endoscopic approaches to surgery of the foregut, in particular, the upper oesophageal sphincter (UES), continue to develop, providing options for management that were previously unavailable. This review examines endoluminal surgical tunnelling approaches (third space endoscopy) to treat Zenker diverticulum and the more rare condition of complete oesophageal stenosis. RECENT FINDINGS: Endoluminal surgical approaches to the UES such as Z-POEM (per-oral endoscopic myotomy) that utilize gastroscopes and upper gastrointestinal endoscopic instrumentation, are increasingly being u

Bacterial Pneumonia in Horses: Insights from a 10-Year European Single-Centre Retrospective Study.

Bacterial pneumonia in horses typically occurs secondary to predisposing factors that compromise defence mechanisms. This 10-year retrospective study evaluated bacterial pneumonia in 81 horses treated at the equine clinic of Freie Universität Berlin from January 2015 to December 2024. Horses infected with primary pathogens such as Rhodococcus equi or Streptococcus equi ssp. equi were not included in the study. The most common predisposing events were aspiration, mainly following oesophageal obstruction (37.0%), post-anaesthetic disease (27.2%), primary respiratory signs (24.7%), and other cond

Late age at first birth is a protective factor for oesophageal cancer and gastro-oesophageal reflux: the evidence from the genetic study.

OBJECTIVE: The primary objective of this research endeavor was to examine the underlying genetic causality between the age at first birth (AFB) and four prevalent esophageal diseases, namely oesophageal obstruction (OO), oesophageal varices (OV), gastro-oesophageal reflux (GOR), and oesophageal cancer (OC). METHODS: We conducted a two-sample Mendelian randomization (MR) analysis to examine the causal association between AFB and four prevalent esophageal disorders. We employed eight distinct MR analysis techniques to evaluate causal relationships, encompassing random-effects inverse variance w

Multidisciplinary, minimally invasive approach for oesophageal perforations with delayed presentation.

BACKGROUND: The present study aims to report the outcomes of a multidisciplinary, minimally invasive approach to treating patients with delayed presentation of oesophageal perforation. PATIENTS AND METHODS: The present study is a retrospective analysis of prospectively maintained data at a tertiary care centre. All patients with oesophageal perforation presenting over 48 h after the onset of symptoms and without oesophageal obstruction were included in the study. Self-expanding Metallic Stent (SEMS) or endoscopic clip placement was performed in all the patients, followed by video-assisted tho

Zenker's diverticulitis: a bitter pill to swallow.

Acute oesophageal obstruction from food bolus impaction is often triggered by underlying oesophageal pathology, both benign and malignant. These can be readily detected with standard investigations such as oesophagoscopy or computed tomography. Zenker's diverticulum (ZD) is a benign condition frequently presenting with chronic dysphagia or may be asymptomatic. We report the case of an 81-year-old man with a previously undiagnosed 1-cm ZD causing complete oesophageal obstruction secondary to localized oedema from an impacted ibuprofen tablet. Although initial clinical, endoscopic and radiologic

A retrospective study of choke (oesophageal obstruction) in 64 one-hump Dromedary camels (Camelus dromedarius) in Saudi Arabia.

BACKGROUND: Choke (oesophageal obstruction) is an important oesophageal disorder in large domestic animals. Published studies on choke in the dromedary camel (Camelus dromedarius) are few in number and deal with small number of cases. METHODS: Sixty-four camels with choke were presented to the Veterinary Teaching Hospital, King Faisal University. History, breed, age, sex, duration of obstruction and clinical signs were recorded. The diagnosis was established using examination with a stomach tube, oesophageal radiography and endoscopy. Choke was managed either by using alligator forceps guided

Oesophageal Obstruction in a Donkey Due to Mediastinal Lymphadenitis Caused by Mycobacterium avium Complex.

Mycobacterial infections are rare in horses, donkeys and mules. Although there are a few reports in horses, mycobacterial disease is poorly documented in the donkey. Mycobacterial infection of equine species typically affects the alimentary tract, causing granulomatous enterocolitis resulting in diarrhoea and chronic weight loss, while lymph nodes and liver may also be affected. We now document recurrent oesophageal obstruction, secondary to cranial mediastinal lymphadenitis caused by Mycobacterium avium complex (MAC). To the best of our knowledge, this is the first report of MAC infection in

A multinational review: Oesophageal cancer in low to middle-income countries.

Oesophageal cancer (OC) is an aggressive neoplasm that manifests in the gastrointestinal tract and is the result of numerous factors that can contribute to the development of the disease. These may include old age, nutritional deficiencies, oesophageal obstruction and food ingestion difficulties. Environmental factors serve a large role in increasing the risk of developing OC. Two factors that serve an increasing risk of developing OC are the use of tobacco and the consumption of alcohol. Genetic factors also exhibit a large effect on the risk of developing OC, for example, the causative genes

[Peculiarities of diagnosis and emergency surgical care in aorto-oesophageal fistula].

An aorto-oesophageal fistula is a rare but life-threatening pathological condition developing on the background of diseases of the aorta and oesophagus, as well as after surgical interventions on the aorta. The article deals with a clinical case report regarding management of a patient presenting with an aorto-oesophageal fistula resulting from a thoracic artery aneurysm. The main clinical manifestations of the diseases included dysphagia (due to oesophageal obstruction caused by thrombotic masses of the aneurysm) and the occurring gastrointestinal haemorrhage. Comprehensive instrumental diagn

Oesophageal obstruction due to trichobezoars in two cats.

CASE SERIES SUMMARY: This case series describes two cats diagnosed with oesophageal obstruction due to trichobezoars. Both cases presented for acute dyspnoea, with thoracic radiographs revealing changes consistent with oesophageal foreign material causing ventral displacement of the trachea. Endoscopic removal was unsuccessful and both cases required surgical intervention. Case 1 died within 24 h of trichobezoar removal, likely from aspiration pneumonia. Case 2 developed laryngeal collapse 10 days after trichobezoar removal and required a permanent tracheostomy. Case 2 has been followed up for

Etorphine-Ketamine Constant Rate Infusion for Maintenance of Anaesthesia in a Compromised White Rhinoceros (Ceratotherium simum).

A subadult white rhinoceros bull presented for oesophageal endoscopic evaluation and foreign body removal under general anaesthesia. The animal had a history of nasal and oral regurgitation of water and ingesta with weight-loss for 6 days prior to the procedure and had been diagnosed with oesophageal obstruction caused by a bailing wire. Anaesthesia was induced with intramuscular etorphine and azaperone delivered remotely by dart, followed by an intravenous bolus of ketamine. The trachea was intubated, and anaesthesia was maintained with an etorphine-ketamine constant rate infusion (CRI). The

Swing-back and trap-door technique repair for interrupted aortic arch with right-sided descending aorta.

A right-sided descending aorta with a left aortic arch is a rare congenital anomaly in which the aortic arch crosses the midline from the left side of the trachea coursing to the descending aorta in the right thoracic cavity. The surgical repair of an interrupted aortic arch with a right-sided descending aorta carries great risks of bronchial and oesophageal obstruction. Herein, we describe a case of successful surgical repair of an interrupted aortic arch with a right-sided descending aorta using the swing-back and trap-door techniques.

Large mediastinal haematoma after paraoesophageal hernia repair.

Mediastinal haematoma is a rare complication of laparoscopic paraoesophageal hernia repair with few documented cases in the current literature. Presentation of this unique clinical problem can range from extrinsic oesophageal obstruction to life-hreatening cardiac tamponade and therefore, warrants further discussion of at-risk population aetiology, diagnosis and successful management strategies. We present the case of a 71-year-old woman who underwent laparoscopic paraoesophageal hernia repair with nissen fundoplication complicated by severe dysphagia on postoperative day 12. Dysphagia as a symptom of anterior cervical hyperostosis - Case report.

Degenerative lesions with hyperostosis on the anterior surface of cervical spine are common in clinical practice. In addition to pain being an effect of spinal dysfunction, they sometimes cause difficulties in swallowing or speaking as well as breathing disorders. A 52-year-old farmer with 4-year history of gradually intensifying dysphagia was admitted to hospital due to inability to intake a solid food, significant weight loss, and because of the appearance of a new symptom - dysphonia. Previous conservative treatment for swallow difficulties was ineffective. CT revealed a bone excrescence on

Esophageal obstruction due to a right-sided Zenker diverticulum.

A Zenker's or pharyngoesophageal diverticulum may represent a rare cause of upper digestive obstruction, or more often, cervical dysphagia, regurgitations and cough. It develops most often on the posterior left side of cervical oesophagus in elderly patients, and depending on the severity of clinical symptoms may warrant surgical or endoscopic treatment. For large lesions with a difficult endoscopic access to the diverticular neck, surgery is recommended. In our case, we illustrate a giant right-sided Zenker's diverticulum responsible for complete aphagia in a 78-year-old male patient. Open su
