A Zenker's or pharyngoesophageal diverticulum may represent a rare cause of upper digestive obstruction, or more often, cervical dysphagia, regurgitations and cough. It develops most often on the posterior left side of cervical oesophagus in elderly patients, and depending on the severity of clinical symptoms may warrant surgical or endoscopic treatment. For large lesions with a difficult endoscopic access to the diverticular neck, surgery is recommended. In our case, we illustrate a giant right-sided Zenker's diverticulum responsible for complete aphagia in a 78-year-old male patient. Open su

A 57-year-old man presented with a 4-month history of worsening symptoms of oesophageal obstruction. The physical examination was unremarkable. An oesophagogastroduodenoscopy revealed an exophytic tumour in the distal oesophagus. A biopsy demonstrated malignant melanoma. Staging of the melanoma showed disseminated lymph node and bony-spine metastases. He had no prior history of cutaneous or ocular melanoma. Following full multidisciplinary team input, he was palliated with a metal mesh stent and immunotherapy. He died 3 months later following community management.

AIM: To investigate the combined antegrade-retrograde endoscopic rendezvous technique for complete oesophageal obstruction and the swallowing outcome.
METHODS: This single-centre case series includes consecutive patients who were unable to swallow due to complete oesophageal obstruction and underwent combined antegrade-retrograde endoscopic dilation (CARD) within the last 10 years. The patients' demographic characteristics, clinical parameters, endoscopic therapy, adverse events, and outcomes were obtained retrospectively. Technical success was defined as effective restoration of oesophageal

A previously healthy 1-year-old boy suddenly began choking and coughing after ingesting a coin. The child's mother attempted to extract the coin by inserting her fingers in his mouth and sweeping the oral cavity. The mother felt the object momentarily with her fingertips but was unable to retrieve it, and brought her son to a local hospital for assistance. The patient was referred to our emergency department (ED) for possible oesophageal obstruction by a foreign body based on the X-ray findings. On arrival at our ED, the child exhibited mild gagging but presented no respiratory symptoms and no

AIM: To investigate the efficacy of double-layered covered stent in the treatment of malignant oesophageal obstructions.
METHODS: A systematic review and meta-analysis was performed following the PRISMA process. PubMed (Medline), EMBASE (Excerpta Medical Database), AMED (Allied and Complementary medicine Database), Scopus and online content, were searched for studies reporting on the NiTi-S polyurethane-covered double oesophageal stent for the treatment of malignant dysphagia. Weighted pooled outcomes were synthesized with a random effects model to account for clinical heterogeneity. All stud

Formation of an intestinal pharmacobezoar is a rare condition. It may form after intake of various oral pharmaceutical preparations of drugs, both as a result of an acute overdose and through chronic use of therapeutic doses of a drug. We report a case with a patient presenting with an oesophageal pharmacobezoar and complete obstruction of the oesophagus and severe toxic symptoms and death related to oral ingestion of multiple drugs.

CONCLUSION: Bidirectional oesophageal dilatation for severe chemoradiation-induced oesophageal strictures is efficacious in improving luminal patency but ineffective in relieving functional dysphagia.
OBJECTIVE: To assess the efficacy of bidirectional oesophageal dilatation in the severely strictured oesophagus induced by radiation therapy following the treatment of head and neck malignancies.
METHODS: The study design was a case series in the setting of a tertiary cancer centre. We carried out a retrospective analysis of patients who underwent bidirectional oesophageal dilatation for oesoph

Enteral nutrition has a relatively low incidence of major complications. The most common complications are mechanical problems, bronchoaspiration and diarrhoea. A rare complication associated with the use of enteral nutrition is oesophageal bezoar. A bezoar is a body of undigested and partially digested matter in the gastrointestinal tract. The main risks factors are gastric motility dysfunction and the use of opiates or sucralfate. The aim of this paper was to present two cases of oesophageal obstruction resulting from the formation of bezoars due to enteral nutrition. Both patients experienc

INTRODUCTION: We report the rare case of a patient presenting with dysphagia secondary to a large vertebral osteophyte, which formed from his previous occupation.
PRESENTATION OF CASE: A 76-year-old gentleman presented with a year-long history of dysphagia to solids, at the laryngeal level. He was otherwise well, with no red-flag symptoms. Nasoendoscopy showed a left-sided bulge obstructing the piriform fossa. Barium swallow demonstrated a large C4/C5 vertebral osteophyte. Excluding other abnormalities the patient's dysphagia was determined to be due to the osteophyte. The patient mentioned c

Endoluminal stenting has revolutionised the practice of gastrointestinal endoscopy for many years. What started as rigid, inflexible, plastic stents have now evolved into flexible, easy-to-deploy self-expanding stents with a myriad of choices, including covered, partially covered and uncovered types. Many of these also come along with special features including anti-reflux, anti-migration and lasso for stent adjustment and retrieval. Numerous papers with meta-analyses and systemic reviews have without doubt confirmed the efficacy, safety and cost effectiveness of endoluminal stenting in the pa

Oesophageal cancer is the sixth leading cause of cancer-related death worldwide. At the time of presentation, more than half of patients with oesophageal cancer have advanced disease not eligible for curative resection. Due to this, the treatment of oesophageal malignancy is usually palliative, with the main aim to relieve dysphagia, maintain nutritional intake and occlusion of tracheoesophageal fistulas. Palliative surgery offers the optimal alleviation for oesophageal obstruction symptoms such as dysphagia and vomiting. However, due to the poor prognosis, the short survival time, the signifi

Spontaneous retropharyngeal haematoma and dissecting intramural haematoma of the oesophagus are two distinct, but rare, phenomena. We describe the first case of complete tracheo-oesophageal obstruction due to spontaneous retropharyngeal haematoma presenting with chest pain and dysphagia. Rapid imaging allowed life-saving transfer to the regional specialist centre, with immediate surgical intervention. The importance, aetiology and clinical features of both diagnoses are discussed.

Oesophageal lithobezoar is a very rare clinical entity, especially presenting as a case of acute dysphagia. A 54-year-old man, with long-standing psychiatric illness, presented with a recent episode of vomiting and chest pain followed by dysphagia. Chest x-ray revealed a radio opaque shadow in the posterior mediastinum. Endoscopy showed a calcified foreign body in the lower oesophagus. Endoscopic attempts to remove or fragment the foreign body were unsuccessful. Foreign body was removed successfully through oesophagotomy using left posterolateral thoracotomy. Pathology revealed oesophageal lit

Persistent right aortic arch (PRAA) in cats is an uncommon vascular anomaly with clinical signs referable to oesophageal obstruction. To our knowledge no reports of axial skeletal malformations concomitant to PRAA have been reported in cats. The aim of this study is to depict a new clinical feature in cats affected by PRAA. In the study six cats with a diagnosis of vascular ring anomaly were enrolled. A complete physical examination, a neurological examination and a total body radiograph were performed on each animal. Four of the six cats showed contemporary PRAA and skeletal malformations. Ad

Foreign body oesophageal obstruction is a medical emergency. It may be accidental, particularly in children, or deliberate, for example with suicide attempts. We present two cases illustrating accidental oesophageal foreign body impaction occurring after consumption of food that had been heated in a plastic container in a microwave oven, then cut and eaten directly from the softened container. To date, we are not aware of any similar reports. In view of potential complications, care needs to be taken when food is eaten directly from plastic takeaway containers.

Oesophageal stenting for palliation of malignant obstruction with self-expanding metallic stents is undoubtedly well established. Numerous studies have proven it to be safe and easy to perform with good clinical outcomes. However, stenting of the upper oesophagus is still regarded with apprehension, with many endoscopists concerned about cough, significant throat discomfort or intolerable foreign body sensation. Owing to relatively lack of experience of stenting in the upper oesophagus, many patients ended up with feeding jejunostomy instead. Endoscopic laser boring or dilatation of the upper

Achalasia is characterised by the loss of peristaltic movement in the distal oesophagus and failure of the lower oesophageal sphincter relaxation, which results in impaired oesophageal emptying. We report a case of a 92-year-old frail woman with a history of achalasia, who presented with acute oesophageal obstruction due to impaction of a large amount of food material. She was treated successfully with nifedipine, in combination with Coca-Cola (original product, not sugar free), so avoiding the risks associated with repeated endoscopic intubation and piecemeal removal of the oesophageal conten

BACKGROUND: Foreign bodies in the esophagus in children are usually swallowed accidentally during play. An oesophageal foreign body in neonate is rare and thus the diagnostic work-up may be difficult.
CASE REPORT: We present a case of an 8-day-old baby with symptoms of oesophageal obstruction. An unusual foreign body was found.
CONCLUSIONS: Foreign bodies in neonate's esophagus are rare; non-accidental action should be suspected. Atypical clinical and radiological symptoms may cause diagnostic difficulties.
