Causal immune-inflammation mapping of the GERD-Barrett-adenocarcinoma cascade identifies FGF19-HLA-DR⁺ T-cell axis driving esophagogastric junction metaplasia.

BACKGROUND: Gastroesophageal reflux disease (GERD), Barrett's esophagus (BE), and esophageal adenocarcinoma (EAC) form a recognized pathological continuum, but the causal immune and inflammatory processes driving progression remain incompletely defined. METHODS: Summary-level genome-wide association study (GWAS) data were analyzed for 91 circulating inflammatory proteins (n = 14,824), 731 immune cell phenotypes (n = 3,757), and three esophageal diseases (GERD, BE, and EAC). Bidirectional and two-step Mendelian randomization (MR) were used to infer causal effects and mediation, with Cochran's

Transformer-based InsightGWAS improves GERD genetic discovery via pretraining on GWAS for major depressive disorder.

Gastroesophageal reflux disease (GERD) is a highly prevalent gastrointestinal disorder with complex genetic underpinnings. While genome-wide association studies (GWAS) have identified several GERD-associated loci, traditional GWAS approaches rely on stringent significance thresholds and may miss variants with modest effects that still contribute to disease biology. To enhance the discovery of GERD-associated loci, we developed InsightGWAS, a Transformer-based deep learning model. Using transfer learning, the model was pre-trained on major depressive disorder GWAS data and fine-tuned with GERD G

Evaluating Long-Term Functional and Symptomatic Outcomes of Pneumatic Dilatation in Achalasia: An Experience from Pakistan.

Achalasia, a rare esophageal motility disorder, is managed with medications, botulinum toxin, pneumatic dilation (PD), or surgery. In resource-limited settings like Pakistan, PD is preferred first-line non-surgical treatment due to its accessibility and cost-effectiveness so we aim to assess remission rates and clinical outcomes of PD.Between 2015 and 2024, 213 achalasia patients were diagnosed via HRM at Center for Liver & Digestive Diseases, Holy Family Hospital, Rawalpindi. 191 patients opted for PD and completed follow-up interviews using Eckardt score and ASQ questionnaire. Diagnostic Performance and Reproducibility of the CARS Endoscopic Score in Achalasia: A Systematic Review and Meta-Analysis.

BACKGROUND AND AIM: Achalasia's insidious onset and nonspecific presentation contribute to diagnostic delays, often exceeding 20 months from symptom onset to confirmation. The four-domain CARS endoscopic score (Content, Anatomy, Resistance, Stasis) may expedite risk stratification. We performed a meta-analysis to assess diagnostic accuracy, interobserver reliability, and treatment-monitoring potential. METHODS: We searched PubMed, Embase, Web of Science, the Cochrane Library, and ClinicalTrials.gov through June 2025. Study-level 2 × 2 contingency data were then synthesized using bivariate ran

The Use of Four-Dimensional Impedance Manometry in Predicting Therapeutic Outcomes of Peroral Endoscopic Myotomy for Achalasia.

BACKGROUND: Clinical non-response to peroral endoscopic myotomy (POEM) is a major clinical challenge of achalasia. Four-dimensional high-resolution impedance manometry (4D-HRIM) enables evaluation of both esophageal motility and bolus clearance. This study investigated whether 4D-HRIM could predict outcomes of POEM in achalasia patients. METHODS: Achalasia patients who underwent preoperative 4D-HRIM evaluation and POEM were retrospectively included. The baseline characteristics, myotomy metrics, traditional preoperative examinations metrics, and 4D-HRIM metrics were compared between clinical

Machine Learning Algorithm Applied to pH-Impedance Monitoring Tracings Identifies Pathologic Supragastric Belching.

BACKGROUND AND AIMS: Supragastric belches (SGB) are not detected by commercial software and are cumbersome to identify manually. We determined whether machine learning can accurately identify SGBs. METHODS: pH-impedance studies from a convenience cohort (20 patients each of pathologic GERD, excessive SGB, and healthy volunteers) were randomly assigned to training (2), validation (4), and testing (4) from each group. First, studies were manually reviewed to mark SGBs, then converted to .csv files and input into a convolutional neural network (CNN) developed de novo using open-source software.
