Proximal Esophageal Impedance Contour Predicts Increased Reflux Burden in Patients With Laryngopharyngeal Symptoms.

INTRODUCTION: The value of esophageal baseline impedance (BI) in assessing proximal reflux and laryngopharyngeal symptoms (LPSs) is unclear. METHODS: Two hundred eighteen patients with LPS underwent 24-hour combined hypopharyngeal-esophageal impedance-pH monitoring. Proximal/distal BI was obtained, and a slope-and-intercept model of proximal BI contour was constructed. RESULTS: Proximal BI correlated with proximal/pharyngeal reflux (r = -0.21, P < 0.01) and reflux symptom index (r = -0.14, P = 0.08). The proximal BI contour model incorporating both the BI change (slope) and BI just below upp

Characteristics and predictors of technically difficult peroral endoscopic myotomy in esophageal motility disorders: a high-volume center analysis of 676 procedures.

BACKGROUND: As peroral endoscopic myotomy (POEM) indications expand, proactive identification and management of technically challenging cases are critically important. This study comprehensively evaluated characteristics, perioperative outcomes, and predictors of technically difficult POEM. METHODS: In this retrospective cohort study (2011-2023), consecutive POEM procedures for esophageal motility disorders at a tertiary center were analyzed. Technically difficult POEM was defined as procedure time ≥ 90 min or aborted POEM. Characteristics and operation parameters were compared between the te

Enhancing Quality of Life of Proximal Gastrectomy: A Comprehensive Review of Anastomotic Innovations and Challenges.

BACKGROUND: In recent years, the incidence of early upper gastric cancer has increased, leading to a wider adoption of proximal gastrectomy (PG) as a possible treatment option. PG is preferred over total gastrectomy (TG) due to its superior postoperative nutritional status and improved surgical safety. However, PG is associated with a considerable risk of anastomosis-related complications, such as gastroesophageal reflux and anastomotic stenosis, which limit its widespread application. METHODS: We conducted a narrative review of comparative studies, prospective studies and retrospective study

Reduced esophagogastric junction contractile integral in patients with ineffective esophageal motility.

BACKGROUND: Ineffective esophageal motility (IEM) is observed in patients with gastroesophageal reflux disease (GERD) and healthy individuals, yet its underlying pathogenesis remains poorly understood. The esophagogastric junction contractile integral (EGJ-CI) is a recently introduced metric designed to assess the contractile function of the esophagogastric junction (EGJ) barrier. This study aimed at assessing the function of the EGJ complex in patients with IEM to understand its role in disease pathogenesis. METHODS: Total 157 participants who underwent high-resolution manometry (HRM) were s

High-Resolution Impedance Manometry (HRIM) Using Yogurt (Viscosity Matched to Barium) for Bolus Clearance After Therapy in Achalasia.

OBJECTIVE: Timed-barium esophagogram (TBE) is established test for assessing bolus clearance in achalasia post-treatment. However, radiation exposure is an important disadvantage. Esophageal bolus transit can be evaluated by HRIM. Previous studies comparing Impedance bolus height (IBH) using saline and TBE showed fair correlation. We compared bolus clearance by TBE versus HRIM using yogurt (viscosity matched to barium-calculated using Ostwald viscometer) in patients treated for achalasia. METHODS: HRIM was performed using solid-state system (MMS, Netherlands). Twenty patients (age 45.1 years;
