What Is the Significance of Surgeons Possessing a Thorough Understanding of Esophageal Function Tests?

Esophageal function tests (esophageal manometry and 24-hour pH monitoring) are essential to diagnose benign esophageal disorders such as gastroesophageal reflux disease and achalasia. Unfortunately, these tests are rarely taught during medical school or surgical training, so that surgeons who treat these diseases must rely on the interpretation and conclusions made by other physicians, mostly gastroenterologists, just reading the report without looking at the tracings themselves. Potentially, this approach may lead to poor outcomes or even malpractice.

Determinants of high chronic obstructive pulmonary disease assessment test scores according to airflow limitation severity: a three-year prospective study.

BACKGROUND: The determinants of chronic obstructive pulmonary disease (COPD) assessment test (CAT) scores according to airflow limitation severity are not fully understood. METHODS: We aimed to elucidate this by analyzing data from 378 patients with COPD, categorized into two groups based on airflow limitation severity (FEV1 > 50% [GOLD-mild] and ≤ 50% [GOLD-severe]). Within each severity group, patients were further sub-stratified by CAT score (≥ 10 vs. <10). RESULTS: Patients with CAT scores ≥ 10 in the GOLD-mild group were older, with more severe airflow limitation and higher prevalence of

Patient Experience, Gender, Age, Race, and Social Determinants of Health.

Women are disproportionately affected by disorders of gut-brain interaction (DGBI), including esophageal, gastroduodenal, bowel, and anorectal conditions, and frequently experience overlapping pain syndromes. This review synthesizes clinical and preclinical evidence on sex- and gender-related differences in DGBI, highlighting alterations in central pain processing, autonomic regulation, stress, microbiota composition, epithelial permeability, motility, genetic and epigenetic factors, and immune responses that may underlie observed disparities. Emerging data indicate that gonadal hormones, inte

Impact of PAR2 on Tight Junctions and Esophageal Mucosal Inflammation in Rats with Gastroesophageal Reflux Disease.

BACKGROUND: Gastric content reflux activates protease-activated receptor 2 (PAR2), which is a critical step in the pathophysiological mechanisms underlying gastroesophageal reflux disease (GERD). Claudins serve as the primary constituents of tight junctions (TJs). They are in charge of the barrier and polarity of epithelial cells, and the disruption of their function can lead to chronic inflammation. The present research is designed to investigate whether PAR2 influences TJs in the development of GERD. METHODS: GERD models were established both in vivo and in vitro. Rabeprazole and/or GB-88 (

Sleeve Gastrectomy with Fundoplication in Patients with Gastroesophageal Reflux Disease: A Prospective Observational Study with 5-Year Follow-Up.

INTRODUCTION: Sleeve gastrectomy (SG) is currently the most commonly performed bariatric procedure worldwide. However, the development or persistence of gastroesophageal reflux disease (GERD) remains a significant concern, particularly in patients with preexisting reflux symptoms. In selected bariatric patients affected by symptomatic GERD, alternative surgical strategies may be required. This prospective observational study aimed to evaluate the long-term outcomes of SG combined with Rossetti fundoplication (SGRF) in a selected population of patients with morbid obesity and documented GERD.

Immediate discontinuation versus gradual tapering of proton pump inhibitors in long-term users: a randomized controlled trial.

BACKGROUND: Proton pump inhibitors (PPIs) are often continued after symptoms have improved, despite no clear indications for maintenance therapy. Although unnecessary long-term PPI use should be avoided, concerns about symptom rebound after discontinuation frequently discourage withdrawal; evidence comparing discontinuation strategies remains insufficient. METHODS: We conducted a multicenter, randomized, parallel-group trial comparing immediate discontinuation and gradual tapering of PPIs, including PCABs, in clinically stable outpatients with gastroesophageal reflux disease receiving long-te

[Strategies and innovative practices of European minimally invasive surgery for gastroesophageal reflux disease].

Gastroesophageal reflux disease (GERD) is a common esophageal disorder worldwide and can progress to erosive esophagitis, Barrett's esophagus, and even esophageal adenocarcinoma. Long-term use of proton pump inhibitors cannot fundamentally correct the functional impairment of the anti-reflux barrier, and symptom control is inadequate in approximately 40% of patients. Exploring individualized, standardized, and innovative anti-reflux surgical strategies is conducive to enhancing therapeutic efficacy and improving patient prognosis. Based on existing literature and European clinical practice exp

Is pneumatic dilatation still needed for the treatment of achalasia?

BACKGROUND: The uncertainty regarding its long-term effectiveness and perforation rates sometimes reaching 4% have led centers to abandon pneumatic dilatation (PD). We evaluated the efficacy and safety of a treatment strategy based on stepwise pneumatic dilations (30, 35 ± 40 mm) in real-world practice among patients with achalasia. METHODS: This was a retrospective study including consecutive, treatment naive, achalasia patients treated with PD at a single center. The study was based on a prospectively maintained database. The primary endpoint was the rate of clinical remission, defined as a

Attitudes toward the use of complementary and alternative medicine in children with gastrointestinal symptoms, a multicenter survey study among parents and pediatricians.

UNLABELLED: Disorders of gut-brain interaction (DGBI) are common in children and often persist despite conventional treatments, leading families to seek complementary and alternative medicine (CAM). However, evidence for CAM in pediatric DGBI is scarce. This multicenter survey study assessed parents' and pediatricians' experiences with and attitudes toward CAM for children with gastrointestinal symptoms. Parents and pediatricians of children (0-18 years) with gastroesophageal reflux disease (GERD) and DGBI (infant colic (IC), functional abdominal pain, and functional constipation) from six Dut
