Outcomes of conversion from sleeve gastrectomy to Roux-en-Y gastric bypass for the treatment of gastroesophageal reflux disease.

BACKGROUND: Sleeve gastrectomy (SG) is the most commonly performed bariatric procedure in the United States, but it is associated with an increased risk of de novo or worsened gastroesophageal reflux disease (GERD). Conversion to Roux-en-Y gastric bypass (RYGB) is considered the gold standard for treating refractory GERD after SG, though objective outcomes remain under-reported. OBJECTIVES: To evaluate the efficacy of conversion from SG to RYGB for medically refractory GERD using objective measures of improvement. Secondary objectives included assessing whether weight loss, additional anatomi

Prevalence of CYP2C19 phenotypes in patients undergoing a hiatal hernia repair.

OBJECTIVES: Gastroesophageal reflux disease is primarily treated with proton pump inhibitors (PPIs), which are metabolized by cytochrome P450 2C19 ( CYP2C19 ) in the liver. CYP2C19 polymorphisms affect PPI plasma levels, with rapid (2-27%) and ultra-rapid (<1-5%) metabolizers needing higher doses, while poor (3-15%) and intermediate (27-47%) metabolizers require lower doses for therapeutic effectiveness. Antireflux surgery is recommended for patients refractory to medical therapy or with symptomatic hiatal hernias. METHODS: This is a multisite retrospective review of adult patients from 2012

The role of gene polymorphisms in the occurrence of gastroesophageal reflux disease: A systematic review and meta-analysis of genetic association studies.

BACKGROUND: Gastroesophageal reflux disease (GERD) is a prevalent gastrointestinal disorder with multifactorial etiology, including genetic components. Despite numerous genetic association studies (GAS), the role of specific gene polymorphisms in GERD susceptibility remains unclear due to inconsistent findings. AIMS: To systematically review GAS on GERD and conduct a meta-analysis to evaluate the association between specific gene polymorphisms and GERD risk. METHODS: A systematic review of PubMed, Cochrane, ScienceDirect, Scopus, DisGENET, GWASCatalog, and HuGE Phenopedia databases (2012-202

Incidence and Predictors of Reflux Esophagitis Following Per-oral Endoscopic Myotomy: A Two-Year Prospective Study.

BACKGROUND AND AIMS: Peroral endoscopic myotomy (POEM) is a highly effective treatment option for achalasia cardia. However, lack of an anti-reflux mechanism predisposes patients to gastroesophageal reflux disease (GERD). This study was undertaken to assess the incidence of reflux esophagitis two years post POEM and to identify predictive factors associated with GERD development. METHODS: Consecutive patients who underwent POEM at our institution between July 2021 and December 2021 were prospectively studied to assess the incidence and predictors of GERD. Subjective GERD was evaluated using a

Applications on proximal gastrectomy with gastroesophageal muscular flap anastomosis in laparoscopic surgery: a narrative review.

In recent years, the detection rate of early upper gastric cancer has increased substantially, leading to improved overall patient prognosis and elevating postoperative quality of life to a key clinical priority. Consequently, function-preserving proximal gastrectomy has emerged as a promising surgical strategy. With the widespread adoption of minimally invasive techniques, laparoscopic proximal gastrectomy has become the standard approach; furthermore, the integration of robotic-assisted surgery has further refined the precision and feasibility of minimally invasive proximal gastrectomy. Comparing safety and short-term outcomes of proximal gastrectomy with double-flap technique and other reconstructions for proximal gastric cancer: a systematic review and meta-analysis.

BACKGROUND: Proximal gastrectomy (PG) for proximal gastric cancer (PGC) is associated with complications such as gastroesophageal reflux. The double-flap technique (DFT) has been proposed as an effective anti-reflux reconstruction method. This systematic review aims to compare the safety and short-term outcomes of DFT versus other reconstruction methods for proximal gastrectomy. METHODS: The present meta-analysis was conducted, following PRISMA guidelines. Studies comparing DFT with other reconstruction methods for proximal gastric cancer were included. Outcomes assessed included surgical par

Best hypnotic drug choice for rapid sequence induction in the operating room: study protocol for a 3-arm superiority open-labeled randomized controlled trial with blinded evaluation of the primary outcome (the HyPnotiKs study).

BACKGROUND: Propofol is the most frequently used hypnotic during rapid sequence induction in patients at risk of pulmonary aspiration of gastric contents worldwide. Its most common side effect is to induce hypotension due to dose-dependent vasoplegia. Ketamine is an alternative with pharmacological hemodynamic stability but a higher risk of postoperative delirium. Ketofol, an equimolar combination of these two hypnotics, has recently been proposed in this setting. The objective of this study is to demonstrate the superiority of ketamine (or ketofol) compared to propofol, in association with a

Delayed gastric emptying induced by glucagon-like peptide-1 receptor agonists and its implications for perioperative risk during anesthesia.

Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are increasingly prescribed for managing type 2 diabetes mellitus and obesity because of their glucose-lowering, weight-reducing, and cardiovascular benefits. However, their potential to delay gastric emptying has raised growing concern in perioperative care, particularly regarding the risk of pulmonary aspiration during general anesthesia. Recent studies have shown that patients treated with GLP-1 RAs may retain considerable gastric contents even after standard preoperative fasting, thereby increasing the likelihood of regurgitation and as

Genetic evidence for associations between gastroesophageal reflux disease and site-specific extraesophageal cancers: A Mendelian randomization study.

Observational studies have suggested an increased risk of extraesophageal cancers in patients with gastroesophageal reflux disease (GERD), but whether these associations are causal remains uncertain. This study systematically evaluated the potential associations between genetically predicted GERD and 22 extraesophageal cancers. Genetic instruments for GERD were obtained from a large genome-wide association study including 129,080 cases and 473,524 controls. Outcome data for 22 site-specific extraesophageal cancers were obtained from multiple genome-wide association study datasets, including th

Outcomes of peroral endoscopic myotomy in patients with cirrhosis: a multicenter experience.

BACKGROUND AND AIMS: Patients with cirrhosis and esophageal motility disorders are frequently excluded from peroral endoscopic myotomy (POEM) because of concerns related to portal hypertension and bleeding risk. As a result, data on POEM in this population are limited. We describe a multicenter experience evaluating the feasibility and short-term outcomes of POEM in patients with cirrhosis. METHODS: We retrospectively reviewed patients with cirrhosis who underwent POEM at 6 U.S. tertiary centers between 2020 and 2024. Technical success was defined as completion of the myotomy without conversi

Feasibility of Small Bowel Length Shortening with Sleeve Gastrectomy for Metabolic and Nutritional Complications After RYGB.

INTRODUCTION: Metabolic and nutritional complications-including intractable dumping syndrome (DS), gastroesophageal reflux disease (GERD), anemia, and weight recurrence-may develop long after Roux-en-Y gastric bypass (RYGB). Current revisional options, such as reversal or distalization, can be technically demanding and may worsen malabsorption. Restoring gastric continuity and pyloric function may offer physiological benefits. This study evaluated the feasibility and early outcomes of small bowel length optimization and shortening with sleeve gastrectomy (BOS-SG) as a revisional strategy after

Gastroesophageal Reflux Disease After Sleeve Gastrectomy versus Roux-en-Y Gastric Bypass: An 8-Year Prospective Cohort Study.

BACKGROUND: The long-term impact of bariatric procedures on gastroesophageal reflux disease (GERD) remains insufficiently characterized. Sleeve gastrectomy (LSG) is technically simpler but may predispose patients to reflux, whereas Roux-en-Y gastric bypass (LRYGB) is recognized for its durable anti-reflux effect. Robust prospective data comparing the procedures beyond 5 years are scarce. This study compared 8-year persistent or new-onset GERD after LSG versus LRYGB using multimodal assessment based on Lyon Consensus 2.0 criteria. METHODS: In this prospective dual-cohort extension of a previou
