Congenital esophago-bronchial fistula with diverticulum is rare, and in Japan only 29 cases have been reported. A 59-year-old man had suffered from recurrent pulmonary infections and often complained of coughing while drinking water during the previous two years. An UGI series for follow-up of gastric ulcer revealed an esophago-bronchial fistula, located between the mid-esophagus and B10 of the right lung. Pharyngoesophageal diverticulum is an acquired defect resulting from an incoordination of the cricopharyngeal muscle. Common symptoms are dysphagia, regurgitation, and aspiration. The defect is repaired through a cervical incision and should include a diverticulectomy and a myotomy. Results are excellent and complications are unusual.

We report an unusual case of a pharyngeal diverticulum arising as a result of extrusion of a bone graft inserted during anterior cervical fusion operation. The dense scar tissue at the site of extrusion of the bone graft probably caused traction on the posterior pharyngeal wall, with the development of a pharyngeal pouch. This is believed to be the first case with acquired pharyngeal diverticulum following anterior cervical fusion to be recorded in the literature.

The case of a 52-year-old woman with an oesophagobronchial fistula is presented. The diagnosis was not made preoperatively. The patient was operated twice, with resection of a diverticulum of the oesophagus at the last operation. The congenital or acquired origin of the fistula is discussed.

In connection with an esophageal diverticulum the case of an infrequent acquired esophago-bronchial fistula appearing in symptom-poor form is described. The fistula persisted over a long period however owing to the poor symptoms and absence of pulmonary complications operation was left out of consideration. In the course of the regular control the complaints of the patient ceased and the endoscopy showed shrinking of the fistula. No similar observation was found in the Hungarian literature.

Pharyngoesophageal (Zenker's) diverticulum is a relatively common acquired condition that may cause dysphagia and regurgitation of food. Squamous carcinoma may develop in such a diverticulum, but this complication is exceedingly rare and may not be suspected preoperatively. This report describes a case in which the diagnosis of squamous carcinoma was made after regurgitation of malignant tissue from a radiographically proven pharyngoesophageal diverticulum. The patient was treated by simple diverticulectomy and postoperative radiation therapy.

Cases of acquired benign fistula of the respiratory tree and esophagus have been reported in relation to Mycobacterium tuberculosis and trauma. Fistula formation between the trachea and a Zenker's diverticulum is a similar type of condition in which nutritional and immunological deficiencies can rapidly result in further complications which may mimic a malignancy. Spontaneous fistula formation and vocal cord paralysis may result. Surgical management of this condition includes interposing viable muscle between the esophagus and trachea at the repair site.

A case report of a child with an acquired oesophageal pouch is presented. A chronically impacted radio-lucent coin in the oesophagus resulted in a dilatation of the oesophagus above the obstruction. This dilatation mimicked a pharyngeal pouch and the problems of diagnosis are discussed.

Two patients with intraluminal esophageal diverticulum are described and illustrated. Both had chronic esophagitis. One had a distal esophageal stricture, while the other showed persistent retrograde gastroesophageal reflux following hiatal hernia repair. Possible causes are considered, based on the presumption of intermittently or chronically increased intraluminal pressure associated with an area of congenital or acquired weakness of the esophageal wall.
