Association of Gallstone Disease and Cholecystectomy With Duodenal Polyps and Barrett's Oesophagus.

BACKGROUND AND AIM: We hypothesized that duodenal polyps (DP) and Barrett's oesophagus (BE) coincide in the same patients and that biliary disease, such as cholelithiasis (GS) or cholecystectomy (CCx), increases the risk for both DP and BE. We used a case-control study design to test our hypothesis. METHODS: Using the healthcare records of the University of Pennsylvania Health System (UPHS), we compared the frequency of GS or CCx in patients with DP or BE with their corresponding frequencies in a control population of patients without DP or BE. Odds ratios (OR) and their 95% confidence interv

Can bile salts affect the contractile oesophageal activity associated with gastroesophageal reflux disease?

Bile salts are essential molecules that have evolved beyond their digestive surfactant properties to act as relevant pathophysiological signalling modulators. In the gastroesophageal reflux disease (GERD), recurrent exposure of the oesophagus to harmful agents of gastric and duodenal origin, including bile salts, results in chronic inflammation and damage to the oesophageal mucosa, ultimately promoting the progression of the normal oesophageal epithelium to pre-malignant or malignant lesions, such as Barrett's oesophagus and adenocarcinoma. Although the acidity of the refluxed material has lon

Basal crypt dysplasia in Barrett's oesophagus: ready for prime time?

Barrett's oesophagus (BE) continues to rise in prevalence alongside oesophageal adenocarcinoma; understanding and identifying early neoplastic changes is critical. Crypt dysplasia (CD) in BE is an emerging concept characterised by dysplasia confined to the crypt base without surface involvement. Recent studies suggest that CD shares molecular alterations with low-grade and high-grade dysplasia, such as TP53 mutations and chromosomal instability, and may represent an early phase of neoplasia. Grading of CD remains inconsistent, with limited correlation to clinical outcomes, although classifying

Clinical significance of hiatus hernia on Barrett's oesophagus: a scoping review.

BACKGROUND: Hiatal hernia (HH) is commonly observed in patients with Barrett’s oesophagus (BO), a premalignant condition that may progress to oesophageal adenocarcinoma (OAC). While HH has been implicated in BO pathogenesis, it is not formally recognised as a major risk factor in leading international clinical guidelines. This systematic scoping review aimed to evaluate the role of HH in the development, progression, and treatment outcomes of BO. METHODS: A systematic scoping review was conducted, searching through databases (PubMed, Medline, Embase, Scopus). RESULTS: A total of sixty-six arti

Lymphocyte Micronucleus Formation Is Driven by Inflammation-Induced Oxidative DNA Damage in Oesophageal Cancer Development.

We investigated mechanisms underlying circulating DNA damage across the gastro-oesophageal reflux disease (GORD), Barrett's oesophagus (BO) and oesophageal adenocarcinoma (OAC) spectrum using the lymphocyte micronucleus (MN) assay. MN frequency (MN%) was quantified in lymphocytes from healthy volunteers (HVs), GORD, BO and OAC patients, alongside plasma biomarkers of inflammation and oxidative stress. Ex vivo challenge assays assessed lymphocyte responses to hydrogen peroxide (H2O2), vinblastine and the bile acid deoxycholic acid (DCA). Tissue-based NF-κB expression was also correlated with MN

Diagnostic impact and reproducibility of p53 immunohistochemistry in Barrett's oesophagus: results of the Dutch Esophageal Pathology Panel (DEPP).

INTRODUCTION: Evidence supporting the use of p53 immunohistochemistry (p53-IHC) in Barrett's oesophagus (BE) is largely based on selected series, limiting generalizability. This study evaluates p53-IHC in a nationwide, real-world BE-cohort within a universal healthcare system, assessing concordance with referring hospitals, interobserver agreement (IOA) and its impact on routine diagnostic decisions. METHODS: Revision cases submitted to the Dutch Esophageal Pathology Panel (DEPP) were assessed by trained and experienced pathologists for haematoxylin and eosin (H&E)-based classification (Vienn

Review Article: Advances in the Diagnosis of Dysplasia in Barrett's Oesophagus.

BACKGROUND: Barrett's oesophagus (BE), the precursor to oesophageal adenocarcinoma, progresses through a stepwise dysplastic sequence. Accurate dysplasia detection is critical, as endoscopic eradication therapy is highly effective. The current standard of care is limited by poor real-world adherence to the Seattle protocol, sampling error, and significant interobserver variability in pathologic interpretation. Post-endoscopy oesophageal neoplasia rates approaching 26% underscore the need for improved detection of dysplasia. AIMS: This review evaluates emerging and established technologies for

Mapping Molecular Transitions in Barrett's-Associated Oesophageal Adenocarcinoma via Multi-Omics Integration and Pathway Activity Modelling.

Background/Objectives: Interpretation of high-dimensional molecular profiling data requires computational strategies that go beyond single-layer analyses to resolve coordinated biological programs. Although multi-omics integration has advanced latent structure discovery, translating these cross-omics signals into interpretable, functionally meaningful molecular states remains a central challenge. Methods: Here, we present an interpretable, pathway-centric multi-omics integration framework that combines expression, DNA methylation, and copy-number alteration data to capture nonlinear cross-omic

Dedicated Endoscopy for Barrett's Oesophagus With Higher Dysplasia Yield May Reduce Seattle Protocol Biopsies: Results From UK Multicentre Study.

BACKGROUND: Dedicated service for Barrett's oesophagus (BO) surveillance may be more effective than conventional service, according to some single centre studies. OBJECTIVE: To determine whether the surveillance of BO through a dedicated service can improve key performance indicators (KPIs), and dysplasia detection rate (DDR) compared with conventional service in a multi centre study. METHODS: A retrospective cohort study was conducted across 6 NHS-hospitals, capturing BO surveillance data over 8 years. Factors associated with DDR were assessed by logistic regression. RESULTS: There were 10

Mechanisms underpinning the progression of nondysplastic Barrett's oesophagus to cancer.

Barrett's oesophagus is the metaplastic precursor to oesophageal adenocarcinoma; it is initiated by exposure of the squamous oesophageal epithelium to acid reflux. Wound repair mechanisms lead to the migration of columnar cells into the denuded lower oesophageal niche. These columnar cells remain exposed to inflammatory acid and bile reflux, which serves as a mutagenic pressure and induces the appearance of intestinal phenotypic features. The patchwork accumulation of genomic instability after TP53 loss provides a cancerised field where dysplastic clones can be selected for. In our review, we

Primary carE PPi dEprescRibing (PEPPER) trial: a protocol for determining the optimal strategy for stopping chronic proton pump inhibitor therapy in primary care patients.

INTRODUCTION: Current Belgian guidelines state that chronic proton pump inhibitor (PPI) therapy is indicated for oesophagitis grade C and D, Barrett's oesophagus, Zollinger-Ellison syndrome, or to prevent bleeding ulcers with chronic non-steroidal anti-inflammatory drugs (NSAID) intake in patients at risk. Guidelines justify empiric short-term PPI therapy in other cases to control symptoms. Yet, there is insufficient PPI down-titration and/or cessation. As such, concerns have risen related to the impact of PPIs on the healthcare budget and increasing number of risks and side effects. This stud

Genetic Assessment of Oesophageal Safety of GLP-1 and GIP Receptor Perturbation: A Drug-Target Mendelian Randomisation Study.

Background/Objectives: Glucagon-like peptide-1 (GLP-1) receptor agonists and dual glucose-dependent insulinotropic polypeptide (GIP)/GLP-1 receptor agonists are used long term for obesity and diabetes, but their oesophageal safety is uncertain: weight loss may reduce Barrett's oesophagus and adenocarcinoma risk, whereas delayed gastric emptying may worsen reflux. We performed a genetic assessment of target-mediated oesophageal safety. Methods: Two-sample drug-target Mendelian randomisation used cis-expression quantitative trait loci for both receptors (31,684 participants). Outcomes were Europ

Prior endoscopic surveillance and diagnosis of Barrett-related dysplasia and cancer: evidence from a nationwide Dutch cohort.

BACKGROUND: In patients with Barrett oesophagus (BE), surveillance aims to detect dysplasia or oesophageal adenocarcinoma (EAC) early. Whether most asymptomatic patients undergoing endoscopic treatment are identified through surveillance remains uncertain. OBJECTIVE: To assess the contribution of surveillance to the diagnosis of dysplasia and cancer. DESIGN: Patients in the Netherlands with curative treatment for BE-related dysplasia or EAC between 2008 and 2018 were identified from the Barrett Expert Center Registry for endoscopic therapy and the Cancer Registry for surgery or chemoradiothe
