Reflux-Related Abnormalities at Distal oesophagus, Gastric Pouch and Anastomotic Site 4 Years After OAGB: Diagnostic Accuracies of Endoscopy Compared to Biopsy and of Symptoms Compared to Both.

BACKGROUND: The purpose of the current study is to appraise the diagnostic accuracy of upper endoscopy (UE) vs histopathological assessment of patients after one-anastomosis gastric bypass (OAGB), and the presence/absence of symptoms vs these two diagnostic modalities. METHODS: Retrospective study of 50 consecutive patients who underwent OAGB during April 2019-April 2020 and consented to participate. Symptoms (symptoms score questionnaire), macroscopic and microscopic data were collected 4 years later to assess distal oesophageal, gastric pouch and anastomotic site changes. Diagnostic accurac

Barrett's Oesophagus Surveillance Versus Endoscopy at Need Study (BOSS): A Randomized Controlled Trial.

BACKGROUND & AIMS: Barrett's esophagus (BE) is a precursor lesion for esophageal adenocarcinoma (EAC). Surveillance endoscopy aims to detect early malignant progression; although widely practiced, it has not previously been tested in a randomized trial. METHODS: BOSS (Barrett's Oesophagus Surveillance Versus Endoscopy at Need Study) was a randomized controlled trial at 109 centers in the United Kingdom. Patients with BE were randomized to 2-yearly surveillance endoscopy or "at-need" endoscopy, offered for symptoms only. Follow-up was a minimum of 10 years. The primary outcome was overall surv

Infectious agents and progression from Barrett's oesophagus to oesophageal adenocarcinoma: a nested case-control study.

BACKGROUND: A causal role of high-risk HPV in oesophageal adenocarcinoma development has been hypothesised, but longitudinal evidence is limited. This study aims to investigate a potential causal role of infectious agents in the malignant progression of Barrett's oesophagus. METHODS: Using a retrospective nested case-control study design, index Barrett's biopsies were retrieved for individuals within the Northern Ireland Barrett's oesophagus register who subsequently progressed to oesophageal adenocarcinoma (n = 150) and matched non-progressors (n = 298). Index Barrett's biopsies were assesse

The future is now: advancing p53 immunohistochemistry in Barrett's oesophagus and its implication for the everyday pathologist.

Barrett's oesophagus (BE) is a precancerous condition where the normal squamous epithelium of the distal oesophagus is replaced by specialised intestinal-type columnar epithelium. Patients with BE have an increased risk of developing oesophageal adenocarcinoma (EAC). p53 is a tumour suppressor protein frequently mutated in BE, and its overexpression is detectable by immunohistochemistry (IHC) and correlates with TP53 mutations. This review summarises recent literature on p53 IHC as a diagnostic aid for dysplasia and a predictive biomarker of neoplastic progression risk in BE. While there is a

Saturated fat exacerbates mitochondrial dysfunction through remodelling of ATP production and inflammation in Barrett's oesophagus compared to monounsaturated fat, particularly in contrast to oesophageal adenocarcinoma.

Obesity-related oesophageal adenocarcinoma (OAC), arising from Barrett's oesophagus (BO), incidence rates are rising coincident with high-fat diets. However, adipose tissue phenotype drives metabolic characteristics. Prior feeding studies demonstrated that obesogenic diets enriched in saturated fatty acids (SFA) induce a more adverse metabolic and pro-inflammatory adipose phenotype, compared to monounsaturated fatty acids (MUFA) enriched high-fat diets, despite equal obesity. We hypothesise that different fatty acids may alter the progression of BO to OAC, wherein SFA may be more pathogenic co

Cost-Effectiveness of Regular Surveillance Versus Endoscopy at Need for Patients With Barrett's Esophagus: Economic Evaluation Alongside the Barrett's Oesophagus Surveillance Study (BOSS) Randomized Controlled Trial.

BACKGROUND & AIMS: The Barrett's Oesophagus Surveillance Study (BOSS) was the first randomized study of surveillance. This study reports the costs and quality of life outcomes from the BOSS trial and models the outcomes and cost-effectiveness of surveillance beyond the follow-up period of the BOSS study. This trial showed similar stages and rates of esophageal cancer in both arms, but the regular surveillance arm did identify more high-grade dysplasia after a median of 12.8 years follow-up. METHODS: We used a decision tree model based on results from BOSS to conduct a cost-effectiveness analy

Texture and Colour Enhancement Imaging versus White Light Endoscopy for Detection of Dysplasia within Barrett's Oesophagus: A Pilot Study.

UNLABELLED: <p>Introduction: Oesophageal cancer is a leading global health issue, with increasing prevalence of oesophageal adenocarcinoma and its precursor lesion, Barrett's oesophagus (BE). Despite the opportunity to treat dysplasia prior to adenocarcinoma development, rates of missed advanced dysplasia at BE surveillance remain high. This pilot study aimed to assess whether Texture and Colour Enhancement Imaging (TXI), a new advanced mucosal imaging modality, improves dysplasia detection during BE surveillance compared to white light endoscopy (WLE). METHODS: Patients undergoing endoscopy

Biomarker risk stratification with capsule sponge in the surveillance of Barrett's oesophagus: prospective evaluation of UK real-world implementation.

BACKGROUND: Endoscopic surveillance is the clinical standard for Barrett's oesophagus, but its effectiveness is inconsistent. We have developed a test comprising a pan-oesophageal cell collection device coupled with biomarkers to stratify patients into three risk groups. We aimed to prospectively evaluate the prespecified risk stratification tool to establish whether it can identify those at highest risk of dysplasia or cancer to prioritise the timing of endoscopy; and safely be used to follow up the low-risk group, thus sparing patients from unnecessary endoscopies. METHODS: Participants wer

Tethered optoacoustic and optical coherence tomography capsule endoscopy for label-free assessment of Barrett's oesophageal neoplasia.

Endoscopic detection of oesophageal cancer (EC) often occurs late in disease development, leading to high mortality rates. Improved technologies are urgently needed for earlier EC detection. Here we research an endoscopic ultra-broadband acoustic detection scheme and introduce a 360-degree hybrid optoacoustic and optical coherence endoscopy to enable interrogation of surface and subsurface precancerous and cancerous features at a three-dimensional micrometre scale. In the following pilot tissue investigation, the dual-modal imaging features are assessed for classifying different mucosal types

Importance of preoperative gastroscopy prior to bariatric surgery: a perspective from a regional centre.

We report a case of an Australian man in his early 50s, with a BMI of 42.2 kg/m2, who was being worked up for elective bariatric surgery. Initial gastroscopy was unremarkable; however, repeat preoperative gastroscopy 20 months later identified Barrett's oesophagus. Subsequent histology confirmed oesophageal adenocarcinoma. This report highlights the importance of preoperative workup of patients undergoing bariatric surgery. We advocate for preoperative gastroscopies as a part of the immediate workup prior to any bariatric procedure.

Curriculum for optical diagnosis of esophageal neoplasia and precursor lesions: European Society of Gastrointestinal Endoscopy (ESGE) Position Statement.

PRE-ADOPTION: Endoscopists performing upper gastrointestinal (UGI) endoscopy should train to recognize the appearance of early esophageal cancer and its precursor lesions, for Barrett neoplasia and for squamous neoplasia, to improve detection rates.Every endoscopist should have achieved competency in UGI endoscopy before commencing training in optical diagnosis in the UGI tract, and this requires personal experience of 300 UGI endoscopies and meeting the ESGE quality measures for UGI endoscopy. TRAINING: Endoscopists should undertake interactive training courses to improve diagnostic accuracy

Endoscopic Treatment for Early Barrett's Neoplasia: A Western Perspective.

BACKGROUND: Barrett's oesophagus (BE) is the sole established precursor to oesophageal adenocarcinoma, with striking contrasts in its epidemiology and management between the East and the West. SUMMARY: This review, based on a structured literature search, examines the key divergences in endoscopic management: highlighting the Western emphasis on eradication therapy using ablation techniques such as radiofrequency ablation, contrasted with the Eastern expertise in advanced resection techniques including endoscopic submucosal dissection. KEY MESSAGES: Despite improving patient outcomes, signif

Research Communication: Prevalence of Asymptomatic Premalignant Oesophageal Lesions in Patients With Fanconi Anaemia.

Fanconi anaemia (FA), a rare genetic disorder of DNA repair, predisposes to squamous cell carcinomas of head, neck and oesophagus. We assessed the value of screening esophagogastroduodenoscopy (EGD) in 20 asymptomatic adults (median age 23.5, 65% female) with FA, compared to age-and sex-matched non-FA patients enrolled at a 3:1 ratio. Among 11 FA patients with abnormal oesophageal mucosa, 15% had low-grade dysplasia, nodular high-grade dysplasia or squamous carcinoma in situ, 10% had non-dysplastic Barrett's oesophagus and 30% had columnar epithelium without goblet cells, compared to none in t

Cancer Burden on Piecemeal Endoscopic Resection of Early Adenocarcinoma in Barrett's Oesophagus Correlates With the Risk of Neoplastic Recurrence.

BACKGROUND AND STUDY AIMS: Endoscopic resection (ER) is curative for early-stage oesophageal adenocarcinoma (OAC) without high-risk features. Piecemeal endoscopic mucosal resection (pEMR) prevents assessment of lateral margins, complicating risk estimation for neoplastic recurrence. We investigated the risk factors for residual and recurrent OAC post-pEMR. METHODS: We performed a longitudinal study of two independent patient cohorts: the test cohort who underwent piecemeal or en-bloc ER (n = 140) and the validation cohort who underwent pEMR only (n = 89). Inclusion criteria were: OAC stage T1
