Tissue culture of epithelium derived from Barrett's oesophagus.

Barrett's oesophagus is a preneoplastic condition in which the squamous mucosa of the oesophagus is replaced by columnar epithelium. Epithelial cells of Barrett's oesophagus were isolated from resected oesophagus specimens by two methods not previously applied to the culture of Barrett's oesophagus cells. These techniques included trypsinisation of small fragments of mucosa, followed by plating in tissue culture dishes, and a direct tissue explant technique. A modified MCDB-153 growth medium was used. Primary trypsin technique cultures were plated on uncoated plastic, or plastic coated with ty

Dysplasia and aneuploidy as markers of malignant degeneration in Barrett's oesophagus. The Rotterdam Oesophageal Tumour Study Group.

The role of dysplasia and aneuploidy as markers in columnar epithelium for malignant degeneration in Barrett's oesophagus was compared in a case control study comprising 38 patients with benign Barrett's oesophagus and 50 patients with Barrett's oesophagus associated with adenocarcinoma. Tissue specimens of columnar epithelium were reviewed for the presence of specialised columnar epithelium and the grade of dysplasia. Ploidy was determined using the method for formalin fixed paraffin wax embedded tissue described by Hedley. There was no significant difference in the frequency of specialised c

Proliferating cell nuclear antigen in the assessment of Barrett's mucosa.

Proliferating cell nuclear antigen (PCNA) expression was assessed in oesophageal epithelium from 70 patients with Barrett's oesophagus. PCNA counts were expressed in the form of a PCNA index. There were 24 patients with dysplastic epithelium, nine with regenerative atypia, 18 with benign histology and 19 with adenocarcinoma arising in Barrett's epithelium. The PCNA index of malignant tissue was significantly different from that of benign (P < 0.001) and dysplastic (P < 0.05) specimens. PCNA indices in histologically normal Barrett's epithelium adjacent to a tumour were significantly different

[Barrett esophagus. Current situation of the risk and surveillance policy].

Barrett's oesophagus is a condition in which a metaplastic columnar mucosa replaces the normal squamous epithelium of the lower oesophagus. Barrett's oesophagus develops as a complication of gastro-oesophageal reflux and predisposes to the development of oesophageal adenocarcinoma. Most adenocarcinomas arising in Barrett's mucosa are far advanced at the time of diagnosis, and prognosis is consequently poor. Regular endoscopic surveillance of patients with Barrett's oesophagus is recommended to detect the oesophageal malignancy in an early presymptomatic stage. The concept of screening is based

Gastric cancer.

Most countries with adequate statistical infrastructure have registered declines in gastric cancer mortality and incidence rates. Such a trend is dominated by the most frequent variant, namely the so-called intestinal type of adenocarcinoma, usually ulcerated and occupying predominantly the antrum and the antrum-corpus junction. This variant is considered the endstage of a prolonged precancerous process with gradual progression from (a) chronic active gastritis to (b) multifocal atrophic gastritis to (c) intestinal metaplasia, first resembling the phenotype of the small intestine and later tha

Microvascular lesions in reflux oesophagitis and Barrett's oesophagus.

In 30 patients with reflux oesophagitis, the mucosal microvessels have been studied by transmission electron microscopy. The aim of the study was to test the hypothesis that a microvascular injury is involved in reflux oesophagitis, and to clarify if the epithelial metaplasia is correlated with a contemporaneous modification of the microvasculature in Barrett's oesophagus. In squamous epithelium-lined mucosa, signs of microangiopathy were found in all patients and capillaries regularly showed an interrupted, duplicated, or thickened basal lamina. In areas of columnar metaplasia, capillaries sh

Helicobacter pylori and gastro-oesophageal reflux disease--a prospective study.

In a prospective evaluation of the relationship between Helicobacter pylori infection and gastro-oesophageal reflux disease (GORD), 93 consecutive patients (47 female: 46 male: mean age, 46 years: range 13-93) with symptoms and endoscopic evidence of GORD were studied. A total of 50 patients (54%) were H. pylori-positive on gastric antral biopsies. No significant correlation was detected between H. pylori status and grade of oesophagitis. The prevalence of H. pylori infection showed a gradual increase with age. Of 64 patients with a hiatal hernia, 28 (44%) had histological evidence of H. pylor

Barrett's oesophagus: mucin composition, neuroendocrine cells, p53 protein, cellular proliferation and differentiation.

There is strong association of Barrett's oesophagus (BO) with adenocarcinoma. The sequence of events preceding malignancy appears to be reflux oesophagitis - ulceration - BO - dysplasia. One hundred and five biopsies of heterotopic columnar epithelium were stained for H&E, PAS/Alcian Blue and HID/Alcian Blue for the routine histology and neutral/acidic sialo- and sulphomucin staining. Other sections were silver impregnated by the Grimelius technique. Immunohistochemical techniques were applied for the assessment of the accumulation of p53 protein, "S" phase of the replication cell cycle using

24 hour ambulatory oesophageal pH monitoring in uncomplicated Barrett's oesophagus.

The aim of this study was to compare acid reflux in 16 patients with uncomplicated Barrett's oesophagus with 37 patients with reflux oesophagitis and 10 control subjects of comparable age. All patients had 24 hour ambulatory pH monitoring within one week of endoscopy. The controls differed significantly from the other groups in all indices of reflux but there was no difference between the Barrett's oesophagus patients and the reflux oesophagitis patients in any index of reflux over the total 24 hour period. Barrett's oesophagus patients differed significantly, however, with respect to the perc

Prevalence of metaplasia at the gastro-oesophageal junction.

Specialised columnar epithelium (SCE), a form of intestinal metaplasia usually found in Barrett's oesophagus, cannot be distinguished endoscopically from normal gastric epithelium. Endoscopists seldom obtain biopsy specimens from a normal-appearing gastro-oesophageal junction, and therefore short segments of SCE in this region may go unrecognised. We studied patients who had short segments of SCE at the gastro-oesophageal junction. All patients scheduled for elective endoscopic examinations in our general endoscopy unit, irrespective of indication, were questioned for symptoms of gastro-oesoph

Barrett's oesophagus and carcinoma. Recent insights into its development and possible prevention.

Barrett's oesophagus or columnar epithelium-lined oesophagus is a condition due to chronic gastro-oesophageal reflux. In addition to acid-peptic reflux, reflux of duodenal contents may have a role in its aetiology. The clinical importance of Barrett's oesophagus is the increased risk for development of oesophageal adenocarcinoma. Measures to prevent cancer development have so far been limited to regular screening and early oesophagectomy in the case of severe dysplasia or early cancer. Other modes of intervention, directed toward prevention of dysplasia and possibly regression of Barrett's epi

Carcinosarcoma in Barrett's oesophagus: a case report with immunohistological examination.

A case of a carcinosarcoma which developed in a Barrett's oesophagus is presented. The tumour consisted of an adenocarcinoma and a spindle cell sarcoma. Immunohistological examinations demonstrated vimentin positivity in the sarcomatous portion with a negative reaction for keratin. Immunohistological and histological findings did not rule out the possibility of a double or collision tumour in this case.

Malignant Barrett's oesophagus.

Barrett's oesophagus is a pre-malignant condition with an increased risk of adenocarcinoma. The prevalence of adenocarcinoma in Barrett's oesophagus is about 10% but its true incidence in the general population is unknown. The development of adenocarcinoma in Barrett's oesophagus is a multi-step process. Gastro-oesophageal reflux symptoms are absent in many Barrett's patients and both Barrett's oesophagus and adenocarcinoma are usually but not always diagnosed simultaneously. When a carcinoma is identified, the treatment of choice is resection. Three-stage oesophagectomy is considered the most

Surgical treatment of reflux stricture of the oesophagus.

The choice of surgery in patients with reflux-induced oesophageal stricture remains controversial. From 1976 to 1990, a total of 65 patients underwent fundoplication (36 patients), Collis gastroplasty plus fundoplication (ten), total duodenal diversion (four) and oesophageal resection (15). The postoperative mortality rate was 5 per cent (three patients): necrosis of the colon transplant in two patients and acute pancreatitis in one. The median follow-up was 25 (range 6-120) months. After conservative surgery, the median number of dilatations per patient per year significantly decreased (P < 0
