Regulatory peptides in Barrett's oesophagus.

Barrett's epithelium refers to the presence of ectopic mucosal types in the squamous-lined oesophagus. Previous studies have documented argentaffin and argyrophil-positive cells as well as gastrin-like immunoreactivity in oesophageal tissue extracts from patients with Barrett's mucosa. In the present study, 125 oesophageal biopsies obtained under direct vision at endoscopy from 22 patients with Barrett's oesophagus were systematically studied using fluorescence and peroxidase antiperoxidase single and double-staining immunocytochemical methods employing highly specific antibodies to localize t

[Which hiatal hernias require surgery?].

Several clinical situations may justify surgery for hiatal hernia. The approach varies depending on whether the problem is a mechanical complication or a gastro-oesophageal reflux responsible for oesophagitis, peptic stenosis, chest pain, respiratory disorders or Barrett's oesophagus. Recurrence of reflux after surgery raises even more complex problems. Evaluation by modern exploratory techniques helps in establishing precise indications for the operation which must, be reserved to selected cases.

Partial gastrectomy with Roux-en-Y reconstruction in the treatment of persistent or recurrent oesophagitis after Nissen fundoplication.

The results of partial gastrectomy with Roux-en-Y reconstruction in the treatment of persistent or recurrent oesophagitis after failed Nissen fundoplication in six patients were analysed. There were no postoperative deaths. Postoperative complications (pneumonia and atelectases, postoperative ileus) developed in three patients. The results were evaluated by clinical and endoscopic examination 1 to 3.5 years after the operation. Clear clinical and endoscopic improvement was observed in five of the six patients. In the remaining patient the procedure failed to prevent progression of the oesophag

Barrett oesophagus: analysis of 19 cases.

This article reviews our experience with 19 cases of Barrett oesophagus. Radiological findings included hiatus hernia, gastro-oesophageal reflux, strictures and oesophagitis. The strictures were most commonly distal. The recently described lace-like reticular pattern of the mucosa was noted in one case of the 15 who had air-contrast views. It was associated with an adjacent stricture. The low incidence of this radiological finding emphasises that Barrett oesophagus is a histological diagnosis. Six patients had only the non-specific and common radiological findings of reflux with or without hia

Mucin histochemistry of the columnar epithelium of the oesophagus (Barrett's oesophagus): a prospective biopsy study.

Columnar epithelium-lined oesophagus (CELO) is an acquired disorder associated with a high incidence of cancer. CELO consists of three histological types of epithelium: gastric-fundic, junctional, and specialised columnar, the last resembling intestinal metaplasia of the stomach. In a previous study of CELO an incompletely differentiated variant of intestinal metaplasia secreting sulphomucins (type II B) was found. This was shown to be associated with well differentiated adenocarcinoma, as in the stomach. The purpose of this paper has been to define by histochemistry the mucin profile of CELO

Adenocarcinoma in Barrett's oesophagus. Two case reports and discussion.

Two cases of adenocarcinoma in columnar epithelium-lined oesophagus are reported. Two discrete carcinomas were found in one case, and areas of epithelial atypia were demonstrated in regions devoid of tumour in both. Similar observations in the literature together with statistical evidence indicate that Barrett's oesophagus should be regarded as a premalignant condition. Its clinical implications are outlined.

Columnar-lined oesophagus (Barrett's syndrome) - congenital or acquired?

A segment of the distal oesophagus lined with columnar epithelium (endobrachy oesophagus, Barrett's oesophagus), is very frequently seen in association with chronic reflux conditions. Usually, this entity is an asymmetrical columnar-cell metaplasia of varying extent, with an unsharp epithelial junction line cranial, and showing various large residual islands of squamous epithelium. In addition, usually well-developed peptic lesions are also found. More rarely seen are cases with a longer, symmetrical, cranially sharply delimited columnar-epithelial-lined segment, in which peptic lesions and st

Mucin histochemistry of the columnar epithelium of the oesophagus: a retrospective study.

Reflux oesophagitis may be accompanied by replacement of squamous epithelium by columnar epithelium (Barrett's oesophagus). The columnar epithelium may resemble either gastric or intestinal mucosa, though ultrastructural and histochemical studies indicate that the intestinal type does not necessarily resemble normal small intestine. Twenty-two malignant and 10 benign oesophageal specimens were examined histologically and by mucin histochemistry in an attempt to classify the various types of columnar epithelium. An incompletely differentiated variant of intestinal metaplasia secreting sulphomuc

Effect of long-term treatment with cimetidine and antacids in Barrett's oesophagus.

The effect of a long-term treatment (one to two years) with cimetidine (1.6 g per day) and an antacid (Regla pH) was evaluated in nine patients with a Barrett's oesophagus. The results showed that such long-term treatment had a beneficial effect on the symptoms and endoscopic signs of oesophagitis and on the healing of a Barrett's ulcer, but did not result in a regression of the squamocolumnar junction back towards the cardia. No significant changes were observed in the histological epithelial types in the biopsies taken below the squamocolumnar junction. No clinical or biochemical side-effect

Non-invasive investigation of the upper gastrointestinal tract using technetium-99m.

The use of technetium-99m in the noninvasive investigation of the upper gastrointestinal tract is discussed with particular reference to the evolution of a method of assessing gastric function or gastric acid secretion non-invasively and to the applications of this method in the investigation of surgical patients with disease of the upper gastrointestinal tract. The assessment of maximal acid output and the insulin response is described and the use of the test in the diagnosis of pernicious anaemia, hypo- and hyperchlorhydric states, gastric cancer, hiatus hernia and Barrett's oesophagus, coel

Primary adenocarcinoma of the upper oesophagus.

Adenocarcinoma of the upper one-third of the oesophagus is an uncommon entity. A review of the literature reveals 20 reported cases. This paper reports an additional case. The lesion may arise from oesophageal glands, foci of ectopic gastric mucosa, or a Barrett's oesophagus. In contrast with squamous cell carcinomas submucosal spread is not prominent; however, transverse penetration through the oesophageal wall is common. The prognosis is grave. Treatment modalities that have been employed include supportive care, surgery alone, pre-operative radiation therapy and surgery and radiation therap

Adenocarcinoma in a Barrett oesophagus.

A case of adenocarcinoma developing at the squamocolumnar epithelial junction of a Barrett oesophagus is reported. This rare tumour was remarkable because of the youth of the patient and because of the signet-cell cytological pattern of the neoplasm. It is postulated that both the columnar epithelial lining of the lower part of the oesophagus and the malignant change are a consequence of long-standing oesophageal reflux.

Adenocarcinima in the columnar epithelial lined lower (Barret) oesophagus.

Hawe, A., Payne, W. S., Weiland, L. H., and Fontana, R. S. (1973).Thorax, 28, 511-514. Adenocarcinoma in the columnar epithelial lined lower (Barrett) oesophagus. At the Mayo Clinic, five patients with Barrett oesophagus developed adenocarcinoma of the oesophagus at sites cephalad from the anatomic cardia but separated from it by a definite segment of columnar lined oesophagus. Twelve similar cases have been reported in the literature, including one from this institution. Of 85 Mayo Clinic patients in whom a clinical diagnosis of benign Barrett oesophagus was made in the 20-year period 1950-69
