An impaired T-cell-mediated mitogen response as a potential biomarker to aid in clinical diagnosis of pulmonary tuberculosis.

PURPOSE: A considerable proportion of suspected pulmonary tuberculosis (pTB) cases are identified and treated on the basis of clinical diagnoses, as it is often not possible to confirm Mycobacterium tuberculosis (Mtb) infection. In the absence of pathogen detection, supplementary indicators, such as immunological biomarkers, would facilitate TB diagnostic decision-making. METHODS: In this prospective case-control study conducted in Ghana, patients with suspected pTB (n = 83) were divided into three subgroups: confirmed cases with high (pTBhigh) or low (pTBlow) Mtb sputum load, and clinically

Tuberculous Cervical Lymphadenitis (Scrofula) Associated with Massive Pleural Effusion in an Immunocompetent Adult: A Rare Presentation of Extrapulmonary Tuberculosis Confirmed by GeneXpert MTB/RIF.

Background: Tuberculous cervical lymphadenitis (scrofula) is the most common form of extrapulmonary tuberculosis, although its coexistence with massive pleural effusion is uncommon and may delay diagnosis because of its nonspecific clinical presentation. We report the case of a 47-year-old man presenting with a large right-sided pleural effusion associated with a suppurative cervical lymphadenitis, in whom rapid molecular testing established the diagnosis of disseminated extrapulmonary tuberculosis. Case Presentation: A 47-year-old male presented with progressive right-sided chest pain, exerti

Melioidosis in South Asia: Scientific Advances, Regional Challenges and One Health Perspectives-An Integrated Research Synthesis from the 4th South Asian Melioidosis Congress.

BACKGROUND: Melioidosis, caused by Burkholderia pseudomallei, is a severe tropical infection with diverse clinical manifestations and high mortality if untreated. Although endemic in Southeast Asia and northern Australia, its burden is increasingly recognized in South Asia, where the disease remains underdiagnosed and is frequently misclassified as tuberculosis or other infections. This review synthesizes the contemporary scientific evidence and expert consensus emerging from the 4th South Asian Melioidosis Congress held in Guwahati, Assam, India, in November 2025. METHODS: We conducted an in

HLA-A29-Positive Birdshot Chorioretinopathy in Egyptian Patients.

PURPOSE: To report the first HLA-A29‑positive Birdshot Chorioretinopathy (BSCR) patients from Egypt. METHODS: Two patients, from 2 separate cities along the Egyptian Nile Valley, were found to have chorioretinopathy with characteristic "birdshot" pale ill-defined choroidal lesions arranged in a radial fashion emerging from the optic disc, with a relatively quiet anterior segment. On OCT, both patients showed varying degrees of photoreceptor disruption and choroidal structural changes related to long-standing inflammation. RESULTS: HLA-A29 testing was positive in both patients. Both patients

Self-reported prevalence of tuberculosis and selected associated vulnerability factors among rural populations in six districts of India: A community-based cross-sectional study.

BACKGROUND: Tuberculosis is a major public health challenge in India, contributing to one-fourth of global burden of TB disease. Comorbidities in uninfected individuals make them vulnerable to TB disease. This study estimates the self-reported prevalence of tuberculosis and selected vulnerability factors (HIV, current smoking, silicosis, occupational silica-dust exposure, diabetes mellitus and persistent cough >2 weeks) among the rural demography of six districts in India, with the aim of informing targeted active case finding under the National Tuberculosis Elimination Programme (NTEP). Comparative serum cytokine and chemokine profiles across tuberculosis, latent TB, HIV, and COVID-19 infections in Namibian adults.

BACKGROUND: The devastating pandemic of coronavirus disease 2019 (COVID-19) highlighted the importance of early detection and global collaboration in combating infectious diseases. Namibia is one of the countries that bears a high burden of tuberculosis (TB) and human immunodeficiency virus (HIV) infection. The overlap of these diseases underscores the need to better understand host immune responses, which play a critical role in disease pathogenesis and may differ across infections. This study aimed to profile selected cytokines and chemokines among Namibian individuals with TB, latent TB inf

Predictors of tuberculosis preventive therapy initiation among people living with HIV in the Volta region, Ghana.

BACKGROUND: Tuberculosis (TB) remains a leading cause of morbidity and mortality among people living with HIV (PLHIV) despite the availability of effective preventive interventions. Tuberculosis Preventive Therapy (TPT), strongly recommended by the World Health Organization, substantially reduces the risk of active TB among PLHIV, however, uptake of TPT remains suboptimal in many settings. This study, therefore, assessed the predictors of TPT initiation, beginning the course of anti-TB medications among PLHIV. METHODS: This was a health facility-based analytical cross-sectional study among 80

Comprehensive Imaging Review of the Ureter.

The ureter is an important conduit between the kidney and the bladder and is frequently imaged to rule out cancer in the upper genitourinary tract. However, while malignancy can occur, a spectrum of other pathologic ureteral findings can exist, and these can be divided into congenital, intrinsic, and extrinsic abnormalities that range from postoperative and neoplastic conditions to inflammatory disease. This article provides a comprehensive overview of the ureter, including the ureteral anatomy, ureteral imaging strategies, and various ureteral conditions and their associated imaging features.

Tuberculous Meningitis Alters the Proteomic Landscape of Brain-Derived Extracellular Vesicles.

Tuberculous meningitis (TB meningitis), the deadliest form of Mycobacterium tuberculosis infection, causes devastating mortality and severe neurological disability despite standard therapy. Brain injury and microglial activation are major determinants of outcome, yet the molecular mechanisms linking infection, inflammation, and neuronal injury remain poorly understood. Extracellular vesicles (EVs), critical mediators of cell-to-cell communication, have been investigated in pulmonary TB, but their role in TB meningitis remains unexplored. We used a young New Zealand White rabbit model of TB men

The determinants of incorrect inhaler use in patients with obstructive airway disease.

Inhaler therapy is the standard treatment for obstructive airway disease. Even in today's world, it is very challenging to utilize this therapy optimally. Errors in inhaler technique are very common. Identifying the errors is the first step towards correction to improve the disease. This is a cross-sectional observational study conducted at AIMS, Mohali, where 212 patients on inhaler therapy were enrolled who were diagnosed with asthma or chronic obstructive pulmonary disease or post-tuberculosis lung disease. The inhaler technique was assessed by a 5-point inhaler technique checklist by the c

A specialized CD107a+ macrophage subset drives hyperphagocytosis of mycobacteria.

Macrophages are essential for pathogen clearance, yet phagocytic specialization among subsets is poorly defined. Bone marrow-derived macrophages cultured with L929 supernatant or macrophage colony-stimulating factor (M-CSF) separate into FSCloSSCloF4/80loCD11blo (FSCloSSClo) and FSChiSSChiF4/80hiCD11bhi (FSChiSSChi) subsets. Transcriptomic and functional analyses reveal that FSCloSSClo cells possess a hyperphagocytic program driven by enhanced actin cytoskeleton regulators (e.g., Arp2/3) and pro-inflammatory signaling (NF-κB). These cells excel at internalizing Mycobacterium tuberculosis virul

Concurrent Middle Ear and Vertebral Tuberculosis Secondary to Mycobacterium Africanum in an Immunocompetent Young Adult in a Low-Incidence Setting: Diagnostic Value of Middle Ear Biopsy.

Otic tuberculosis is an exceedingly rare manifestation of extrapulmonary tuberculosis (EPTB) and typically mimics more common otologic conditions, leading to delayed diagnosis and treatment. This report discusses a case of middle ear tuberculosis in a New York City-born-and-raised young woman. She presented with ear drainage, hearing loss, generalized weakness, and a painful lump on her posterior neck. She was subsequently diagnosed with multiple abscesses along the cervical and thoracic spine and a large mass in her middle ear that protruded through her tympanic membrane into the external aud
