Psoriasis: Which therapy for which patient, updated, part II. Focus on special populations and chronic infections.

Choosing systemic therapy for psoriasis remains challenging in populations underrepresented in clinical trials, including pregnant and pediatric patients, patients with chronic infections, and those with difficult-to-treat phenotypes. Since the 2019 version of this article, therapeutic options have expanded substantially, including new pediatric approvals, oral targeted agents, and biologics with increasingly phenotype-specific evidence. This second article in the continuing medical education series reviews systemic treatment selection for moderate-to-severe psoriasis in pregnancy, pediatric p

Phenothiazine-Driven Autophagic Response in Macrophages as an Anti-cutaneous Tuberculosis Approach.

Cutaneous tuberculosis (CTB), caused by Mycobacterium tuberculosis (Mtb) infection of the skin, remains a global health burden, underscoring the urgent need for more effective drugs and therapeutic strategies. In this study, we discovered a phenothiazine-based derivative, pyridine-embedded phenothiazinium (PEP), as a promising anti-cutaneous tuberculosis agent with potent efficacy both in vitro and in vivo. Upon light illumination, PEP produces abundant reactive oxygen species (ROS), including type I and type II species, enabling direct mycobacterial killing. Beyond its photodynamic bactericid

Robotic Centrifugal Microfluidic Automation Enables Library Preparation of Nine Samples with a Tenfold Reduction in Reagent Consumption for Decentralized Sequencing.

Next-generation sequencing (NGS) has become a crucial tool for medical diagnostics in decentralized laboratories. While sequencing and bioinformatics themselves employ standardized pipelines, wet laboratory workflows in these settings still rely on manual pipetting and sample handling. In particular, the library preparation procedures represent a critical bottleneck, accounting for up to 50% of total NGS costs and involving complex steps. Automation offers potential solutions to these challenges. However, existing automation approaches have limitations in satisfying throughput, investment cost

An XNA-CRISPR Platform for PAM-Free Detection of the katG Ser315Thr Mutation Associated with Isoniazid Resistance in Tuberculosis.

Drug-resistance Tuberculosis (TB) is a major threat to global public health. Simple and precise detection of the mutations associated with drug resistance is still a significant challenge. CRISPR-based nucleic acid detection has emerged as a transformative technology in molecular diagnostics due to its high specificity and programmability. However, the CRISPR-Cas12a system requires a protospacer adjacent motif (PAM) in the target double-stranded DNA (dsDNA) for efficient recognition and activation, which limits its application for detecting single-nucleotide variants (SNVs). To overcome this l

Protocol for DNA extraction from QuantiFERON-TB gold tubes for PCR and sequencing applications.

Latent tuberculosis infection (LTBI) remains a significant barrier to global TB control and elimination efforts. The QuantiFERON-TB Gold (QFT) assay is commonly used for the diagnosis of LTBI. However, blood collected in QFT tubes is seldom utilized for molecular and genetic analysis due to the presence of heparin and a dense gel barrier that hinders efficient DNA extraction. To address this limitation, we aimed to develop a method for directly isolating high-quality DNA from blood in QFT tubes, eliminating the need for additional blood sampling and enabling their use in both diagnostic and mo

Ag85A-Dependent Surface Lipid Remodeling Impairs Adhesion and Infection of Therapeutic Mycobacteriophage.

Patients infected with mycobacteria, particularly Mycobacterium abscessus, frequently experience antibiotic treatment failure. Phage therapy, used under compassionate care, offers a promising alternative and has already demonstrated clinical efficacy. However, phage-bacteria interactions and their coevolution remain critical factors that can compromise therapeutic success. Trehalose polyphleates (TPPs), complex lipids of the outer membrane, play a key role in the adhesion of the therapeutic mycobacteriophage BPsΔ33HTH_HRM10, and loss of TPP synthesis confers bacterial resistance to the phage.

Mycobacterium smegmatis MSMEG_6906 modulates IL-1β via NF-κB pathway to benefit intracellular survival.

Tuberculosis (TB), instigated by Mycobacterium tuberculosis (Mtb), exerts a detrimental impact on public health worldwide. Mtb features unique and abundant lipids components and genes involved in lipogenesis, which are believed to provide the pathogen an advantage in the host, but the underlying mechanisms remain inconclusive. In the present study we show that Mycobacterium smegmatis (M. smegmatis) MSMEG_6906, annotated as a hydrolase that probably involved in lipid biosynthesis, functions in affecting bacterial morphology, and plays a role in modulating IL-1β expression in macrophages. The ab

Randomized pilot trial of intraoperative hemoadsorption in clopidogrel treated patients undergoing urgent coronary artery bypass grafting.

BACKGROUND: Patients receiving clopidogrel who undergo urgent CABG before the recommended 5-day washout period are at increased risk of perioperative bleeding. This study evaluated whether intraoperative hemoadsorption can reduce bleeding in such patients. METHODS: Patients undergoing CABG within 24 h of clopidogrel discontinuation were randomized 1:1 to intraoperative hemoadsorption (Group 1) or control (Group 2). In the intervention group, the device was incorporated into the cardiopulmonary bypass (CPB) circuit. Efficacy was assessed by 24-hour chest tube drainage (CTD), blood product tran

Mycobacterium tuberculosis Rv3248c Inhibits Type I Interferon Response and Promotes Intracellular Survival by Mediating Ubiquitin-Proteasome-Dependent Degradation of TBK1.

Mycobacterium tuberculosis (M. tb) is a leading cause of infectious disease mortality worldwide, and M. tb subverts host immunity to persist intracellularly. Type I interferon (IFN-I) modulates host defense during M. tb infection, but the mechanisms controlling their induction remain incompletely defined. In this research, a previously uncharacterized mycobacterial factor, M. tb Rv3248c, which regulates IFN-I response, was identified using pull-down and mass spectrometry. Overexpression of Rv3248c suppresses IFN-β expression in macrophages at both the transcriptional and protein levels. In add

Bacteria inside kidney tumors associate with myeloid inflammation and disease progression: A TRACERx Renal study.

BACKGROUND: Intratumoral bacteria are increasingly recognized as active contributors to neutrophil-driven inflammation and disease progression in solid tumors. Yet, in clear cell renal cell carcinoma (ccRCC), where myeloid inflammation is a key feature of aggressive disease, live bacterial presence, their intratumoral distribution, and interaction with the tumor microenvironment remain unexplored. METHODS: We genetically characterized the microbiome of primary ccRCC tumors across two independent cohorts (TRACERx: 616 samples, 143 patients; The Cancer Genome Atlas [TCGA]: 511 samples and patie

Control of M. tuberculosis infection requires CD28 to generate lung-homing T cells and CTLA-4 to limit immunopathology.

Mycobacterium tuberculosis (Mtb)-specific CD4 T cells differentiate into protective lung parenchyma-homing and non-protective intravascular subsets, but the signals that instruct these fates are poorly understood. Here we show that CD28 co-stimulation regulates host protection during Mtb infection. Cd28-/- mice are susceptible to Mtb infection, displaying higher bacterial loads and reduced survival compared to wild type mice. Cd28-/- Mtb-specific T cells expand normally but are skewed towards the lung intravascular phenotype. CD4 T cells primed in the absence of CD28 co-stimulation in B7 defic

18F-Fluorodeoxyglucose PET/CT in Tuberculosis and Other Granulomatous Diseases (except Sarcoidosis and Fungal Disease).

Granulomatous inflammation consists mainly of activated macrophages, epithelioid cells, and lymphocytes, which have high glucose metabolism and therefore show increased 18F-fluorodeoxyglucose (18F-FDG) uptake on PET/computed tomography (CT). This makes FDG PET/CT useful in detecting and mapping granulomatous disease, including clinically silent or extrapulmonary involvement. It is particularly crucial in tuberculosis, where it can help identify active disease, guide biopsy, assess treatment response, and detect dissemination. FDG PET/CT also has applications in granulomatous vasculitis such as

18F-fluorodeoxyglucose Positron Emission Tomography with Computed Tomography in Pediatric Non-oncological Conditions: A Comprehensive Review.

This article examines the expanding role of 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography with computed tomography (PET/CT) in pediatric non-oncological conditions, moving beyond its established oncologic use. It outlines radiation dosimetry principles unique to children-emphasizing the As Low as Reasonably Achievable principle, reduced tracer activity, and low-dose CT protocols-given their heightened radiosensitivity. In pediatric neurology, FDG PET/CT aids evaluation of drug-refractory epilepsy (temporal and extratemporal foci) and neuronal migration/proliferation anomalies l

AX-35 Targets the Cytochrome bc1 Complex in Mycobacterium abscessus and Reveals a Novel Resistance Mechanism.

In patients with cystic fibrosis, nontuberculous mycobacteria are opportunistic, intracellular and extracellular pulmonary pathogens that contribute to deterioration of lung function and, ultimately, morbidity from respiratory failure. Among nontuberculous mycobacterial respiratory pathogens, members of the Mycobacterium abscessus complex pose additional treatment complications as they are notoriously drug-resistant. As part of our routine screening against mycobacterial species, one of the lead antituberculosis compounds, the cytochrome bc1 inhibitor AX-35, was found to be active against isol

Two heads are better than one: Single stranded DNA translocation of UvrD-family dimers vs. monomers.

UvrD-family Superfamily 1A helicases are processive ATP-dependent motor proteins that function during DNA replication, recombination, repair, and transcription. UvrD-family monomers translocate along single stranded (ss) DNA with 3'-to-5' directionality but must be activated by dimerization to become helicases in the absence of force or accessory factors. Mycobacterium tuberculosis (Mtb) UvrD1 helicase forms dimers via a disulfide bond between native cysteines in the 2B sub-domains of each monomer. E. coli UvrD forms non-covalent dimers using the same 2B domain interface as in Mtb UvrD1. Using

Mycobacterium tuberculosis the development of a metal-enhanced fluorescence (MEF)-based biosensor in this study enabled the detection of Mycobacterium tuberculosis DNA extracted from sputum samples with high sensitivity and without the need for DNA amplification. The detection process involved the utilization of silver dendritic nanostructures, which were grown on the surface of silicon wafers and subsequently modified with a probe that was designed to target the specific DNA Insertion Element 6110 (IS6110) gene of M. tuberculosis. A comparison was made between the performance of the developed
