Achalasia is a primary esophageal motility disorder defined by impaired lower esophageal sphincter relaxation and absent peristalsis. Late-stage disease can lead to megaesophagus, a severe dilation and tortuosity of the esophageal body. While most cases are idiopathic, secondary achalasia can be caused by Chagas disease (Trypanosoma cruzi infection), an important consideration in patients from endemic regions of Central and South America. Our case highlights a 51-year-old female from Nicaragua who presented to the emergency department with an acute onset of shortness of breath. A computed tomo

Due to migration phenomena, Chagas disease (CD) is emerging in Europe, where CD-related gastrointestinal complications are rarely reported. We describe a case of CD-related megaoesophagus in a patient from El Salvador, in Italy since 14 years, highlighting the importance of considering complicated CD in patients at risk with suggestive symptoms.

Chagasic esophagopathy, resulting from Trypanosoma cruzi-induced myenteric plexus denervation, leads to lower esophageal sphincter hypertonia, esophageal body hypertrophy, and dilation (megaesophagus), manifesting as progressive dysphagia and regurgitation that impair nutritional status and quality of life. This case report aims to describe chronic esophageal dysphagia in a patient from an endemic area in northeastern Brazil, highlighting diagnostic and therapeutic challenges within the public health system, including delays due to COVID-19 restrictions and resource limitations. A 67-year-old

BACKGROUND: Esophageal motor patterns associated with Chagas disease (CD) have been examined in several high-resolution esophageal manometry (HREM) studies involving cohorts with varying characteristics and outcomes. None of these studies applied the Chicago Classification v4.0 (CCv4.0).
GOALS: This study aimed to identify the spectrum of esophageal motor disorders related to CD based on the criteria outlined in CCv4.0.
STUDY: The study included 129 patients with positive serologic tests for CD and no anatomic obstructive lesions, referred to HREM from identified sources. The HREM plots were

Chagas disease (CD) remains a persistent public health challenge in endemic areas of Brazil, where access to diagnosis, clinical staging, and etiological treatment is often limited. Tremedal and Novo Horizonte, municipalities in the state of Bahia, continue to report chronic cases despite advances in vector control. This study provides a retrospective assessment of CD detection and clinical management in these municipalities prior to the full implementation of the Oxente Chagas Bahia Project. We analyzed individuals tested for CD serology between 2019 and 2023 using data from the Laboratory En

Myenteric plexus injury is responsible for the morpho-functional alterations observed in chagasic megaesophagus (CME). The inflammatory response, characterized by elevated synthesis of IFN-γ, TNF-α, and IL-4, contributes to the persistence of parasitism and inflammation. This study assessed the mRNA expression of cytokines, transcription factors, and metalloproteases in subjects with CME. From 2011 to 2017, esophageal samples were collected from 54 subjects with CME (38 advanced and 16 nonadvanced) and eight subjects with idiopathic megaesophagus (IME). The quantitative mRNA expression of TNF-

Patients carrying Chagasic megaesophagus (CME) are at high risk for esophageal carcinoma. We aimed to investigate mutations in the TP53 in patients carrying CME. Blood samples from 114 patients with Chagas disease (CD) were used. The samples were subjected to PCR-SSCP analysis and DNA sequencing in exons 5 and 7 of the TP53 gene. We observed mutations in the exon 5 codon 184 (GAT > AAT) in 14.8% of G1 (11/74), 10% of G2 (4/40), and 5% of G3 (2/40). We also observed the codon 185 mutation (AGC > AGG) in 14.8% of G1 (11/74), 10% of G2 (4/40), and 7.5% of G3 (3/40). Regarding Exon 7, a mutation (

Chagasic megaesophagus is a relatively uncommon clinical manifestation in individuals with chronic Chagas disease (CD), and it has not been extensively documented in literature. However, individuals may exhibit negative or inconclusive serology for CD. This study aimed to assess the performance of molecular diagnostics for CD in participants with these conditions. This was a prospective cohort study that included 26 participants with negative or inconclusive conventional CD serology (Group I), 33 participants with positive CD serology and megaesophagus (Group II), and 10 participants with nega

Chagas disease, caused by the protozoan parasite Trypanosoma cruzi (T. cruzi), is a neglected disease affecting around 6 million people, with no effective antiparasitic drugs or vaccines. About 40% of Chagas disease patients develop symptomatic forms in the chronic phase of infection, chronic Chagas cardiomyopathy (CCC) or digestive forms like megaoesophagus and megacolon, while most infected patients (60%) remain asymptomatic (ASY) in the so-called indeterminate form (IF). CCC is an inflammatory cardiomyopathy that occurs decades after the initial infection. Death results from heart failure o

BACKGROUND: Achalasia is a rare primary esophageal motility disorder of the esophageal smooth muscle, characterized by abnormal relaxation of the lower esophageal sphincter and associated with abnormal, spastic, or absent esophageal body peristalsis.
SUMMARY: The primary pathophysiological defect is abnormal esophageal inhibitory nerve function from neuronal death in the esophageal neuronal plexuses and ganglia that control esophageal smooth muscle peristalsis. This is a consequence of an autoimmune cytotoxic insult from molecular mimicry following an intercurrent viral infection, typically h

INTRODUCTION: achalasia is a chronic disease. Since there is no curative treatment, diagnosed patients have pharmacological and/or surgical techniques available, aimed at minimizing the condition. POEM appears as a promising new type of palliative treatment with good rates of symptom improvement.
OBJECTIVE: evaluate the profile of POEM at the Clinical Hospital of the Federal University of Pernambuco (HC - UFPE) and correlate it with the world scenario.
METHODS: data collection was performed retrospectively from September 2017 to October 2019 with all patients undergoing POEM at the HC - UFPE

Alterations caused by Trypanosoma cruzi in the composition of gut microbiome may play a vital role in the host-parasite interactions that shapes physiology and immune responses against infection. Thus, a better understanding of this parasite-host-microbiome interaction may yield relevant information in the comprehension of the pathophysiology of the disease and the development of new prophylactic and therapeutic alternatives. Therefore, we implemented a murine model with two mice strains (BALB/c and C57BL/6) to evaluate the impact of Trypanosoma cruzi (Tulahuen strain) infection on the gut mic

BACKGROUND/AIMS: Multiple sclerosis (MS) is an inflammatory disease characterized by the demyelination of primarily the central nervous system. Diffuse esophageal spasm (DES) and achalasia are both disorders of esophageal peristalsis which cause clinical symptoms of dysphagia. Mechanisms involving dysfunction of the pre- and post-ganglionic nerve fibers of the myenteric plexus have been proposed. We sought to determine whether MS confers an increased risk of developing achalasia or DES.
METHODS: Cohort analysis was done using the Explorys database. Univariate logistic regression was performed

BACKGROUND: Investigation of aged Chinese herbal materials will help us to understand their use and sources in ancient time and broaden the historical perspective of Chinese material medica. To reach this aim, the basic understanding of aged herbal materials, including physical and chemical characters, is of great importance. Delayed luminescence (DL) technique was developed as a rapid, direct, systemic, objective and sample loss-free tool to characterize the properties of Chinese herbal materials. In this study, we measured DL values in aged Chinese herbal materials that were transported from
