Chronic gastritis and Helicobacter pylori in digestive form of Chagas' disease.

Patients with the digestive form of Chagas' disease frequently present chronic gastritis. As the microorganism Helicobacter pylori is now accepted as the most common cause of human chronic gastritis, the present work was undertaken to verify a possible relationship between the presence of this bacterium and inflammatory changes of antral mucosa in chagasic patients. Seventeen chagasics, with megaesophagus and or megacolon were studied. Fragments from two different regions of antral mucosa were obtained by endoscopy, fixed in 4% neutral formaldehyde and embedded in paraffin. The sections were s

[Prevalence of electrocardiographic abnormalities in 2,000 aged and non-aged chagasic patients].

PURPOSE: To assess the prevalence of main electrocardiographic alterations in chagasic patients at University of Triângulo Mineiro Medical School, and compare such alterations between aged and non-aged patients. METHODS: Electrocardiograms (ECG) of 2,000 chagasic patients were interpreted. Age, color, sex and concomitant systemic hypertension, congestive heart failure (CHF) and megaesophagus and/or megacolon were registered. Chi-square test was employed to compare aged (60 years or more; n = 378) and non-aged (age less than 60 years; n = 1622) chagasics. RESULTS: ECG were normal in 9.6% of t

The surgical treatment of megaesophagus and megacolon.

The effect of Chagas' disease on the digestive tract is evaluated based on the experience acquired in the surgical management of 929 cases of megaesophagus: 807 in the nonadvanced state of the disease and 122 with dolichomegaesophagus. The 807 subjects with nonadvanced megaesophagus were submitted to wide esophagocardiomyectomy on the anterior esophagogastric junction combined with an antireflux valvuloplasty procedure. There was no mortality. On the other hand, dolichomegaesophagus was always resected, with an esophagogastroplasty through the esophageal bed. The index of mortality of the seri

Achalasia occurring years after acute poliomyelitis.

We present a case of a 19 year old female patient with dysphagia for 4 months. Radiologic, endoscopy and manometric examinations were compatible with the diagnosis of idiopathic achalasia. Clinical, epidemiologic and serologic investigation was negative for Chagas' disease. When she was three years old she had acute poliomyelitis that left muscular atrophy in her left leg. It is possible that lower esophageal sphincter achalasia was the consequence of lesion in the dorsal motor nucleus of the vagus nerve caused by poliomyelitis. The association between poliomyelitis and achalasia supports the

[Simultaneous digestive and cardiological studies in asymptomatic chagasic patients].

Prognosis in patients with Chagas disease is related to systemic involvement. 43 asymptomatic chagasic subjects, 28 males and 15 females aged 28 to 85 years, were subjected to electrocardiograms and upper and lower gastrointestinal radiological studies. 23 patients (53%) had evidence of involvement of at least one organ. Multiple involvement was demonstrated in 5 individuals (12%): one patient had megaesophagus, megacolon and abnormal EKG; 2 had megaesophagus and abnormal EKG; 1 had megaesophagus and dolicocolon and 1 had dolicocolon and abnormal EKG. Dilated esophagus was found in 6 patients

[Incidence of symptomatic megacolon in patients with chagasic megaesophagus].

The authors analyze the incidence of megacolon associated to megaesophagus during a 7 year period. Of the 192 patients with megaesophagus only 15 who presented associated intestinal symptoms were further investigated through barium enema. No statistically significant differences were found between the group with megaesophagus and the group presenting association of megaesophagus and megacolon as to sex and age of the individuals and severity of megaesophagus. The esophageal symptoms preceded the intestinal symptoms in 93.3% of the cases. Many treatments were used, but recently good results wer

Parasympathetic denervation of the iris in Chagas' disease.

The parasympathetic innervation of the iris along with cardiovascular reflexes involving parasympathetic and sympathetic function were studied in 45 patients with Chagas's disease and in 36 controls. The autonomic features in Chagas' disease included 15 with cardiomyopathy, three with megaoesophagus and five with megacolon. None of the patients had orthostatic hypotension. Parasympathetic cardiac reflexes (deep breathing 30:15 ratio, Valsalva) were abnormal. There were exaggerated pupillary responses to dilute pilocarpine. These studies suggest iris parasympathetic denervation and favour more

Short report: comparison of the effects of sublingual nifedipine and isosorbide dinitrate on oesophageal emptying in patients with Chagasic achalasia.

The effects of sublingual nifedipine and isosorbide dinitrate on oesophageal emptying were compared in 11 patients with Chagasic achalasia. The oesophageal emptying of a radiolabelled test meal was assessed three times in each patient by a scintigraphic technique. No treatment preceded one of the studies (basal study). Nifedipine (20 mg) by the sublingual route 30 min before the meal, preceded one study. Isosorbide dinitrate, 5 mg by the sublingual route 5 min before the meal, preceded the third study. The order of the studies was allocated randomly for each patient. Oesophageal retention at t

[A longitudinal radiologic study of the esophagus in an endemic area of Chagas' disease over a 6-year period].

A longitudinal study over six years was undertaken of 494 residents of the municipality of Mambaí, Goiás. Two hundred and twelve (43%) were seropositive in 1975/76 and 199 of 464 of the same patients group, positive in 1980/82 (42.8%). At both examination single radiographs of the oesophagus were obtained immediately after ingestion of 75 ml of barium sulphate solution and a second X-ray taken one minute later. Among the 201 seropositive patients without megaesophagus in the first study 4 (2%) evolved megaesophagus during the six years of observation. During this time, using Rezende's classifi

Autonomic neuropathy and immunological abnormalities in Chagas' disease.

Chagas' disease (American Trypanosomiasis) is caused by infection with the haemoflagellate parasite Trypanosoma cruzi, which is transmitted from animals to man by the Reduviidae bug. The human disease is characterized by two phases. In the first (acute phase) parasitaemia is high and general symptoms variable. The next, which is lifelong (chronic phase), is characterized by inflammatory lesions in cardiac and skeletal muscle, gastrointestinal, the autonomic nervous system. Parasites are difficult to detect in blood and affected tissues. Lesions within the autonomic nervous system, lead to deve

[Esophageal achalasia of chagasic origin: report of a case diagnosed in the pre-symptomatic stage].

A 42 year old male that presented with functional gastrointestinal symptoms is presented. X ray examination showed an esophageal diameter of 3 cm, measured 6 cm above the gastroesophageal junction. This finding, according to previous experiences of the authors, is highly suggestive of Chagasic esophageal involvement. The patient resided in a highly endemic area for Chagas disease; hemagglutination, latex agglutination and ELISA serological tests for Chagas disease were positive. The manometric study of the esophagus confirmed the diagnosis showing an elevated lower esophageal sphincter pressur

Use of isosorbide dinitrate for the symptomatic treatment of patients with Chagas' disease achalasia. A double-blind, crossover trial.

1. A randomized, double-blind, placebo-controlled trial was carried out to determine the efficacy of isosorbide dinitrate (ISD) on dysphagia in patients with Chagasic achalasia. 2. Twenty-three patients with Chagas' disease and dysphagia entered the study and 20 (87%) completed the two 7-day treatment periods. Subjects were given either 5 mg ISD (12 patients) or placebo (11 patients) by the sublingual route for the first 7 days. On the 8th day, patients crossed over and began another 7-day period during which they received the opposite, identical-appearing tablets. 3. Scores attributed by unin

[Tissue levels of 6-ketoprostaglandin Fl alpha and integrity of gastric mucosa in patients with Chagas disease].

An increased prevalence of chronic gastritis has been reported in patients with chronic Chagas' disease (American trypanosomiasis), which is regarded to be model of intrinsic denervation of the gastrointestinal tract. In order to investigate whether this condition is associated with a reduction of prostaglandin levels in gastric mucosa, we studied 14 Chagas' disease patients with megaesophagus and/or megacolon and compared them with 10 control subjects. All patients and controls underwent an upper gastrointestinal tract endoscopy where biopsies were collected from the antrum for histology and
