Cholinergic innervation of the lower esophageal sphincter in Chagas' disease.

1. The effect of 12 micrograms/kg iv atropine on the lower esophageal sphincter (LES) pressure was studied by continuous perfusion manometry in 14 Chagasic patients, 9 controls, and 3 patients with achalasia, and the effect of 3 ml iv saline was studied in 7 Chagasic patients. 2. Resting LES pressure did not differ between Chagasic patients (11.5 +/- 4.1 mmHg) and controls (15.9 +/- 4.9 mmHg, P greater than 0.05). 3. Atropine caused a significant decrease in LES pressure in both Chagasics and controls, but the reduction in controls was significantly greater (56%) than in Chagasics (25%). 4. Sa

Use of isometric exercise to demonstrate cardiac parasympathetic impairment in the digestive form of Chagas' disease.

Standardized handgrip at 100% of maximal voluntary capacity was used to study heart rate responses to isometric exercise in patients with the digestive form of Chagas' disease. The chronotropic responses of the group were significantly lower (P less than 0.001) than those exhibited by control subjects, and comparable to those observed in a group of cardiac chagasic patients. These results show that severe impairment of autonomic control of the heart may occur in chagasic patients apparently having only digestive tract involvement.

[Esophageal achalasia: review of the results after 15 years' experience].

The purpose of this paper is to evaluate the experience acquired along a 15 years period (1971-1985) in the treatment of achalasia of the esophagus. One hundred and fifty six patients were evaluated. The average age was 50.8 years, and the M/F ratio 0.9/1. Dysphagia was present in 100%, regurgitation in 78.2%, weight loss in 61.5%, and chest pain in 50% of the cases, being the main symptoms. Serology for Chagas disease was positive in 21.2% of the patients. When classified by radiologic criteria the groups were: grate I 18.5%, grate II 53.8%, grate III 14.7% and grate IV 12.8%. [Effect of isosorbide dinitrate on the lower esophageal sphincter pressure in patients with Chagas' disease].

The effect of 5 mg of sublingual isosorbide dinitrate on the lower esophageal sphincter pressure of 28 chagasic patients with esophageal involvement, was studied by continuous perfusion manometry. The pressure was measured at 5 minute intervals for 60 minutes after drug administration. Isosorbide dinitrate reduced sphincter pressure from 5 to 60 minutes (p less than 0.01). Only two patients hadn't sphincter pressure reduced lower than 50% of initial value at 20 minutes after drug administration. These results indicate that isosorbide dinitrate may be useful to reduce lower esophageal sphincter

[The esophagus in Chagas' disease: physiologic, pharmacologic and clinical studies].

Intramural denervation confirmed anatomopathologically and by means of a pharmacological test is the main factor responsible for achalasia of the cardia and for the absence of peristalsis in the esophageal body in Chagas' disease. The resulting difficulty in transit and stasis cause the main symptoms and complications of megaesophagus. Among the recent phenomena observed in the physiopathology and pharmacology of megaesophagus by the manometric method, we may mention: delayed pharyngo-esophageal time, concomitance of peristalsis and aperistalsis and abnormal responses of the lower sphincter to

Effect of nifedipine on the lower esophageal sphincter pressure in chagasic patients.

The effect of 10 mg of sublingual nifedipine on the lower esophageal sphincter pressure (LESP) was studied by continuous perfusion manometry in 15 Chagasic patients and 9 controls. Resting LESP was lower in Chagasic patients (13.51 +/- 2.37 mmHg) than in controls (19.60 +/- 2.51 mmHg, P less than 0.02). Nifedipine caused a gradual decrease in the LESP in both Chagasics and controls. Maximal reductions occurred 50 minutes after the drug administration when LESP was reduced to 60% of the resting LESP in the control group and to 43% in the Chagasic group. These results indicate that the striking

[Prevalence of cholelithiasis in necropsies of patients with chronic Chagas' disease in the mining triangle--correlation with megaesophagus, megacolon and cardiac insufficiency].

Retrospective analysis of the diagnosis of 2.517 autopsies performed in the western region of the State of Minas Gerais, Brazil, showed that cholelithiasis has significant association with megaesophagus and megacolon but not with Chagas' disease (when considered in all of its anatomo-clinic presentations) or with heart failure due to chronic chagasic cardiopathy. Possible explanations to these findings are discussed and the scanty pertinent literature is commented.

In a clinical, radiological and electrocardiographical, follow-up study of the "case control" type performed in Virgem da Lapa, Minas Gerais State, Brazil, 124 chagasic patients were followed during six years. The results of the patients, the majority in the indeterminate form, did not register any change, in 32.2% there was a progress in the disease and in 5.6% the electrocardiogram returned to normal. These results when compared to that achieved by the control group, composed of pairs of non chagasic persons with the same age and sex, was shown to be 27.4% higher than among patients with pos

Gastroesophageal sphincter pressure and histological changes in distal esophagus in patients with achalasia of the esophagus.

A prospective study was performed in 17 patients with achalasia of the esophagus determining the manometric characteristics of the gastroesophageal sphincter, correlating it with histological analysis by biopsies taken during surgery at the distal narrowed segment of the esophagus, at the location of the sphincter. The histological findings were compared to 10 control cases. Presence or absence of ganglion cells at the Auerbach's plexuses and appearance of smooth muscle fibers were evaluated. Only one case (6%) had Chagas' disease. The mean sphincter pressure was 41 mm Hg, with incomplete rela

[Pharyngo-esophageal time in Chagas esophageal disease].

The pharyngoesophageal time, the time interval between the peak pharyngeal pressure and the peak esophageal pressure ten centimeters apart, after wet deglutitions, was measured in 50 controls and 37 chagasic patients. The time was 2.90 +/- 0.64 seconds in controls and 3.69 +/- 1.32 seconds in chagasics (P less than 0.01). In Chagas' disease the esophagus shows a delay in contraction produced by wet deglutitions.

Megaesophagus and seroreactivity to Trypanosoma cruzi in a rural community in northeast Brazil.

The relationship of symptoms and radiographic abnormalities suggestive of esophageal motility disorders with electrocardiographic (ECG) alterations and seroreactivity to Trypanosoma cruzi was studied in a defined population in a rural area endemic for Chagas' disease in Bahia, Brazil. Between January and June 1981, 680 individuals 5 years of age or older were examined with serologic tests, ECGs and questionnaires for esophageal motility disorder. Of these, 39.9% were seropositive for Chagas' disease. Symptoms of dysphagia occurred 2.5 times more frequently among seropositive individuals than a

Esophageal involvement in congenital Chagas' disease. Report of a case with megaesophagus.

A full-term infant developed dysphagia, vomiting, and regurgitation immediately after birth. On examination at 5 months of age, he was malnourished and had radiographically proven megaesophagus. On direct examination his blood was positive for Trypanosoma cruzi. Additionally an enzyme immunoassay for T. cruzi IgM antibodies was positive. He underwent corrective surgery for the megaesophagus but died post-operatively of aspiration pneumonia. At autopsy, the esophagus was markedly dilated. There was an extreme reduction of neurons in Auerbach's plexus, and inflammation of this plexus and the mus

Pulmonary findings in patients with chagasic megaesophagus. Study of autopsied cases.

We studied retrospectively 69 cases of chagasic megaesophagus (group 1) and 207 cases of chronic chagasic myocardiopathy (CCM) without megaesophagus (group 2) by autopsy in the Pathology Department of the Federal University of Bahia, Brazil, from 1953 to 1975. It was shown that pulmonary tuberculosis was significantly more frequent in group 1 (21.7 percent) than in group 2 (3.4 percent). The patients of group 1, who had megaesophagus without associated cardiomyopathy (n = 36), had a much higher rate of pulmonary tuberculosis (36.1 percent) than those with associated CCM (6.0 percent), and they

Sphincter of Oddi pressure in chagasic patients with megaesophagus.

Autonomic denervation is found throughout the entire length of the digestive tract in Chagas' disease. Anatomic evidence of myenteric ganglia reduction in chagasic gallbladders has been noted; however, the sphincter of Oddi has not been studied. The purpose of this study is twofold: first, to determine sphincter of Oddi pressure in 11 patients with chronic Chagas' disease and megaesophagus, and to compare the results with those obtained in 27 control subjects; and second, to evaluate the effect of cholecystokinin-octapeptide on sphincter of Oddi pressure in both groups of patients.
