Endoscopic electrocoagulation was performed on 40 occasions for 38 patients with bleeding gastrointestinal lesions. Cessation of bleeding was achieved in 95%. Fifteen gastric ulcers, 14 duodenal ulcers, six Mallory-Weiss tears, one gastric varix, one hemorrhagic antral gastritis, and one esophageal ulcer were successfully electrocoagulated. Three duodenal and three gastric ulcers rebled. One duodenal ulcer and one gastric ulcer were successfully reelectrocoagulated. The effects of aspirin, salicylate formulations and substitutes, smoking (nicotine), indomethacin, corticosteroids, phenylbutazone, ethanol, caffeine and reserpine on the gastric mucosa are discussed. Spontaneous pneumopericardium is a relatively rare event, although cases have been recorded over the past 130 years. A personally observed series of 19 patients with hour-glass stomach is presented. In two cases carcinoma was proven. Among the remaining 17 patients with benign ulcer, females outnumbered males by 15 to 2. In neither of the carcinoma patients, but all of the benign ulcer patients, there was clear evidence of abuse of aspirin-containing analgesic preparation. The hypothesis is proposed that hour-glass stomach is produced by gross scarring around a severe benign chronic gastric ulcer under the influence of prolonged exposure to aspirin.

Ligation of the left gastric and right gastroepiploic arteries and veins resulted in chronic gastric ulcer formation in the rat. Linear mucosal corpus hemorrhages appeared within 8 hr of ligation. Various criteria in gastric analysis currently used for the diagnosis of the Zollinger-Ellison syndrome were assessed in cases of endoscopically normal gastric mucosa, gastric ulcer and duodenal ulcer. New criteria consisting of BSV greater than 350 ml/hr, BAO greater than 25 mEq/hr, MAO greater than 60 mEq/hr, and BAO/MAO greater than 0.7 were proposed to differentiate the Zollinger-Ellison syndrome and peptic ulcer.

The factors that determine the recurrence rate of chronic gastric ulcer were studied in 105 patients. It was found that complete healing of the ulcer significantly reduced the recurrence rate and subsequent need for hospital admission because of ulcer symptoms when this group was compared with those who left hospital with their ulcers unhealed. The distributions of acid alpha1-glycoprotein, alpha1-fetoprotein, beta-galactosidase and gastrin in gastric carcinoma and gastric ulcer as well as in the neighbourhood of these lesions were studied by means of immunohistochemical methods on imprint preparation. The incidence of gastric carcinoma was studied in a series of 210 patients presenting with apparently benign gastric ulcer. In eight cases (3.9 per cent), carcinoma was diagnosed within 18 months and was almost certainly present from the outset; in 2.4 per cent the diagnosis was delayed for over three months, and the five-year death-rate due to gastric carcinoma was 3.2 per cent. In a mean follow-up period of 5.7 years after the first diagnosis of an ulcer (8.2 years after first symptoms), three fresh cases of gastric carcinoma were found, and in two of these the cancer was at a different site
