A Case Report of a Successful Bilateral Lichtenstein Repair Using Elastic Tacks.

This manuscript is dedicated to the examination of a case involving a 55-year-old male patient who has experienced bilateral inguinal hernias for a duration of three years. The patient was subsequently referred to our department from the outpatient clinic for the purpose of determining an appropriate surgical intervention. The patient presented with comorbidities including primary arterial hypertension, duodenal ulcer, chronic prostatitis, gastroesophageal reflux disease (GERD), chronic esophagitis, and occurrences of supraventricular tachycardia. Our research team elected to implement the Lic

A Rare Case of Follicular Dendritic Cell Sarcoma of Tonsil-Clinical Report.

BACKGROUND: Duodenocolic fistulas are uncommon but serious complications of peptic ulcer disease (PUD), resulting from the erosion of a duodenal ulcer into the colon. This connection typically forms when a chronic ulcer perforates or penetrates through the duodenum, leading to inflammation and necrosis that disrupts the normal anatomical barriers. CASE REPORT: A 66-year-old male with a history of inadequate medical follow-up, chronic nonsteroidal anti-inflammatory drug use, alcohol use disorder, and PUD, developed a duodenocolic fistula secondary to a perforated duodenal ulcer. Endoscopy reve

Over-The-Scope Clip Misdeployment Resulting in Acute Cholangitis and Biliary Pancreatitis.

Over-the-scope clips (OTSCs) have improved the endoscopic management of acute nonvariceal upper gastrointestinal bleeding. They allow for effective hemostasis of high-risk lesions in challenging locations and have been shown to be superior to through-the-scope clips. However, misdeployment of OTSCs may be associated with obstructive complications. In this study, we present the case of a 67-year-old man who developed recurrent gastrointestinal bleeding from a duodenal ulcer. His course was ultimately complicated by acute cholangitis and pancreatitis from a misdeployed OTSC around the ampulla. E

Duodenocaval Fistula Caused by Post-Bulbar Duodenal Ulcer: A Case Report.

INTRODUCTION: Duodenocaval fistula (DCF) is a rare, life-threatening disease with a mortality rate of approximately 40%. There are several types of etiologies for DCF. Among these, duodenal ulcers are the most dangerous, with a mortality rate of 63.6%. This report describes a case of DCF caused by a duodenal ulcer that was successfully diagnosed and treated, and discusses why duodenal ulcer-induced DCF results in severe outcomes, with a review of the literature. CASE PRESENTATION: A 64-year-old male patient was transferred to our hospital for surgical treatment of DCF. An emergency laparotomy

Fatal Duodenal Ulcer Hemorrhage in a Post-Operative Patient of Proximal Femur Fracture Undergoing Carbon Ion Radiotherapy for Locally Advanced Pancreatic Carcinoma: A Case Report.

The incidence and mortality rates of pancreatic cancer have been increasing. Due to the high likelihood of metastasis and vascular invasion, most pancreatic cancer cases are inoperable at the time of diagnosis. Carbon-ion radiotherapy (CIRT) has recently been introduced as a treatment option for locally advanced pancreatic cancer (LAPC). However, potential complications, including peptic ulcer disease (PUD), have been noted. This report presents a case of death due to PUD following a proximal femur fracture, with a history of CIRT for unresectable LAPC. A 59-year-old female undergoing CIRT for

An Unexpected Culprit: Fatal Gastrointestinal Hemorrhage due to Duodenal Histoplasmosis in a Patient With End-Stage Renal Disease.

Histoplasmosis has a wide spectrum of presentations ranging from mild pulmonary disease to a more widespread infection leading to disseminated histoplasmosis. Gastrointestinal histoplasmosis is a rare form of disseminated histoplasmosis that most commonly causes nonspecific symptoms and rarely presents with serious symptoms. Duodenal involvement is further rare but can cause ulcerated lesions leading to upper gastrointestinal bleed. The potential for a nonspecific initial presentation quickly progressing to a fatal upper gastrointestinal bleed makes it important to keep Gastrointestinal histop

Analysis of risk factors and nursing care for nonspecific abdominal pain after endoscopic retrograde cholangiopancreatography (ERCP): A case report.

RATIONALE: Endoscopic retrograde cholangiopancreatography (ERCP) with endoscopic nasobiliary drainage is the standard treatment for common bile duct (CBD) stones. Post-ERCP abdominal pain is typically linked to complications like pancreatitis, perforation, or cholangitis. However, cases of nonspecific abdominal pain caused by duodenal ulcers induced by the nasobiliary drainage tube after ERCP have not been reported to date, highlighting the novelty and clinical relevance of this case. PATIENT CONCERNS: A 48-year-old male with a history of recurrent upper abdominal pain presented with persiste

Concurrent strongyloidiasis and allergic bronchopulmonary aspergillosis complicating diagnosis: case report.

Herein, we report a case of allergic bronchopulmonary aspergillosis (ABPA) complicated by Strongyloides stercoralis infection. A 73-year-old man with a history of duodenal ulcer perforation and prior Billroth II gastrectomy presented with a 14-year history of recurrent cough and wheezing, recently accompanied by new gastrointestinal symptoms. He was initially diagnosed with chronic obstructive pulmonary disease (COPD). Subsequent laboratory evaluation revealed a markedly elevated total serum IgE (1,219 kUA/L) and increased Aspergillus fumigatus-specific IgE (0.38 kUA/L, positive cutoff >0.35 k

Antimicrobial susceptibility-guided treatment is superior to empiric therapy for Helicobacter pylori infection in children.

OBJECTIVES: Helicobacter pylori (HP) clinical guidelines encourage obtaining gastric biopsy culture and antimicrobial susceptibilities to guide therapy. Susceptibility-guided treatment (SGT) reduces unnecessary exposure to ineffective antibiotics, minimizes adverse events, and promotes antimicrobial stewardship. However, real-world data on its effectiveness remains limited. We aim to compare the efficacy of SGT to empirical therapy (ET). METHODS: Retrospective cohort study, single pediatric tertiary care center, Boston Children's Hospital, January/2019 to June/2024. First-time H. pylori-histo

Impact of smoking and age on long-term recurrence after laparoscopic primary closure for duodenal ulcer perforation: a 5-year observational study.

PURPOSE: This study investigates risk factors for recurrence in patients who underwent laparoscopic primary closure (PC) for pyloroduodenal ulcer perforation (PUP). METHODS: We retrospectively analyzed data from patients who underwent laparoscopic PC with or without highly selective vagotomy (HSV) for PUP at a tertiary hospital from 2010 to 2019. Demographics, surgical outcomes, proton pump inhibitor (PPI) use, Helicobacter pylori status, and endoscopic findings were reviewed. Long-term (5 years) endoscopic and clinical outcomes regarding ulcer and perforation recurrence, were collected. RES

Severe illness as a risk factor for live burial.

Asphyxiation due to airway obstruction by sand is a rare occurrence, predominantly associated with accidental environmental or occupational hazards. Cases involving intentional live burial as a means of homicide are scarcely reported in forensic literature. This report presents an unusual case in which a severely ill victim was buried alive without resistance, ultimately succumbing to mechanical asphyxiation. A 32-year-old woman, suffering from perforated duodenal ulcer and peritonitis, was buried in a shallow grave inside a shed by the partner, under the mistaken belief that death had already

Endoscopic Removal of an Ingested Bread Bag Clip Penetrating the Pyloric Ring.

A 54-year-old man with autism presented to our hospital for the evaluation of anemia and body weight loss. The patient had mild upper right abdominal pain, and computed tomography revealed wall thickening of the duodenal bulb. Esophagogastroduodenoscopy revealed that a bread bag clip (BC) pierced the pyloric ring and that a duodenal ulcer was present at the puncture site. It was endoscopically removed using grasping forceps. Ingestion of BCs often cause gastrointestinal perforation. We recommend that emergency endoscopy be performed when BC ingestion is suspected.

Alternative approach of complicated peptic ulcer perforation in recurrent and delayed-two cases.

Gastric and duodenal ulcer perforation represents one of the most critical causes of acute abdomen, necessitating urgent surgical intervention. While primary repair with omental patch remains the standard treatment for uncomplicated cases, alternative surgical approaches may be preferable in complex or delayed presentations. We illustrate this variability through two distinct cases. Case I: A recurrent gastric perforation managed via wedge resection using linear staplers, reinforced with Lembert sutures to ensure staple-line integrity. Case II: A delayed duodenal perforation treated with Heine

Post-operative Outcomes of Laparoscopic Versus Open Repair of Perforated Duodenal Ulcer: A Systematic Review.

Perforated duodenal ulcer is a surgical emergency associated with substantial morbidity and mortality. While open repair has long been the standard, laparoscopic repair is increasingly adopted for its minimally invasive advantages. This systematic review, conducted in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines and registered with PROSPERO (CRD420251117651), compared outcomes of laparoscopic versus open repair in adult patients with perforated duodenal ulcers.
