A total of 55 gastric ulcer patients and 1045 healthy persons were investigated for 36 HLA antigens (A, B, C loci). The study revealed that HLA B12, B15, B35, Cw4 were more common in the patients (34.55, 18.18, 27.27 and 25.45%, respectively). HLA A3 (10.9% in the patients and 25.74% in those healthy) is found to be protective against gastric ulcer. The data obtained are considered with regard to heterogeneity of the disease.

A total of 30 patients (29 with peptic ulcer and 2 with erosive gastroduodenitis) received lecedil, a new famotidine blocker of histamine H2-receptors. The effect of the drug was not related to the dose regimen (20 mg twice or 40 once a day). In good tolerance side effects were not reported. Healing of duodenal ulcer occurred within 4 and 5 treatment weeks in 17 (85.0%) and 19 (95.0%) out of 20 patients, respectively, within 6 and 8 treatment weeks ulcer healed in 5 and 6 out of 8 gastric ulcer patients, respectively.

Inheritance of ulcer disease was analyzed genetico-mathematically using two methods: the method of probands and the method of siblings. Families of 351 probands were enrolled in the study: 57 had type II and 27 type III gastric ulcer disease. 267 were with duodenal ulcer disease. Recent studies strongly suggest that Helicobacter pylori might have a significant role in the pathogenesis and rate of recurrence of peptic duodenal ulcer. Various factors, such as the existence of ulcerogenic strains of the bacteria and susceptible subjects, might explain the fact that not more than 10% of the patients infected with H pylori actually have duodenal ulcer. Its association with gastric ulcer and gastric carcinoma is not well defined.

Since Helicobacter pylori was first cultured 10 years ago, there have been remarkable changes in our approach to the therapy of peptic ulcer disease. We now know that 90% of duodenal ulcer patients and 70% of all gastric ulcer patients are infected with H. pylori. The occurrence of IgG antibodies to Helicobacter pylori was measured by means of ELISA in 175 patients who had undergone surgery for duodenal or gastric ulcer. There was a significant correlation between surgery with resection and the absence of IgG to Helicobacter pylori in about 80% of cases. In patients undergoing surgery without resection there was no influence of the surgical technique on the presence of Helicobacter pylori antibodies.

Experience is recorded with the operative treatment of 185 patients with perforated gastric and duodenal ulcer over the period 1981-1991. The period was divided in two: 1981-1985 and 1986-1991.
