Prostaglandins (PG) and cyclic nucleotides (CN) were assessed radioimmunologically in the blood plasma under basal conditions and after a standard breakfast in 20 patients with duodenal ulcer, 16--with gastric body ulcer and 18 patients with atrophic gastritis. It was established that the patients showed an increased PG level, cAMP-dependent processes. Neurohumoral mechanisms of regulation of gastric secretion were abnormal.

Helicobacter pylori is strongly associated with both chronic gastritis and duodenal ulcer. Certain strains of H. pylori produce a vacuolating cytotoxin in vitro. At New Orleans Charity Hospital, concentrated culture supernatants from 119 of 144 (83%) H. pylori strains isolated from 86 patients at high risk of developing gastric cancer, caused vacuolization in HeLa S3 cells. Duodenogastric reflux (DGR) as seen on hepatobiliary scintigraphy has been reported as a useful secondary sign for the diagnosis of acute cholecystitis. We evaluated the association of reflux with cases of acute cholecystitis as compared to those with chronic cholecystitis or other conditions. The seasonal pattern of community-based acute bleeding from the upper gastrointestinal (UGI) tract was studied prospectively in 1988-1991. Out of 3343 emergency admissions to the Departments of General Surgery, 321 (9.6%) were due to acute UGI bleeding. Ultrasonography (US) is routinely used as a diagnostic approach in the surgical acute abdomen, even though its major limitation is the demonstration of the hollow viscus. This article describes four patients with perforated gastroduodenal ulcers related to smoking "crack" cocaine. All patients came to the hospital with acute abdominal pain; only one patient had a history of ulcer disease.
