Hemorrhagic Pericardial Effusion and Gastrointestinal Bleed After Atrial Fibrillation Epicardial Ablation: A Case Report.

A 47-year-old male with persistent atrial fibrillation presented with melena and presyncope 3 weeks after epicardial ablation and left atrial appendage ligation. He was found to have hemorrhagic pericardial effusion with tamponade physiology and gastrointestinal bleeding from a clean-based duodenal ulcer. Management included pericardiocentesis, anticoagulation reversal, and stabilization. This case underscores the interplay between cardiac tamponade and GI bleeding, highlighting the importance of early recognition and multidisciplinary management.

Limited utility of endoscopy in young patients without alarm features.

OBJECTIVE: Current guidelines recommend using the presence of alarm features to help determine if an upper endoscopy is appropriate as an initial diagnostic tool in young adults with dyspepsia. The study aimed to assess whether young adults without alarm features had clinically significant findings on gastroscopy. METHODS: This was a retrospective study on patients 18-50 years who underwent their first gastroscopy at Landspitali University Hospital 2018-2022. Data was collected on symptoms and endoscopic findings. American Society for Gastrointestinal Endocopy (ASGE) from 2015 were used to de

A cross-sectional study of demographic profile and incidence of Helicobacter pylori in patients with duodenal ulcer perforation.

INTRODUCTION: Duodenal ulcer Perforation (DUP) is a life-threatening complication of peptic ulcer disease, disproportionately burdening resource-limited regions. Helicobacter pylori (H. pylori) is a key etiology, yet data from Nepal are limited. This study evaluated demographics, risk factors, and H. pylori incidence in DUP patients at a Nepalese tertiary center. METHODS: A hospital-based prospective cross-sectional study (March 2024-April 2025) included 61 surgically managed DUP patients aged 18-70 years. Exclusion criteria: recent antibiotic/proton pump inhibitors use, gastric ulcers, or tr

Caffeine Overdose-Induced Diabetic Ketoacidosis With Gastrointestinal Injury in Undiagnosed Type 2 Diabetes.

Caffeine is a widely consumed stimulant contained in beverages and supplements, and intentional overdose is becoming increasingly common. Although hyperglycemia is often seen in caffeine toxicity, diabetic ketoacidosis (DKA) is a rare complication, particularly in those without a known history of diabetes. We report the case of a 46-year-old man without a known medical history who ingested 10 g of caffeine to attempt suicide. He presented with impaired consciousness, arrhythmia, hyperglycemia, and metabolic acidosis. Laboratory testing confirmed DKA, and endoscopy revealed esophageal severe er

Prediction of duodenal ulcers in children with Helicobacter pylori infection.

BACKGROUND: Helicobacter pylori (HP) infection is a major cause of duodenal ulcers (DUs) in children. DUs can lead to gastrointestinal bleeding and even intestinal perforation, both of which may be life-threatening. This study aimed to establish a predictive model for identifying DUs in children with HP infection, in order to assist primary care pediatricians with early diagnosis and intervention. METHODS: A total of 456 children with HP infection admitted to Fujian Children's Hospital and Fujian Maternal and Child Health Hospital were retrospectively included. The training group comprised 30

From duodenitis to stricture: Decoding the impact of partial annular pancreas over years.

Partial annular pancreas (PAP) is the rarer variant of annular pancreas with unknown prevalence, and is characterised by ring of pancreatic tissue partially surrounding the duodenum. PAP are frequently asymptomatic, however, symptoms may only arise in adulthood, manifesting as pancreatitis, duodenal ulceration, or, less commonly, biliary obstruction or intermittent bowel obstruction. It is often overlooked on radiological examinations due to its subtle manifestations. Even asymptomatic individuals are at risk of intestinal obstruction and pancreatitis, underscoring the importance of regular fo

Fully Covered Self-Expandable Metal Stent Placement for Management of a Perforated Duodenal Ulcer With Persistent Leak in a Decompensated Cirrhotic.

Patients with cirrhosis have a high prevalence of peptic ulcer disease, which places them at increased risk for complications such as perforation. We report the case of a patient with decompensated cirrhosis who developed a duodenal ulcer perforation following an upper endoscopy for variceal screening. Due to poor surgical candidacy, he was managed conservatively through antibiotics, percutaneous drains, and endoscopic placement of a fully covered self-expandable metal stent (fcSEMS). With no established guidelines on managing duodenal perforations in cirrhotic patients, this case demonstrates

Duodenal metastases from lung adenocarcinoma causing gastrointestinal bleeding: a rare aetiology.

Gastrointestinal metastases from primary lung adenocarcinoma are rare, with only a few reported cases of gastrointestinal bleeding caused by such metastases. Among the cancers that most commonly spread to the gastrointestinal tract are lung, breast, kidney and melanoma. Symptoms can vary, but patients are usually asymptomatic, presenting in advanced stages. We present a case of gastrointestinal bleeding secondary to a duodenal ulcer found to be a metastatic lesion from primary lung adenocarcinoma. Although the diagnosis can be challenging, especially when no lung malignancy is known yet, immun

[Intractable Hemorrhagic Duodenal Ulcer after Atezolizumab plus Bevacizumab Treatment for Hepatic Cancer with Lymph Node Metastases-A Case Report].

A 72-year-old woman with liver cirrhosis, diabetes, and parkinsonism was treated with atezolizumab plus bevacizumab (Atezo/Bev)for hepatic cancer and multiple lymph node metastases around the duodenum. After 2 doses of this treatment, alpha-fetoprotein(AFP)levels decreased, and the vascularity and size of the tumors were reduced on computed tomography. Eighteen days after the fourth dose of Atezo/Bev, the patient experienced shock due to bleeding from a duodenal ulcer and required hospitalization. Despite treatment with proton pump inhibitors, the duodenal ulcer did not show signs of healing a

Fatal Granulomatosis with Polyangiitis Presenting as Medial Longitudinal Fasciculus Syndrome with Gastrointestinal Perforation: An Autopsy Case Report.

We report a rare and fatal case of granulomatosis with polyangiitis (GPA) initially presenting as medial longitudinal fasciculus (MLF) syndrome. A 64-year-old man with a history of duodenal ulcer and hypertension presented with diplopia. Neurological examination revealed impaired adduction of the left eye with preserved convergence, consistent with MLF syndrome. Brain magnetic resonance imaging showed multiple high-intensity lesions, including one in the left dorsal pons. Laboratory testing revealed an elevated PR3 antineutrophil cytoplasmic antibody level of 205 IU/mL. A kidney biopsy on day

Unveiling the Unexpected: Duodenal Ulcer as a Manifestation of Breast Cancer Metastasis.

Breast cancer typically metastasizes to the lymph nodes, lungs, bones, and liver. However, gastrointestinal (GI) involvement is exceedingly rare, particularly presenting as duodenal ulcers. Such metastases often mimic primary GI malignancies, leading to nonspecific symptoms like abdominal pain, anorexia, bleeding, and vomiting, which can delay accurate diagnosis. Early recognition through appropriate diagnostic evaluations is crucial to prevent disease progression. We report a unique case of an 86-year-old woman with multiple comorbidities who presented with multiple gastrointestinal ulcers. F

Intramural duodenal hematoma following therapeutic endoscopy: a case report of a rare adverse event.

An intramural duodenal hematoma (IDH) is an uncommon clinical entity that can be an adverse event of therapeutic esophagogastroduodenoscopy (EGD). Herein, we present a case of a 60-year-old man who presented to the hospital with vomiting, diarrhea, and overt gastrointestinal bleeding with melena and small-volume hematemesis. EGD was pursued and revealed a bleeding duodenal ulcer that was therapeutically intervened upon to achieve hemostasis. After endoscopy, his hospital course was complicated by an intramural duodenal hematoma, which presented with abdominal pain and worsening anemia without

Analysis of Short-term Outcomes of Pancreatic Resections from a Low Volume Centre in a Tier II City in India.

BACKGROUND AND OBJECTIVE: Advanced age, co-morbid diseases, functional dependence and frailty are associated with polypharmacy and overall high health expenditures. Polypharmacy is commonly defined as the concomitant ingestion of four or more medications, particularly in community-dwelling patients, but the number of drugs being taken by institutionalised and hospitalised patients may be as high as ten. The aim of this study was to compare drug use in newly admitted patients (AP) to a geriatric medical centre for long-term care (LTC) with that of institutionalised patients (IP) at the centre.

A Trace Element-Ulcer Map: Decoding Micronutrient-Ulcer Relationships Through Genetic Architecture and Pleiotropy-Aware Inference.

The contribution of circulating micronutrients to ulcer susceptibility remains poorly defined across anatomical sites. In this study, we constructed a systematic trace element-ulcer map by integrating genetic-instrumented inference, pleiotropy-aware modeling, and heterogeneity-sensitive clustering. Summary-level data were obtained from large-scale genome-wide association studies (GWAS) encompassing 10 circulating micronutrients (calcium, iron, zinc, copper, magnesium, selenium, carotene, vitamin B12, vitamin C, and vitamin D) and nine ulcer phenotypes (corneal ulcer, recurrent oral aphthae, es

Causal relationship between COVID-19, vaccination, and 20 digestive diseases: a comprehensive two-sample Mendelian randomization study.

BACKGROUND: Sequelae and complications have become a significant concern in the post-pandemic era of Coronavirus disease 2019 (COVID-19). However, it remains unclear whether there is a direct causal relationship between COVID-19 or vaccination and digestive diseases, as existing evidence is ambiguous and controversial. In this study, we investigated the associations between multiple COVID-19 infection phenotypes, vaccination, and 20 common digestive diseases, and explored their causal relationships through extensive Mendelian randomization (MR) analysis. METHODS: For individuals of European d

Teaching Caregivers to Implement the Picture Exchange Communication System: An Evaluation of Video Modeling.

BACKGROUND: Around 60% to 80% of patients with periampullary carcinoma are unresectable either due to distant metastasis or local vascular invasion. With the advancement of endoscopic interventional procedures, the role of surgical bypass has diminished. However, surgical bypass is still appropriate in patients with unresectable disease discovered at the time of surgery. This study was conducted to assess the results of palliative surgical bypass for patients with unresectable periampullary carcinoma at our hospital, a tertiary referral center of Northern India. METHOD: The study group compri

Secondary aorto-duodenal fistula: a diagnostic challenge in a patient with fever and anemia.

A 69-year-old male, three years post-endovascular exclusion for an abdominal aortic aneurysm, presented with asthenia and fever. An abdominal CT scan showed no gastrointestinal tract communications, abscess, or contrast extravasation. Tc-99m-HMPAO-labeled leukocytes scintigraphy with SPECT/CT revealed increased uptake on the posterior surface of the aortic graft, along with air bubbles in its right iliac limb. Upper gastrointestinal endoscopy was performed, revealing a duodenal ulcer in the transition between the second and third portions. The ulcer exhibited yellow graft tissue at its center.

Before the Potty Party: Preparing Practitioners for Toilet Training.

Air pollution due to the presence of small particles and gases in the atmosphere is a major cause of health problems. In urban areas, where most of the population is concentrated, traffic is a major source of air pollutants (such as nitrogen oxides or NO x and carbon monoxide or CO). Therefore, for smart cities, carrying out an adequate traffic monitoring is a key issue, since it can help citizens to make better decisions and public administrations to define appropriate policies. Thus, citizens could use these data to make appropriate mobility decisions. In the same way, a city council can ex
