Large gastroduodenal artery pseudoaneurysm and arterioportal fistula in chronic pancreatitis.

Visceral artery pseudoaneurysms, particularly those in the gastroduodenal artery (GDA), are rare but serious complications associated with chronic pancreatitis, posing a significant risk of rupture due to their structural fragility. In this case, a 61-year-old male with a history of chronic pancreatitis, alcohol cirrhosis, duodenal ulcer, and COPD presented with persistent abdominal pain and recurrent fevers. Imaging revealed a 7 cm pseudoaneurysm between the GDA and superior mesenteric vein, which was successfully treated with coil embolization. This case highlights the importance of prompt r

Comparative incidence and risk factors for gastrointestinal bleeding following percutaneous coronary intervention for coronary artery disease: Insights from the Keio Cardiovascular Registry in Japan.

BACKGROUND: In patients with coronary artery disease (CAD) undergoing percutaneous coronary intervention (PCI), antiplatelet medication usage is crucial for preventing thrombotic events. However, it requires careful monitoring, especially because of the risk of life-threatening bleeding complications. In hemorrhagic complications, assessment of patient background and risk of gastrointestinal bleeding (GIB) remain limited for GIB that develops during long-term observation after hospital discharge. This study aimed to examine the incidence of GIB and patient characteristics in CAD post-PCI. MET

The Causal Effects between Mood Swings and Gastrointestinal Diseases: A Mendelian Randomization Study.

BACKGROUND: Numerous studies have examined the links between mental disorders such as depression and bipolar disorder, and gastrointestinal (GI) diseases. However, few studies have investigated the link between mood swings and GI diseases. Given the impact of mood swings on various conditions and the growing comprehension of the gut-brain axis, this study aims to explore their causal relationship using Mendelian randomization (MR) methods. METHODS: Single-nucleotide polymorphisms (SNPs) associated with mood swings were obtained from a recent study. SNPs associated with GI diseases were identi

An Unusual Presentation of Juvenile Systemic Lupus Erythematosus.

Systemic lupus erythematosus (SLE) is an autoimmune disease involving multiple organ systems that often mimic other conditions. The majority of patients with SLE show mucocutaneous manifestations, fatigue, fever, rheumatological manifestations, and weight loss as initial symptoms. SLE is classically managed medically with hydroxychloroquine, glucocorticoids, and/or immunosuppressives. Juvenile-onset systemic lupus erythematosus (jSLE) patients, who account for about 20% of SLE patients, tend to have earlier development of end-organ damage, and increased need for immunosuppressive therapies. We

A rare case of Behçet's disease complicated by Budd-Chiari syndrome and perforated duodenal ulcers in a young male.

Behçet's disease (BD) is characterized by skin lesions, uveitis, and recurrent oral and genital ulcers. Vascular problems, predominantly affecting veins, lead to thrombosis, increasing the risk of ruptured artery aneurysms and Budd-Chiari syndrome (BCS). Morbidity and mortality are significantly heightened by rare occurrences such as pulmonary artery aneurysms, cardiac involvement, and BCS. Prompt diagnosis and treatment are pivotal for prognosis improvement, particularly in males with early onset. We present a case of a 16-year-old male with BD history, who developed abdominal distension, ped

[Simultaneous Quadruple Therapy In The Treatment Of Helicobacter Pylori During Peptic Ulcer Disease].

UNLABELLED: Helicobacter pylori (Hp) infection plays an important role in the genesis of peptic ulcer disease. In the absence of susceptibility testing, concomitant quadruple therapy is one of the most widely used eradication methods. In the absence of a study on four-drug therapy and the difficulty of studying antibiotic susceptibility in our context, we initiated this work in order to evaluate the efficacy of an empirical treatment with concomitant four-drug therapy currently available. METHOD: This was a prospective study from May 2020 to April 2021 in the Hepato Gastroenterology Departmen

A rare case of endoscopic visualization of a metallic coil in a duodenal ulcer after transcatheter arterial embolization.

Transcatheter arterial embolization is the therapy of choice for recurrent peptic ulcer bleeding refractory to standard endoscopic hemostatic techniques. It offers a minimally invasive approach with high efficacy, lower mortality, and complication rates compared to surgery. However, rare adverse events attributed to coil migration including pulmonary embolism, stroke, myocardial infarction, and bowel obstruction have been reported. We report the case of a 72-year-old female with a massive duodenal ulcer bleed refractory to endoscopic hemostatic techniques of epinephrine injection and bipolar c

Small bowel obstruction secondary to impacted pancreatic necrosum through a spontaneous duodenal fistula: a first in the literature.

A 58-year-old man presented with alcohol-induced acute pancreatitis. Imaging revealed complete necrosis of the pancreatic parenchyma. After initial non-operative management, the patient developed a duodenal ulcer and an upper gastrointestinal bleed and later spontaneously formed a fistula between the pancreas and the first segment of the duodenum. Through this fistula, the entire pancreatic necrosum migrated to the distal small bowel, where it became entrapped as a bezoar, causing a high-grade small bowel obstruction. The patient then proceeded to laparotomy, where the obstruction was resolved

Genetic insights into the gut microbiota-duodenal diseases interplay: A Mendelian randomization and Bayesian weighting study.

BACKGROUND: Many observational studies have shown a close association between gut microbiota and the risk of various duodenal diseases. Therefore, we urgently explore the potential causal relationship between gut microbiota and some duodenal diseases, mainly including duodenal ulcers and duodenitis. METHODS: We conducted Mendelian randomization (MR) studies using genetic instrumental variables for gut microbiota from GWAS and duodenal disease datasets. Causal relationships were examined using multiple MR methods with Bonferroni correction. Bayesian Weighted Mendelian Randomization (BWMR) asse

Sodium polystyrene sulfonate as an additional contributing factor to repeated gastric ulcers among other multiple factors in a patient undergoing hemodialysis: a case report.

BACKGROUND: Sodium polystyrene sulfonate (SPS) is a cation-exchange resin used to treat hyperkalemia. Although colorectal ulcers are known side effects of long-term SPS use, few studies have reported SPS-associated gastric ulcers. Herein, we report a case of repeated gastric ulcers during SPS administration. CASE PRESENTATION: The patient was a 55-year-old man who was on SPS treatment of hyperkalemia since the initiation of hemodialysis (HD) at the age of 51 years. At the age of 54 years, he started taking vonoprazan fumarate after developing a bleeding duodenal ulcer. The patient underwent l
