Association Between Rheumatoid Arthritis and Gastrointestinal Comorbidities: Evidence From Taiwan's National Health Insurance Database.

BACKGROUND: Rheumatoid arthritis (RA) is a systemic autoimmune disease with known extra-articular manifestations, including potential involvement of the gastrointestinal (GI) tract. While prior studies have explored malignancy risks in RA, limited large-scale data exist on the association between RA and both cancerous and non-cancerous GI comorbidities. METHODS: This retrospective nested case-control study utilized Taiwan's National Health Insurance Research Database (NHIRD) from 2000 to 2019. A total of 75 670 individuals newly diagnosed with RA were matched 1:4 by age and sex to 302 680 con

Abdominal Compartment Syndrome Following Massive Transfusion in Upper Gastrointestinal Bleeding: A Case Report.

Abdominal compartment syndrome (ACS) is defined as sustained intra-abdominal pressure (IAP) exceeding 20 mmHg, leading to organ hypoperfusion and dysfunction. Although most cases arise from intra-abdominal or retroperitoneal hemorrhage, ACS may also occur after massive transfusion due to capillary leak and fluid overload. We describe an 82-year-old man presenting with upper gastrointestinal bleeding from a duodenal ulcer who required massive transfusion. During resuscitation, he developed progressive ventilatory failure with rising airway pressures and refractory hypoxemia. Intra-vesical IAP m

[Gastrointestinal complications of dual antiplatelet therapy in patients with ischemic heart disease: risk stratification, current management tactics. A review].

Coronary heart disease is one of the leading causes of death worldwide, and antiaggregant therapy is crucial for both its prevention and treatment. The review article presents the data of clinical trials that allow to determine the tactics of management of patients at high risk of gastrointestinal complications on the background of dual antiaggregant therapy. The main risk factors for complications are described, such as: acetylsalicylic acid dose, ulcer or bleeding from the upper GI tract in anamnesis, advanced age, concomitant use of anticoagulants, non-aspirin non-steroidal anti-inflammator

Refractory duodenal hemorrhage in the setting of aberrant SMA anatomy and celiac stenosis: a surgical rescue case report.

INTRODUCTION: Upper gastrointestinal bleeding (UGIB) remains a critical clinical challenge, with peptic ulcer disease accounting for up to two-thirds of cases. While most bleeding duodenal ulcers respond to endoscopic therapy, refractory hemorrhage, particularly in the presence of arterial involvement or vascular anomalies, may necessitate escalation to interventional radiology (IR) or surgery. CASE PRESENTATION: We present the case of a 64-year-old woman with a bleeding duodenal ulcer arising from an aberrant branch of the superior mesenteric artery in the setting of severe celiac artery ste

Molecular characterization of H. pylori: genetic diversity and vacA s1m1/cagA+ prevalence in Western Algeria.

Clinical outcomes of Helicobacter pylori infection range from chronic gastritis to peptic ulcer disease and gastric cancer. Yet data on strain diversity and clinical outcomes remain scarce in North Africa. This study aimed to characterise enzymatic and genetic variation among H. pylori isolates from symptomatic patients in western Algeria. From 428 patients, 72 H. pylori strains were isolated and purified; for genotypic analyses, 14 distinct strains were retained based on phenotypic diversity, clinical profiles, and DNA quality/quantity. Nitrate reductase activity, digestion of htrA and ureB,

[Perforated duodenal ulcer after laparoscopic Roux-en-Y gastric bypass].

In this case report, we present a 55-year-old woman with a history of Roux-en-Y gastric bypass who developed a perforated duodenal ulcer in the gastric remnant. Diagnosing this condition was challenging, as CT showed no free air in the abdomen - a typical limitation in this patient group because air in the gastric remnant is absorbed over time. Therefore, this case report highlights the need for increased awareness of this rare but serious condition and emphasises maintaining a low threshold for intervention.

Association of depression and gastrointestinal diseases: a three-stage study.

BACKGROUND: Depression as a mental illness is commonly observed to co-occur with various somatic diseases, such as gastrointestinal diseases. However, previous studies have primarily focused on the risk of mental disorders following physical illnesses. Our study took depression as a risk factor, attempting to explore its relationship with gastrointestinal diseases. METHODS: A total of 457,940 participants (aged 37-73 years) in the UK Biobank were included. The Cox proportional hazards model was used to assess the relationship between depression and gastrointestinal diseases. Mendelian randomi

Case report - giant duodenal ulcer with Candida infection in a relatively healthy child.

INTRODUCTION: Duodenal candidiasis in an immunocompetent individual, particularly in a child, is extremely rare. Since the clinical presentation and pathological findings of fungal infections in the gut can be non-specific, it is essential to consider fungal infections as a possible cause of gastrointestinal ulcers, even in immunocompetent individuals without a history of chronic use of antacids, non-steroidal anti-inflammatory drugs, or antibiotics. CASE PRESENTATION: We report a rare case of a giant duodenal ulcer complicated by Candida infection in an otherwise healthy 10-year-and-7-month-o

Nonvariceal Upper GI Bleeding in Established Users of Proton-Pump Inhibitors: Higher Incidence But Better Outcomes.

INTRODUCTION: Nonvariceal upper gastrointestinal bleeding (NVUGIB) still occurs despite taking proton-pump inhibitors (PPIs). We tested the hypothesis that PPI efficacy fades with time. METHODS: This is a prospective and controlled observational comparison of cases of NVUGIB arising from the adult PPI-users' and nonusers' subpopulations of a well-defined community. Patients presenting with NVUGIB were grouped into PPI-users (index group) and nonusers (control group). RESULTS: The incidence of NVUGIB in our overall adult population was 89.7 in 2019 and 115.1 per 100,000 in 2022 ( P = 0.01). O

Pedicled Gallbladder Serosal Patch for Complex Duodenal Perforation: Technical Description and Clinical Outcome.

Perforated peptic ulcers are surgical emergencies typically treated by omental patch repair. This report describes the first successful pedicled gallbladder flap for duodenal perforation repair in the setting of inadequate patch material. A previously healthy 28-year-old male was taken emergently for surgery after presenting with intra-abdominal sepsis in the setting of rapid, unintentional weight loss. Findings of feculent peritonitis and multiple colonic perforations were managed with a subtotal colectomy and temporary abdominal closure. At re-exploration, a perforated duodenal ulcer was ide

Exposure to air pollution and risk of gastrointestinal diseases: a systematic review and meta-analysis of epidemiological evidence.

Gastrointestinal (GI) diseases represent a significant global health burden, and emerging evidence suggests air pollution may be a risk factor. We reviewed relevant literature and performed a meta-analysis. We searched original studies investigated the associations between long- and short-term exposure to particulate matter (diameter ≤ 2.5 µm [PM2.5], ≤ 10 µm [PM10]) and gaseous pollutants (nitrogen dioxide, sulfur dioxide, carbon monoxide, and ozone [O3]) and GI diseases. Difference in the risk of gastrointestinal manifestations between peritoneal and hemodialysis patients: a systematic review and meta-analysis.

OBJECTIVE: A large number of patients on dialysis have gastrointestinal (GI) manifestations. However, it remains unclear whether dialysis modality affects their prevalence. We present the first systematic review and meta-analysis of literature comparing the risk of GI manifestations between hemodialysis (HD) and peritoneal dialysis (PD). METHODS: Comparative studies were searched on Embase, PubMed, and Scopus from January 1, 2000, to April 1, 2025. Any GI manifestation reported by at least two studies were quantitatively analysed to obtain the odds ratio (OR) in a random-effect meta-analysis

Efficacy and safety of upper gastrointestinal endoscopy in the neonatal intensive care unit.

BACKGROUND: Upper gastrointestinal (GI) disorders frequently lead to neonatal intensive care unit (NICU) admissions; however, conventional imaging often fails to detect atypical presentations. The development of miniaturized endoscopes has progressively expanded the feasibility of upper GI endoscopy in neonates. Evidence regarding the diagnostic utility and safety of upper GI endoscopy in neonates remains limited, and neonatal-specific guidelines are currently absent in China. This study aimed to evaluate the diagnostic value and safety of upper GI endoscopy in NICU infants. METHODS: We retros

A Case of Refractory Peptic Ulcer with Choledochoduodenal Fistula Successfully Treated by Biliary Stent Placement Using an Ultrathin Endoscopic Rendezvous Technique: A Case Report.

Choledochoduodenal fistula (CDF) is an abnormal communication between the common bile duct and the duodenum. While gallstones are the primary cause, peptic ulcer-related CDF has become rare due to the widespread antiulcer therapy. Nevertheless, CDF remains clinically relevant as symptoms are often vague and diagnosis is frequently incidental. We report the case of a 79-year-old woman with vomiting, epigastric pain, and back pain due to a refractory duodenal ulcer complicated by CDF. Endoscopic retrograde cholangiopancreatography (ERCP) using a standard duodenoscope was attempted to facilitate
