Post-traumatic stress disorder, attention deficit and hyperactivity disorder, and 24 gastrointestinal diseases: Evidence from Mendelian randomization analysis.

Post-traumatic stress disorder (PTSD) and Attention deficit and hyperactivity disorder (ADHD) are common mental illnesses. Observational studies have indicated that these conditions often co-occur with gastrointestinal diseases. However, the causal relationship between PTSD and ADHD with gastrointestinal diseases remain unclear. We conducted Mendelian randomization (MR) analysis to investigate these associations. We selected genetic instrument data with genome-wide significance levels for PTSD and ADHD from the psychiatric genomics consortium open genome-wide association study platform. Summar

Dissecting the bidirectional associations between the progression of gastrointestinal and endocrine diseases.

BACKGROUNDS: The widespread intrinsic link between gastrointestinal and endocrine diseases was poorly understood. We aimed to dissect the bidirectional association in the progression of gastrointestinal with endocrine diseases, either the overall, individual, or comorbidities with each other. METHODS: A bidirectional-designed prospective cohort included 481841 and 452858 participants free of gastrointestinal and endocrine diseases at baseline in the UK Biobank. Multivariable Cox proportional hazard models were used to estimate the hazard ratios (HRs) and 95% confidence interval (CI) for incid

The central role of gastrin in Helicobacter pylori gastric carcinogenesis.

Gastric cancer is still a prevalent and lethal cancer. Gastric hypoacidity and gastritis have long been recognized in the pathogenesis. The identification of Helicobacter(H.) pylori as the main cause of gastritis leading to peptic ulcer disease and gastric cancer was a breakthrough. H. pylori was the first bacterium accepted as a carcinogen. The mechanism was not found before H. pylori was shown to predispose to cancer only after having induced oxyntic atrophy incriminating reduced killing of microorganisms and/or secondary hypergastrinemia. H. pylori has an uncertain carcinogenic role in card

CT assessment of complications from gastric or duodenal ulcers.

Gastric and duodenal ulcers (GDUs) are decreasing both in frequency and rate of mortality. However, GDUs with complications continue to have higher mortality rates and are seldom suspected in patients presenting with acute abdomen. Although upper endoscopy plays a central role in the diagnosis and treatment of GDUs, CT is crucial when serious complications appear, namely uncontrolled acute haemorrhage, perforation, organ penetration, or gastric obstruction. Radiologists have a special role to play in these situations, providing the diagnosis and extent of complications, thus contributing to de

Laparoscopic surgery in acute perforated gastroduodenal ulcers: an underutilized technique despite favorable short-term outcomes.

BACKGROUND: Acute perforation is a serious complication associated with peptic ulcer disease (PUD). Laparotomy remains the traditional operative intervention for repair. However, the laparoscopic approach has gained popularity over the last two decades. This study aims to evaluate the 30-day outcomes of laparoscopic and open approach in patients with acute PUD perforation. METHODS: Patients who presented emergently with acute gastroduodenal ulcer perforation between 2005 and 2017 were identified from the ACS-NSQIP database. Patients were stratified based on whether they underwent laparoscopic

Fournier's Gangrene and Pneumothorax Secondary to Nontraumatic Duodenal Perforation.

Peptic ulcer disease (PUD) results from erosion and ulceration of the upper digestive tract mucosa. Clinical presentations can vary from asymptomatic to severe complications such as perforation, strictures, or bleeding. Perforation can release enteric contents and gas into the abdomen, leading to intra-abdominal sepsis, requiring surgical intervention for source control and repair. We present a case of a 69-year-old male who developed both Fournier's gangrene and a right-sided pneumothorax secondary to a nontraumatic perforated duodenal ulcer. The patient underwent an emergent thoracostomy, la

Gastric Adenocarcinoma at the Anastomotic Site Three Decades after Gastrojejunostomy: A Case Report with Literature Review.

Malignancy at the gastrojejunostomy site, though rare, is a recognized long-term risk factor of the procedure. Such malignancies, though uncommon, pose diagnostic and therapeutic challenges. We report a case of a 63-year-old male presenting with a rare complication of gastric carcinoma at the loop gastrojejunostomy site 30 years after surgery for duodenal ulcer-induced gastric outlet obstruction. The patient's symptoms included chronic intermittent upper abdominal pain, postprandial bilious vomiting, significant weight loss, and anorexia. The diagnosis was established through imaging and endos

Endoscopic Management of Complex Duodenal Ulcer Bleeding Using Over-the-Scope Clips: Clinical Insights From a Regional Core Hospital.

OBJECTIVES: Over-the-scope clips (OTSCs) are considered an effective endoscopic tool for managing upper gastrointestinal bleeding, including duodenal ulcers, mostly based on data from high-volume centers with expert endoscopists. This study aimed to evaluate the clinical safety of OTSCs in regional hospital backgrounds and identify the factors associated with unsuccessful hemostasis. METHODS: We conducted a retrospective study of 30 patients with duodenal ulcer bleeding who underwent OTSC placement at a regional core hospital in Japan between April 2014 and January 2025. Diagnostic accuracy of computed tomography in localizing gastrointestinal perforations: Focusing on gastric and duodenal defects.

PURPOSE: To provide a detailed assessment of the accuracy and reproducibility of Computed Tomography (CT) in localizing gastrointestinal perforations, with a focus on distinguishing gastric and duodenal defects. METHODS: This single-center retrospective study evaluated subjects with surgically confirmed gastrointestinal perforation who underwent preoperative contrast-enhanced abdominopelvic CT. Two subspecialized radiologists reviewed each CT independently to identify the perforation site, to differentiate gastroduodenal from perforations in other locations, and to differentiate between gastr

Duodenal ulcer perforation peritonitis as a cause of acute abdomen in pediatric population: a rare case report and review of literature.

INTRODUCTION: Acute abdomen in pediatric patients presents diagnostic challenges due to the wide range of potential causes and overlapping clinical features. Duodenal ulcer perforation, though rare, can mimic more common conditions like appendicitis, complicating diagnosis and management. Perforated peptic ulcer disease (PUD) in children, especially without chronic nonsteroidal anti-inflammatory drug (NSAID) use or Helicobacter pylori infection, is uncommon but requires prompt recognition. CASE PRESENTATION: A 9-year-old male presented with severe diffuse abdominal pain, fever, and a history

Mochi-Induced Duodenal Obstruction and Ulcers: A Case Report.

Mochi, or rice cake, is a highly adhesive food, and it is known to cause small bowel obstruction and gastric ulcers. However, reports of duodenal involvement due to mochi are rare. We report a case of duodenal obstruction and ulcers caused by mochi. A 64-year-old man with a history of diabetes mellitus presented with abdominal pain and vomiting after consuming mochi. Computed tomography (CT) of the abdomen revealed multiple high-density materials in the duodenum and stomach. Endoscopy identified multiple ulcers in the duodenum and stomach, along with mochi impaction in the duodenum. The obstru

Pancreatico-cutaneous fistula following complicated perforated peptic duodenal ulcer repair.

Pancreatic fistulas are a known complication following gastrointestinal surgeries, especially when there is disruption to pancreatic tissue or ducts. These fistulas occur due to leakage of pancreatic enzymes, which can lead to inflammation, infection, and other complications. While typically associated with pancreatic resections, fistulas can develop when the ulcer involves the pancreas leading to pancreatic ductal disruption. A 59-year-old female presented with hyperacute epigastric pain, hematemesis, septic shock with a blood pressure of 89/62, a Boey score of 3, and a Pulp score of 4. Imagi

Treatment of a Large Gastric Phytobezoar With Coca-Cola, Cellulase, and Endoscopic Fragmentation in a Community Hospital.

Bezoars are indigestible masses that can cause gastrointestinal complications such as obstructions and perforations. We report the case of a 60-year-old man with a history of type 2 diabetes mellitus and duodenal ulcer surgery. The patient developed a large gastric phytobezoar that caused persistent dyspepsia and epigastric pain. Upper endoscopy confirmed a 10-cm phytobezoar and a 1.5-cm active ulcer at the gastric angle. To soften the bezoar, Coca-Cola and cellulase were administered to the patient. This treatment significantly reduced the bezoar's hardness, permitting its successful endoscop

Gastric cancer risk after Helicobacter pylori eradication in gastritis and peptic ulcer: a retrospective cohort study in Japan.

BACKGROUND: Helicobacter pylori infection is an important risk factor for gastric cancer. In Japan, national health insurance has covered eradication therapy for H. pylori infection-associated gastritis from 2013. However, gastric cancer was the fourth leading cause of cancer death in 2023. We aimed to investigate differences in gastric cancer risk among patients with gastritis, gastric ulcer, duodenal ulcer, and gastric ulcer and duodenal ulcer after H. pylori eradication. METHODS: This retrospective cohort study used the JMDC Claims Database from February 21, 2013, to August 31, 2023. Patie
