Helicobacter pylori (H. pylori) infects approximately half of the global population and is closely linked to chronic gastritis, peptic ulcers, and gastric cancer. Antibiotic resistance has become a major global health concern with marked regional variations, severely undermining empirical treatment efficacy. Resistance mechanisms primarily involve target gene mutations, efflux pump overexpression, and altered membrane permeability. This review summarizes resistance mechanisms of commonly used antibiotics against H. pylori, along with diagnostic approaches ranging from phenotypic to molecular m

Gastric precancerous states are important intermediate stages in gastric carcinogenesis, yet strategies that directly modify their persistent biological drivers remain limited. Current management mainly includes Helicobacter pylori eradication, endoscopic surveillance, and mucosal protective strategies. Biotics, including probiotics, synbiotics, postbiotics, and next-generation microbial products, have been investigated as potential strategies to modulate persistent microbial, metabolic, inflammatory, and barrier alterations. However, existing evidence derives from studies of H. pylori-associa

BACKGROUND: The human gastric microbiota is recognized as a diverse and metabolically active ecosystem with significant roles in health and disease. Beyond probiotics, postbiotics, non-viable microbial products with antimicrobial and anti-biofilm properties, offer stable and safer therapeutic alternatives. Rising antimicrobial resistance and biofilm associated infections caused by pathogens such as Staphylococcus aureus and Candida albicans highlight the urgent need for new strategies. Based on this evidence, the present study aimed to isolate bacterial strains from gastric biopsies of patient

BACKGROUND: Gastritis cystica profunda (GCP) is an uncommon gastric lesion characterized by displacement of cystically dilated glands into the submucosa. It may present as a submucosal mass mimicking gastrointestinal stromal tumor (GIST). The coexistence of GCP with high-grade dysplasia is extremely uncommon and may pose a significant diagnostic challenge by simulating invasive carcinoma.
CASE PRESENTATION: We report a case of a 54-year-old patient whose endoscopic and radiographic features suggested a GIST in the gastric cardia. Surgical resection was performed. Histopathological examination

Helicobacter pylori (H. pylori) infection is a major cause of chronic gastritis, peptic ulcer disease, and gastric cancer, while increasing antimicrobial resistance has become a major challenge to successful eradication. This study investigated the phenotypic antimicrobial susceptibility patterns of H. pylori isolates from Egyptian patients and evaluated their associations with clinical characteristics. Gastric biopsy specimens were collected from 60 patients undergoing upper gastrointestinal endoscopy. H. pylori was isolated by culture under microaerophilic conditions and identified using con

H. pylori infection is a major risk factor for chronic gastritis and gastric cancer, and antibiotic resistance severely limits the efficacy of current eradication therapies. In our present study, a sulfated galactan (named as AP-3) was isolated from abalone viscera with a novel structure. The results showed that the molecular weight (Mw) of AP-3 was approximately 10.2 kDa, which was composed of galactose (85.52%), glucose (6.65%), glucuronic acid (5.90%) and fucose (1.93%). The glycosidic linkage of the AP-3 main chain was →4)-α-D-galactose-(1 → with branches at C-3 of the backbone and sulfate

Potassium-competitive acid blockers (P-CABs) provide more rapid, profound, and sustained gastric acid suppression than conventional proton pump inhibitors (PPIs) and are increasingly used for acid-related disorders. Emerging evidence from Japan suggests that vonoprazan may also be effective in eosinophilic esophagitis (EoE), where PPIs are already established first-line therapy. However, comparative studies have thus far shown broadly similar symptomatic, endoscopic, and histological responses with P-CABs and PPIs, suggesting that antisecretory potency alone may not determine therapeutic effic

Alpha-gal syndrome (AGS) is a tick-borne immunoglobulin E (IgE)-mediated hypersensitivity reaction to galactose-α-1,3-galactose (alpha-gal) found in mammalian meat products. Eosinophilic gastrointestinal disorders (EGIDs), including eosinophilic esophagitis (EoE), are T helper 2 (Th2)-mediated conditions characterized by eosinophilic infiltration of the gastrointestinal tract. We present a case of a 63-year-old man with known AGS who developed dysphagia, rash, pulmonary infiltrates, gastrointestinal eosinophilic infiltrates, and marked eosinophilia. Bone marrow biopsy revealed marked hypercell

Gastric tuberculosis represents a rare form of extrapulmonary tuberculosis. Owing to its non-specific clinical presentation, the condition is highly prone to misdiagnosis. It can mimic peptic ulcer disease or malignancy, typically manifesting as ulcerative lesions or gastric outlet obstruction, whereas less frequent presentations include massive gastrointestinal haemorrhage and gastric perforation. Consequently, establishing a definitive diagnosis remains challenging. This case report aims to augment the current understanding of this entity. Herein, we describe a 50-year-old female who present

The discovery of Helicobacter pylori has revolutionized the prevention and treatment of gastritis, peptic ulcers, and, most important, gastric cancer. In the past 40 years, enormous progress has been achieved in the clinical and basic research on H. pylori infection. However, challenges in preventing and treating H. pylori infection exist owing to the lack of an H. pylori vaccine and the ever-increasing resistance to antibiotics worldwide. This review summarizes the history of and key milestones in H. pylori research, provides an update on the understanding of the pathogenesis and global burde

BACKGROUND: Gastric immunoglobulin G4-related disease (IgG4-RD) is exceedingly rare, with fewer than 50 cases reported worldwide. Its mass-forming presentation frequently mimics malignancy, leading to high rates of unnecessary surgical resection. Inebilizumab, a CD19-directed monoclonal antibody and the first FDA-approved therapy for IgG4-RD, has not previously been described in gastric IgG4-RD. We report the first case of mass-forming gastric IgG4-RD treated with inebilizumab, achieving clinical, endoscopic, and histologic remission without surgical intervention.

BACKGROUND: Gastritis diagnosis via endoscopy remains highly subjective, often leading to inter-observer variability and the misdiagnosis of early inflammatory changes as congestive portal gastropathy. While Computer-Aided Diagnosis (CAD) shows promise, the "paucity of labelled data" remains a significant barrier to clinical implementation.
AIM AND OBJECTIVES: This study aimed to develop and validate a scalable deep-learning solution using a hybrid CNN-Vision Transformer (ViT) architecture to assist endoscopists in the real-time severity stratification of gastritis (Normal, Mild, Moderate, an

Emphysematous gastritis (EG) is a rare, potentially life-threatening condition. Extensive gastric pneumatosis and portal venous gas may suggest irreversible ischemia, although radiological severity and mucosal injury do not necessarily indicate transmural necrosis. A 70-year-old man presented with acute epigastric pain and vomiting. Computed tomography (CT) demonstrated extensive gastric and duodenal pneumatosis with portomesenteric venous gas but patent visceral arteries. Immediately after CT, he developed cardiac arrest, with return of spontaneous circulation after approximately 14 min. Emer

Helicobacter pylori (H. pylori) is a highly adapted gastric pathogen responsible for one of the most prevalent chronic bacterial infections worldwide and a major cause of gastritis, peptic-ulcer disease, and gastric cancer. Traditionally considered predominantly a human-associated microorganism transmitted via oral-faecal routes, growing evidence challenges this paradigm by demonstrating its persistence outside the human host through interactions with free-living amoebae (FLA). This review critically examines current knowledge of the biological and ecological interplay between H. pylori and FL

Gastric ischemia is an uncommon entity, as the stomach has an extensive collateral arterial blood supply; clinically significant injury usually requires multivessel mesenteric compromise. When it occurs, it closely mimics peptic ulcer disease and gastric malignancy, and diagnosis is often delayed. A 77-year-old female patient with poorly controlled type 2 diabetes mellitus and dyslipidemia (non-compliant with metformin and atorvastatin) presented with a one-month history of severe postprandial epigastric pain, progressive food avoidance, and 10 kg of unintentional weight loss. She was admitted

Fecal occult blood testing is a widely used diagnostic method for the early detection of colorectal cancer and the assessment of gastrointestinal pathologies in humans. For preclinical research purposes, we developed a cost-effective, non-invasive luminol-based chemiluminescent assay to detect fecal occult blood in murine feces that can function as a predictive tool, with sufficient sensitivity to correlate with disease severity. Fecal samples were collected and weighed, solubilized in a sodium hydroxide solution, then combined with luminol and hydrogen peroxide and immediately analyzed using

INTRODUCTION: Helicobacter pylori infection is a leading cause of peptic ulcer disease, yet amoxicillin trihydrate (AMX) efficacy is limited by its acid-catalyzed degradation and rapid gastric emptying. This study aimed to develop alkalizer-based gastroretentive floating tablets to enhance localized gastric-delivery and improve AMX stability under gastric conditions.
MATERIALS AND METHODS: Floating tablets were prepared by direct compression using HPMC K100M, sodium-bicarbonate and microcrystalline-cellulose. A 23 factorial-design (RStudio-v4.3.3) was used to optimize microenvironmental pH, f

Chronic atrophic gastritis (CAG) progresses through distinct pathological stages to gastric cancer. This computational study employed network pharmacology and bioinformatics to predict the active compounds, targets, and pathways of five medicinal-food homologous plants (Houttuyniae Herba, Mume Fructus, Rhizoma Dioscoreae, Portulacae Herba, and Curcumae longae Rhizoma) against this progression. We identified 28 active compounds and 242 targets from TCMSP. differentially expressed genes from GSE130823 and GSE55696 were integrated with weighted gene co-expression network analysis, yielding 129 ca

BACKGROUND/AIMS: To evaluate the diagnostic performance of an artificial intelligence (AI)-based optical system for chronic atrophic gastritis (CAG) and intestinal metaplasia (IM), and its ability to predict metachronous recurrence.
METHODS: We analyzed 72,794 endoscopic images from 5,000 patients undergoing esophagogastroduodenoscopy with biopsy-confirmed CAG/IM. A convolutional neural network model was developed and compared with experts (n=16) and novices (n=15). The association between AI-predicted IM extent and metachronous recurrence after endoscopic resection of early gastric cancer (n
