Modulation of Ferroptosis: A Novel Mechanism of Traditional Chinese Medicine in Managing Chronic Atrophic Gastritis.

Chronic atrophic gastritis (CAG) is a major precancerous lesion of gastric cancer, characterized by a high incidence rate, a protracted clinical course, and a well-documented risk of malignant transformation. Current Western medical treatments exert limited efficacy in reversing progressive gastric mucosal damage. In traditional Chinese medicine (TCM), CAG is categorized as expressing symptoms such as "stomachache" and "abdominal fullness," with the core pathogenesis defined as "spleen-stomach deficiency as the root, and qi stagnation, blood stasis, and damp-heat as the superficial manifestati

miRNA-Targeted Herbal Compounds Against Gastric Precancerous Evolution in Chronic Atrophic Gastritis.

Gastric cancer (GC) ranks third globally in cancer mortality. It develops via a well-defined pathological sequence: normal gastric mucosa progresses sequentially to superficial gastritis, chronic atrophic gastritis (CAG), intestinal metaplasia, dysplasia, and ultimately invasive carcinoma. Given this stepwise carcinogenic cascade, early intervention during the precancerous stage, particularly in CAG, is paramount for secondary GC prevention. Emerging evidence underscores that microRNAs (miRNAs), a major subclass of non-coding RNAs (ncRNAs), exert crucial regulatory effects on gastric carcinoge

Exploring the mechanism of acupuncture for Chronic Atrophic Gastritis based on bioinformatics and network topology strategies.

BACKGROUND: Chronic atrophic gastritis (CAG) is a progressive gastric disorder with the potential for malignant transformation. Conventional treatments primarily focus on Helicobacter pylori eradication and symptom management; however, long-term use may be associated with hepatic or renal adverse effects. Although acupuncture has demonstrated therapeutic benefits in CAG, its underlying mechanisms remain insufficiently understood. This study aimed to investigate the multi-target mechanisms of acupuncture in CAG through an integrated strategy combining network pharmacology and experimental valid

Rare Concurrent Findings With Collagenous Gastroduodenitis in a Pediatric Patient.

Collagenous gastritis is a rare type of gastritis characterized by thickened subepithelial collagen and chronic inflammatory infiltrate. We describe the unique presentation of a previously healthy 13-year-old girl, who presented with severe fatigue and exertional dyspnea. The gastric and duodenal histologic features were consistent with collagenous sprue, collagenous gastritis, and chronic atrophic gastritis. To the best of our knowledge, the simultaneous presentation of these entities is highly uncommon in children, underscoring the importance of understanding its pathogenesis.

From pre-tumor to tumor: Decoding the endoscopic-pathologic spectrum of neoplastic lesions in autoimmune gastritis.

Autoimmune gastritis (AIG) is a chronic atrophic gastritis predisposing to neuroendocrine tumors (NETs), dysplasia, and gastric cancer (GC). In a cohort of 1,551 patients with suspected AIG, we identified 46 with neoplastic lesions and 134 without such lesions and compared their clinicopathological and endoscopic features. NETs typically appeared as polypoid/submucosal tumors with dilated vessels or central depression; epithelial neoplasms showed variable morphologies, with irregular microvessels under narrow-band imaging (NBI). Extensive corpus atrophy and antral patchy redness were independe

Efficacy and safety of oral Chinese patent medicine combined with quadruple therapy for chronic atrophic gastritis: a systematic review and network meta-analysis.

BACKGROUND: Chronic atrophic gastritis (CAG) is an important precancerous lesion of gastric cancer, with Helicobacter pylori (H. pylori) infection being a major etiological factor. Although quadruple therapy is the first-line treatment for H. pylori eradication, it still has certain limitations. The comparative efficacy of oral Chinese patent medicines combined with quadruple therapy remains unclear. Therefore, this study conducted a network meta-analysis (NMA) to systematically evaluate their efficacy and safety. METHODS: Randomized controlled trials (RCTs) on oral Chinese patent medicines c

Breaking the Size Barrier: Endoscopic Submucosal Dissection of a Massive Flat Gastric Neuroendocrine Tumor.

Type I gastric neuroendocrine tumors, arising in chronic atrophic gastritis, are usually small, mucosal, or submucosal elevated lesions. Current guidelines recommend endoscopic resection of lesions ≤20 mm and surgical treatment for those >20 mm. We report an unusual case of a 56-year-old woman with a completely flat (Paris 0-IIb) gastric body neuroendocrine tumor (NET) measuring 140 × 35 mm, detected by virtual chromoendoscopy and successfully resected by endoscopic submucosal dissection without complications. Histology revealed about 40 well-differentiated low-grade NET foci. After 6 months,

Machine learning-based gastric cancer risk prediction in an asymptomatic screening population: a retrospective cohort study.

BACKGROUND: Early detection of gastric cancer is critical for improving survival. Although established risk factors are known, few studies comprehensively assess individual gastric cancer risk across diverse clinical features, particularly within a general screening population. Existing machine learning (ML) models often rely on high-risk clinical cohorts or utilize classification methods that fail to account for the essential time-to-event nature of survival data, thus limiting their utility for developing personalized, long-term screening strategies in asymptomatic individuals. This study ai

Spasmolytic Polypeptide-Expressing Metaplasia in Chronic Atrophic Gastritis and Its Role in the Progression From Gastritis to Gastric Cancer.

Chronic atrophic gastritis (CAG) is a well-recognized precursor of gastric cancer (GC). Spasmolytic polypeptide-expressing metaplasia (SPEM) is a key intermediate state during this process, representing a reparative response to gastric mucosal injury, and may promote the development of malignancy under persistent inflammation. Recent studies have suggested that SPEM is not a static lesion but a dynamic and heterogeneous process regulated by epithelial injury, oxyntic atrophy, and chronic inflammation. This review summarizes current knowledge on cellular plasticity, lineage dynamics, and molecu

Mechanisms of Hericium erinaceus polysaccharides on chronic atrophic gastritis: An integrated study of network pharmacology, molecular docking, in vivo experiments, and scRNA-seq.

Chronic atrophic gastritis (CAG) is a precancerous lesion that marks a critical stage for preventing gastric cancer progression, yet targeted therapies remain limited. This study evaluated the therapeutic effects and mechanisms of Hericium erinaceus polysaccharide (HEP) in a mouse model of CAG. Comprehensive physicochemical characterization (HPLC, FT-IR, HPGPC, NMR, methylation analysis) identified core structural features of HEP as a highly branched acidic heteropolysaccharide containing five major monosaccharides. Network pharmacology predicted 44 CAG-related targets of HEP, with IKBKB (enco

Development and validation of an interpretable machine learning model for predicting chronic atrophic gastritis in elderly patients.

BACKGROUND: Chronic atrophic gastritis (CAG) is a key precancerous condition, but its screening in the elderly is hindered by the invasiveness, high cost, and limited accessibility of endoscopy. This study aimed to develop and validate interpretable machine learning (ML) models for predicting CAG in elderly patients and identify key predictive factors. METHODS: We adopted a retrospective cohort study design using data from the Endoscopy Department of Shanghai Tenth People's Hospital. Two independent cohorts were included: a development cohort (January 2023 to December 2024, n = 1,268) for mod

Stromal BMP signalling maintains stem cell genome integrity and prevents gastric cancer initiation.

Gastric cancer primarily originates from gastric stem/progenitor cells and is driven by somatic mutations. Although Helicobacter pylori is the main risk factor, how it drives malignancy is still not well understood. Analysis of single-cell RNA sequencing data reveals that human gastric cancer correlates with suppressed BMP signalling, a crucial niche signal for gastric stem cells, in stromal cells rather than epithelial cells. Genetic disruption of BMP signalling in Col1a2+ stromal cells or Acta2+ myocytes/pericytes alone, but not in gastric stem cells themselves, triggers mutations in gastric

Pernicious Anemia: Basic Pathophysiology and Diagnostic Challenges in Neuropsychiatric Patients.

Pernicious anemia (PA) represents a significant diagnostic challenge in neuropsychiatric patients due to its subtle and variable presentation. While PA is traditionally associated with clinical and biochemical manifestations of anemia, many patients, particularly those with neuropsychiatric symptoms, may have normal hematologic parameters, delaying recognition and appropriate treatment. Neurological and psychiatric symptoms, ranging from cognitive impairment and mood disorders to subacute combined degeneration (SCD) of the spinal cord, can precede hematologic abnormalities, leading to misdiagn
