Huangjin Shuangshen decoction alleviates chronic atrophic gastritis by suppressing TNF/NF-κB signaling and promoting CFTR-associated gastric mucosal barrier repair.

BACKGROUND: Chronic atrophic gastritis (CAG) is a precancerous gastric lesion characterized by persistent inflammatory injury, glandular atrophy, and impairment of gastric mucosal barrier-associated integrity. Huangjin Shuangshen Decoction (HJSS) has shown therapeutic potential in gastritis-related disorders, but its effects on CAG and the underlying mechanism remain unclear. This study investigated whether HJSS alleviates CAG by modulating inflammation-associated barrier dysfunction. METHODS: CAG was induced in mice by MNNG combined with ranitidine and irregular feeding, followed by treatmen

[Mechanism of Sijunzi Decoction in treating chronic atrophic gastritis via "gut microbiota-ferroptosis" axis].

This study investigated the therapeutic efficacy of Sijunzi Decoction on chronic atrophic gastritis(CAG) in rats and its potential mechanism of action in mitigating gastric mucosal injury through the "gut microbiota-hepcidin-ferroptosis" pathway. Specific pathogen-free(SPF) grade male Wistar rats were randomly divided into a normal group, a model group, a positive drug vatacoenayme group, and low-, medium-, and high-dose Sijunzi Decoction groups. A CAG model was established using a composite modeling method combining multiple pathogenic factors with irregular feeding. Gut microbiota

Integrating tongue coating microbiome, tongue coating metabolomics, and exhaled breath metabolomics via machine learning to identify chronic atrophic gastritis.

BACKGROUND: Chronic atrophic gastritis (CAG) is a precancerous gastric condition with limited non-invasive diagnostic options. Alterations in the oral microbiome and its localized metabolic profiles may provide early multi-omics signatures of disease progression. METHODS: We performed 16S rRNA gene sequencing of tongue coating samples, along with untargeted metabolomics of both tongue coating and exhaled breath condensate (EBC) in 139 patients, including 99 patients with CAG and 40 with chronic non-atrophic gastritis (CNAG) serving as a disease comparison group. The tongue coating microbiome,

Inflammation as a Unifying Axis: Shared Pathogenic Mechanisms in Coronary Atherosclerosis and Chronic Atrophic Gastritis.

Coronary atherosclerosis (AS) and chronic atrophic gastritis (CAG) are clinically distinct diseases but share core features of chronic, non-resolving inflammation. This review evaluates their convergent immunoinflammatory architecture and discusses how shared pathogenic nodes may inform dual-purpose therapeutic strategies. Both conditions involve barrier dysfunction driven by oxidative stress and nitric oxide dysregulation, maladaptive immune responses involving macrophage polarization and T-cell plasticity, and sustained inflammatory signaling through pathways such as TLR/NF-κB, HIF, and PI3K

Gastric mucosal pit patterns to rule out premalignant conditions in an intermediate risk Western-Hispanic population: a validation study.

BACKGROUND: Endoscopic surveillance for early-detection of gastric cancer in Western populations relies on risk-stratification using OLGA/OLGIM staging. Validated endoscopic classifications to assess risk and guide biopsies are lacking. We aimed to validate the corpus Yagi AB and antral Pimentel classifications in an intermediate risk Western-Hispanic population. METHODS: We conducted a cross-sectional validation-study to assess the diagnostic-accuracy and interobserver agreement of both classifications. Adults undergoing endoscopy with NBI assessment and Sydney protocol biopsies were include

Coupling Dysfunction of Mitochondria and Actin in Loss of Native Gastric Glands: A Mechanistic Hypothesis for Chronic Atrophic Gastritis Pathogenesis.

The progressive loss of native gastric glands in chronic atrophic gastritis (CAG) represents a critical step toward gastric carcinogenesis, yet the underlying cellular structural mechanisms remain incompletely understood. In this review, we formulate an integrative mechanistic hypothesis that disruption of actin cytoskeletal homeostasis, driven by mitochondrial bioenergetic dysfunction, plays a central role in the progressive loss of native gastric glands. Persistent H. pylori infection and chronic mucosal inflammation may initiate a self-reinforcing cycle of metabolic and structural impairmen

Severe Macrocytic Anemia Uncovering Pernicious Anemia and Early-Stage Gastric Adenocarcinoma: A Case Report.

Pernicious anemia (PA) is an autoimmune cause of vitamin B12 deficiency, often secondary to chronic atrophic gastritis, which increases the risk of gastric malignancies. Early signs, like mild macrocytosis, are frequently overlooked, especially in patients with other risk factors such as long-term metformin use. We present a case illustrating the full clinical trajectory from long-standing, unexplored macrocytosis to severe anemia, ultimately leading to the diagnosis of PA and detection of early-stage gastric adenocarcinoma on surveillance esophagogastroduodenoscopy (EGD). Informed consent was

Ménétrier's disease: a narrative review of molecular pathogenesis, clinical spectrum and evolving therapeutic strategies.

Ménétrier's disease (MD) is a rare, acquired premalignant condition characterized by hypertrophic gastropathy. Its defining features include giant gastric rugal folds, predominantly in the fundus and body of the stomach, foveolar hyperplasia with glandular atrophy and a protein-losing gastropathy leading to hypoalbuminemia and peripheral edema. The pathogenesis is primarily driven by the overactivation of the epidermal growth factor receptor (EGFR) signaling pathway, most commonly due to the overexpression of its ligand, transforming growth factor-alpha (TGF-α). The clinical presentation varie
