Helicobacter pylori is a gram negative bacteria which has recently been associated to tissular changes of the upper digestive tract, however, the causal role has not yet been determined. PGE2-like immunoactivity in mucosal specimens from gastric corpus and antrum was measured in individuals with chronic uremia or without renal diseases in absence or presence of gastric ulcerations and in healthy subjects. Regardless the group of patients, compared to normal mucosa, a significant decrease in PGE2-like immunoactivity (50-70%) was found in mucosa from atrophic, but not from superficial gastritis. This study aimed to determine the prevalence of endoscopic and histological gastroduodenitis as well as helicobacter-like organisms in patients with end stage renal failure undergoing maintenance dialysis treatment. A total of 322 out of 422 patients in our dialysis programme underwent endoscopy and gastroduodenal biopsy specimens were taken from 260. Endoscopic gastroduodenitis occurred in 158 (49%). The correlation between urease activity of Campylobacter pylori and atrophic gastritis was studied. On the basis of fundamental study on the optimal pH of C. pylori urease activity, urease activity of 38 biopsied specimens were measured under pH 5 condition, and compared with the positive ratio of C. pylori. In this study, sensitivity was 86.7%, and specificity was 87.0%, respectively. A follow up study with biopsy was initiated in 1982 to define the relations between variants of intestinal metaplasia and the evolution of chronic atrophic gastritis and gastric ulcer. Radioimmunological assay was used to study the content of insulin and C-peptide in the blood bath in conditions of basal incretion and 1 and 2 hours after a test breakfast in 12 healthy subjects, 36 patients with duodenal ulcer and 20 patients with chronic atrophic gastritis. It was found that patients with duodenal ulcer showed an increased hormonal activity of the pancreas leading to exhaustion of its reserve capacities. A patient with pernicious anemia developed severe intractable diarrhea night and day. Investigation revealed chronic atrophic gastritis and a markedly elevated level of serum gastrin. No obvious explantation for the diarrhea was found, but after antrectomy, the gastrin level returned to normal and the diarrhea subsided. Possible mechanisms for an association between diarrhea and hypergastrinemia include colonic hypermotility secondary to release of acetylcholine and inhibition of fluid and electrolyte reabsorption within the small bowel.
