A single-cell multi-omics atlas defines the cellular and molecular landscape of gastric intestinal metaplasia.

BACKGROUND: Gastric intestinal metaplasia (GIM) is a precancerous lesion closely associated with gastric cancer (GC) risk. A systematic characterization of the molecular profiles across distinct cell types during GIM progression is crucial for devising new intervention strategies and enabling early prevention for GC. METHODS: This study integrates single-cell transcriptomics and proteomics data from gastric tissues of patients with mild chronic non-atrophic gastritis (CNAG) and moderate-to-severe GIM. RESULTS: We identified differentially expressed genes (DEGs) and proteins (DEPs) that show co

Single-cell atlas of gastric cancer reveals malignant epithelial evolution and regulatory reprogramming of the tumor microenvironment.

The inflammation-intestinal metaplasia (IM)-carcinoma cascade has been proposed as a framework for gastric cancer (GC) development, yet the cell-level heterogeneity and microenvironmental remodeling underlying this progression remain poorly characterized. Here, we constructed a single-cell transcriptomic atlas by integrating scRNA-seq data from chronic gastritis (superficial, CGS), IM, cancer-adjacent, and tumor tissues through a unified analytical pipeline. Seven major cell lineages were resolved. Relative to CGS, IM and GC tissues exhibited a progressive contraction of epithelial compartment

Upper Gastrointestinal Biopsy Findings in Pediatric Inflammatory Bowel Disease: Histologic and Clinicopathologic Correlations.

OBJECTIVES: We aimed to evaluate histologic findings in upper gastrointestinal (UGI) biopsies of pediatric inflammatory bowel disease (IBD) patients, their correlation with endoscopic features, and association with disease activity. METHODS: UGI biopsies from 50 pediatric IBD patients (31 Crohn's disease [CD] and 19 ulcerative colitis [UC]) were retrospectively reviewed. Esophageal, gastric, and duodenal samples were categorized as highly suggestive, suspicious, or unlikely for UGI involvement. Associations with endoscopic abnormalities, colonoscopic disease extent, and simplified Geboes scor

Dietary nitrate drives gastritis by modulating gastric microbiota and metabolites.

OBJECTIVE: Dietary nitrate has been increasingly recognized as a potential carcinogen associated with gastritis. In this study the mechanistic role of a high-nitrate diet (NaD) in driving gastritis was elucidated with a focus on modulation of the gastric microbiota composition and metabolomic profiles. METHODS: Animals were randomly assigned to two dietary intervention groups using a C57BL/6 mouse model: a NaD containing 7.5% nitrate; or a standard normal diet (ND). Gastric microbiota composition was characterized based on full-length 16S rRNA sequencing and gastric metabolite profiles were a

Dupilumab as a rescue therapy for steroid-dependent eosinophilic gastritis in a child unresponsive to elimination diet: a case report.

BACKGROUND: Eosinophilic gastrointestinal diseases (EGIDs) are chronic, rare and heterogeneous disorders characterized by eosinophilic infiltration of gastrointestinal tract, resulting in gastrointestinal dysfunction. Although standard-of-care therapeutic strategies for eosinophilic esophagitis (EoE) are well-defined, limited evidence on successful treatment of eosinophilic gastrointestinal disorders beyond eosinophilic esophagitis (non-EoE EGIDs) in children is available. While dupilumab, a monoclonal antibody that inhibits interleukin (IL)-4 and IL-13 signaling, is currently approved in chil

Assessment of Salivary Polymerase Chain Reaction as a Non-invasive Method for Detecting Helicobacter pylori Infection in Patients With Symptomatic Gastritis.

INTRODUCTION: Helicobacter pylori  infection is a major etiological factor in chronic gastritis and several gastroduodenal diseases. Although gastric biopsy remains the gold standard for diagnosis, it is invasive and not always suitable for screening or repeated monitoring. Saliva-based polymerase chain reaction (PCR) has emerged as a promising non-invasive diagnostic approach. The present study aimed to evaluate the diagnostic performance of salivary PCR for detecting H. pylori infection and to compare its findings with gastric biopsy results in patients with symptomatic gastritis. MATERIALS

CHRONIC URTICARIA RELATED TO HELICOBACTER PYLORI INFECTION - A CASE REPORT.

Chronic urticaria (CU) is a complex inflammatory skin disorder characterized by recurrent wheals and pruritus lasting longer than six weeks, often resulting in significant impairment in quality of life, sleep disturbance, and psychological distress. Although the majority of cases are classified as chronic spontaneous urticaria without identifiable triggers, a growing body of literature suggests that persistent infections may contribute to disease pathogenesis through immune-mediated mechanisms. Helicobacter pylori infection has been increasingly investigated because of its ability to induce ch

Rationally Engineered D-Amino Acid Peptide DT7-3 Combats Multidrug-Resistant Helicobacter pylori via a Novel "Triple-Hit" Mechanism.

Helicobacter pylori (H. pylori) is the primary etiological agent for chronic gastritis, peptic ulcers, and gastric adenocarcinoma. The alarming rise in multidrug-resistant (MDR) strains, particularly against clarithromycin (CLR), metronidazole (MNZ), and levofloxacin (LVX), has severely compromised standard therapies. Thus, there is an urgent clinical need for novel antimicrobial agents that operate through distinct mechanisms to bypass resistance pathways and mitigate gastric cancer risk. We designed and synthesized a series of antimicrobial peptides, focusing on the proteolytically stable al

[Pediatric collagenous gastritis: a rare etiology of iron-deficiency anemia.].

Collagenous gastritis is a rare pediatric disease characterized by chronic gastritis with superficial, irregular mucosal involvement, focal atrophy of the gastric glands, an inflammatory infiltrate predominantly composed of lymphocytes and plasma cells, and areas of epithelial erosion. Focal collagen deposition in the subepithelial region of the lamina propria is a hallmark feature. Gastric epithelial erosions can cause hematemesis and, more generally, occult gastrointestinal bleeding, leading to chronic iron-deficiency anemia. Currently, there are no established management guidelines, and dat

Genomic characterization of clinical Helicobacter pylori strains from Korea: insights into phylogeny, pan-genome structure, and virulence factors.

BACKGROUND: Helicobacter pylori (H. pylori) is a globally prevalent gastric pathogen whose genetic diversity and virulence contribute to the varied clinical outcomes. Whole-genome sequencing has revolutionized microbial genomics by allowing high-resolution analyses to obtain insights into the evolution of pathogenic organisms; however, few studies have specifically focused on strains from Korea, where H. pylori infection and gastric cancer are highly prevalent. This study aimed to investigate the phylogenetic characteristics and virulence factor profiles of Korean H. pylori strains using whole

Comparative analysis of endoscopic and histopathological findings in patients with chronic gastritis.

Chronic gastritis is a common gastrointestinal disorder with variable clinical presentations and the accuracy of upper gastrointestinal endoscopy in reflecting histopathological changes remains inconsistent. Therefore, it is of interest to compare endoscopic findings with histopathological grading in 189 adult patients with chronic gastritis. The most frequent endoscopic finding was erythematous gastritis, followed by erosive and nodular changes, while histopathology revealed mild to severe chronic gastritis. Therefore, this study advances knowledge by providing valuable insights into the corr

Inflammation as an Etiologic Agent of Gastric Cancer.

Chronic inflammation in the stomach significantly increases the risk of gastric cancer-a deadly disease with a poor prognosis, largely because it is often diagnosed too late. A better understanding of the pathophysiology and molecular mechanisms underlying this increased risk is essential to identify novel diagnostic and therapeutic targets for inflammation-induced carcinogenesis. Gastric epithelial atrophy, a precursor to cancer, can be induced by chemical, autoimmune, and infectious stimuli, primarily infection with Helicobacter pylori. We and others have shown that atrophy can also be induc

Hcn1-dependent engram neurons in the PVN encode gastric inflammatory sensitization.

The brain encodes peripheral inflammatory signals, but the regulatory mechanism by which the brain controls gastric inflammation remains unclear. This study reveals a circuit through which Fos-positive neurons in the paraventricular nucleus of the hypothalamus (PVN) regulate gastritis via a PVN-dorsal motor nucleus of the vagus-stomach neural circuit and the hypothalamic-pituitary-adrenal (HPA) axis. Using activity-dependent genetic labeling, chemogenetics, and optogenetics, we demonstrate that FosPVN neurons are essential for gastritis progression and can be activated by stress to drive chron

Essential minerals and risk of pancreatic diseases: a large-scale prospective cohort study.

OBJECTIVE: Pancreatic disorders are characterised by high mortality rates and limited therapeutic options and pose a global health challenge. As a dual-function organ with endocrine and exocrine roles, the pancreas exhibits a heightened sensitivity to the disruption of mineral homeostasis. However, the pathophysiological associations between mineral imbalances and pancreatic diseases remain controversial, and large-scale population studies validating these relationships are lacking. METHODS: This prospective cohort study used data from the UK Biobank and enrolled 191,875 participants with a m
