Inflammatory Cytokines as Potential Inducements of Early Gastric Mucosal Lesions in Helicobacter pylori-Infected Patients.

BACKGROUND: Chronic inflammation due to Helicobacter pylori infection causes the development of many gastrointestinal diseases, such as gastric ulcer and atrophic gastritis. This study is aimed at investigating the relationship between inflammatory cytokines and pepsinogen levels in H. pylori-infected patients. METHOD: A total of 165 H. pylori-infected subjects without atrophic gastritis were recruited, and their demographic, medical, and lifestyle information were collected. Inflammatory cytokines such as IL-8, IFN-γ, TNF-α, CRP, IL-17A, IL-1β, IL-18, and pepsinogen (PGs I and II) levels wer

Atrophic gastritis of distinct etiologies: malignant potential in Helicobacter pylori-associated and autoimmune forms.

Atrophic gastritis is an important step in the Correa cascading pathway. It forms a pivotal period between chronic inflammation and a biologically-disrupted mucosal epithelial phenotype leading to gastric neoplasia. From a more than superficial perspective, both Helicobacter pylori (H. pylori)-associated atrophic gastritis and autoimmune atrophic gastritis converge on glandular loss and metaplastic reprogramming, but their etiologic pathways, molecular mediators, topographic distribution and neoplastic characteristics differ drastically. H. pylori infection induces multifocal atrophy and incom

Robotic Resection of Caudate Lobe Hepatocellular Carcinoma.

INTRODUCTION: Robotic liver resection is a valuable alternative to both open and laparoscopic techniques. We report a case of hepatocellular carcinoma (HCC) located in the caudate lobe, and the tumor was successfully resected with a robotic approach. When performed by experienced surgeons, robotic resection can be a technically sound and an oncological appropriate option for selected patients with complex hepatic tumors. CASE DESCRIPTION: A 70-year-old man with a history of chronic hepatitis C infection, compensated cirrhosis, cholelithiasis, type 2 diabetes mellitus, and hypertension was ref

Surgical repair of paraesophageal hernia resolves unexplained iron deficiency anemia in the vast majority of patients: a propensity-matched multicenter study.

BACKGROUND: Paraesophageal hernias (PEH) frequently associate with iron deficiency anemia (IDA) through unclear mechanisms. We investigated whether surgical repair effectively treats unexplained IDA in patients with PEH. METHODS: This multicenter retrospective study (2010-2024) analyzed 1700 patients undergoing hiatal hernia repair across Asuta Medical Centers. After excluding patients with identifiable bleeding sources (Cameron lesions, erosive gastritis, ulcers), 522 patients remained: 136 with unexplained IDA and 386 controls. Propensity score matching (1:3) aligned 99 anemic patients with

The prevalence of gastric intestinal metaplasia in the Saudi population: a single-tertiary center retrospective study.

BACKGROUND: Gastric intestinal metaplasia (GIM) is characterized by the transformation of gastric mucosa into an intestinal phenotype. This condition arises from gastric cell inflammation caused by various etiologies, such as infection or autoimmune reactions. GIM is a critical stage in the carcinogenic pathway and may progress into gastric cancer (GC), one of the most prevalent and morbid cancers worldwide. In this study, we examined the prevalence of GIM among patients who underwent upper endoscopy or gastric resection, along with associated risk factors. METHODS: This retrospective study i

Oncometabolite signatures from tumor-stroma crosstalk as potential non-invasive biomarkers.

Tumor metabolism, a crucial component in cancer progression, represents a potential prognostic and diagnostic platform in cancer detection. Here, we show that patient-specific stromal mesenchymal cells exhibit distinct behaviors in promoting either tumor growth or dissemination. Tumor-associated fibroblasts (TAFs) isolated from non-metastasized colorectal adenocarcinomas predominantly supported cancer cell proliferation, whereas TAFs isolated from metastatic adenocarcinomas facilitated cancer cell migration. An in vitro analysis of stromal paracrine factors revealed that variations in mitochon

Neutrophil efferocytosis in chronic inflammatory gastrointestinal diseases: mechanistic insights and therapeutic potential.

Efferocytosis-the phagocytic clearance of apoptotic cells-is central to tissue homeostasis and the active resolution of inflammation. Although its mechanistic basis and disease relevance have been studied in isolation, no review has comprehensively integrated neutrophil efferocytosis mechanisms, their pathophysiological roles across major chronic inflammatory gastrointestinal diseases, and natural product-based therapeutic strategies-a gap this work addresses. We systematically describe the efferocytic recognition cascade, encompassing find-me signals, eat-me signals (including phosphatidylser

One-Pot LAMP-Coupled CRISPR/Cas12b Assay Enables Sensitive Detection of Helicobacter pylori.

Helicobacter pylori (H. pylori) infection is closely associated with the development of chronic gastritis, peptic ulcers, and gastric cancer, highlighting the importance of rapid and accurate detection for disease prevention and clinical management. In this study, a one-pot LAMP-CRISPR/Cas12b assay targeting the CagA gene was developed for H. pylori detection. First, the LAMP system was optimized by systematically screening key reaction components. Subsequently, a one-step LAMP-CRISPR/Cas12b detection platform was established through optimization of the ratio between the LAMP premix and CRISPR

Lymphoplasmacytic Gastritis in Cheetahs Under Human Care: A Bile Acid-Driven Gastroenteropathy Arising from Disrupted Feeding Ecology.

Lymphoplasmacytic gastritis (LPG) is one of the most prevalent chronic diseases affecting cheetahs (Acinonyx jubatus) under human care, yet its underlying cause remains unresolved. Gastric inflammation occurs in the majority of adult captive cheetahs but is uncommon in free-ranging populations, suggesting that management-related factors contribute to disease pathogenesis. This review proposes that LPG represents a bile acid-driven gastroenteropathy arising from disruption of the natural feeding ecology of the cheetah. In free-ranging systems, cheetahs consume large episodic meals separated by

Vitamin D Deficiency and Replacement Challenges in Type 1 Gastric Neuroendocrine Tumors: A Comparative Study.

Background/Objectives: Type 1 gastric neuroendocrine tumors (gNET) arise in the setting of autoimmune chronic atrophic gastritis and secondary hypergastrinemia. Vitamin D deficiency (VDD) has been associated with bone impairment and adverse outcomes in patients with neuroendocrine tumor (NET); however, data specifically addressing gNET remain limited. This study aimed to evaluate vitamin D status, supplementation requirements, and bone involvement in patients with type 1 gNET compared with those with entero-pancreatic NET (EP-NET). Methods: This retrospective study included patients with type

Renqingchangjue ameliorates MNNG-induced chronic atrophic gastritis by inhibiting the TNF/NF-κB/Caspase-3 axis.

BACKGROUND: Chronic atrophic gastritis (CAG) is a pivotal premalignant stage in the Correa cascade, characterized by progressive and largely irreversible loss of gastric glands and an elevated risk of gastric cancer. Renqingchangjue (RQCJ), a classical Tibetan multi-herb formula, has demonstrated clinical and pharmacological benefits in gastritis, yet its mechanisms of action in CAG remain inadequately defined. PURPOSE: This study aims to clarify the therapeutic efficacy and mechanistic basis of RQCJ in CAG. METHODS: We established a network pharmacology-bioinformatics workflow to predict po

Diagnostic Overlap Between Uremia and Severe Hyperkalemia in Chronic Kidney Disease: Emphasizing Laboratory-Guided Urgency Despite Absent EKG Changes.

Hyperkalemia is a potentially life-threatening electrolyte disturbance often associated with characteristic electrocardiographic (EKG) changes and neuromuscular manifestations. However, clinical presentation does not always correlate with biochemical severity. We present the case of a 53-year-old female with chronic kidney disease (CKD) who developed acute-on-chronic kidney injury with severe hyperkalemia (serum potassium 8.7 mmol/L) and elevated urea (94 mg/dL), yet without EKG abnormalities or overt symptoms. The patient reported persistent nausea for 24 hours without abdominal pain or hemod

Oxyntic Gland Intraepithelial T Lymphocytes as Progression Marker of Potential Autoimmune Gastritis.

INTRODUCTION: Autoimmune gastritis (AIG) is a chronic immune-mediated disease with potential for serious clinical consequences, yet early identification remains challenging. We aimed to assess whether deep oxyntic gland intraepithelial CD3 + T lymphocytosis may serve as a predictive histological marker of progression from potential -i.e., parietal cell antibody-positive patients without baseline gastric atrophy- to overt AIG. METHODS: We prospectively enrolled 45 adult parietal cell antibody-positive patients without histological evidence of mucosal atrophy between 2020 and 2022. All underwen

Intestinal Malrotation Through Acute Obstructive Abdomen in an Adolescent From Northeastern Brazil: A Case Report.

Intestinal malrotation is a congenital anomaly of the midgut resulting from abnormal embryonic rotation. Although predominantly a neonatal diagnosis, it is an extremely rare, isolated finding in adolescents, often presenting with nonspecific symptoms that lead to diagnostic delays and increased morbidity. We report a 17-year-old male presenting with acute abdominal pain, distension, and vomiting. Diagnosis was established via contrast-enhanced computed tomography (CT), which identified spiral rotation of mesenteric vessels and colonic displacement. The patient initially underwent a laparoscopi

Pilot Newborn Screening for Vitamin B12 Deficiency in the Czech Republic: Results and Detailed Studies on Identified Babies and Their Mothers.

Neonatal vitamin B12 (B12) deficiency can cause neurodevelopmental harm, and newborn screening (NBS) may enable early detection and treatment. We conducted a multicenter pilot project in four Prague university hospitals between 1 June 2022 and 30 June 2025. Algorithms included the determination of propionylcarnitine-derived primary markers using flow-injection tandem mass spectrometry and second-tier methylmalonic acid (MMA), with total homocysteine measured only when MMA was increased. Of 34,302 screened newborns with consent, 1365 (3.98%) triggered second-tier testing; 9 had MMA > 2.5 µmol/L
