Immunokinetic signatures of Helicobacter pylori infection: mechanistic insights for host immunity.

Helicobacter pylori is a highly adaptable bacterial pathogen that affects nearly half of the global population and plays a significant role in chronic gastritis, peptic ulcer disease, and gastric carcinoma. The immunokinetic profile of H. pylori infection is characterized by a paradox of cellular response that does not eliminate the bacterium, alongside inconsistent mucosal antibody protection. Reported studies generally demonstrate an early increase in mucosal interleukin-8 (IL-8), followed by increased T helper 1 cells (Th1)- and T helper 17 cells (Th17)-associated cytokine responses, includ

[Comparison of clinicopathological features of early gastric cancer and precancerous lesions in different chronic gastritis settings].

Objective: While chronic gastritis caused by Helicobacter pylori (H.pylori)infection is a well-established driver of gastric carcinogenesis, research interest has increasingly focused on the association between autoimmune gastritis (AIG) and gastric cancer. The aim of this study was to compare the endoscopic and pathological features of early gastric cancer (EGC) and precancerous lesions between H. pylori-related gastritis and AIG. Methods: Patients with EGC and precancerous lesions who attended the Department of Gastroenterology at Beijing Hospital between January 2022 and February 2025 were

Mechanistic insight into eosinophilic enteritis and related disorders.

PURPOSE OF REVIEW: Eosinophilic gastrointestinal diseases (EGIDs) affecting the gastrointestinal tract outside of the esophagus are categorized as non-EoE EGIDs, and knowledge regarding their natural history and pathophysiology is rapidly advancing. This review synthesizes evolving mechanistic insights in non-EoE EGID immunopathogenesis, emerging paradigms of disease development, and how these insights influence potential therapeutic options for these conditions. RECENT FINDINGS: The prevalence of non-EoE EGIDs is rising, though overall prevalence remains much lower than that of eosinophilic

Mental Health in Eosinophilic Gastrointestinal Disorders: A Work Group Report of the AAAAI Eosinophilic Gastrointestinal Disorders Committee.

Psychiatric comorbidities, including anxiety and depression, are increased in individuals with eosinophilic gastrointestinal disorder (EGID) compared with the general population. However, there are no established guidelines for screening and evaluating mental health disorders, and screening practices are unclear. We e-mailed an anonymous Web-based survey to members of the American Academy of Allergy, Asthma & Immunology and the Consortium of Eosinophilic Gastrointestinal Disease Researchers to examine mental health screening practices among clinicians caring for patients with EGIDs in the Unit

Autoimmune gastritides: A critical review.

BACKGROUND: Autoimmune gastritis (AIG) has long been considered a host-related autoimmune disease that results in oxyntic atrophy, leading to Pernicious Anemia and increasing neoplastic risk. Current evidence discloses that gastric autoimmunity includes a spectrum of inflammatory diseases (i.e., Autoimmune Gastritides) with distinct etiologies, comorbidities, clinical presentations, histological phenotypes, and outcomes. AIM: To distinguish the clinical-pathological subtypes of immunomediated gastric disorders. RESULTS: Gastric autoimmune diseases comprise Type-1 and Type-2 variants. Type-1

A living hydrogel platform for wireless light-controlled microbial therapy in the stomach.

Engineered microbial therapies provide a promising strategy for in situ production of therapeutic molecules at disease sites; however, their clinical translation remains limited by insufficient retention at target tissues and the lack of external control after administration. Here, we develop a wearable bioelectronic-assisted living hydrogel platform that enables rapid in situ immobilization and remote regulation of engineered bacteria. Optogenetically engineered bacteria capable of green-light-induced anti-interleukin-6 (anti-IL-6) secretion were encapsulated within a photoresponsive Gelatin

From histological recovery to residual gastric cancer risk after Helicobacter pylori eradication: a multi-database bibliometric and clinical decision-domain analysis.

BACKGROUND: Successful Helicobacter pylori eradication reduces gastric cancer risk, but residual risk persists in patients with established atrophy or intestinal metaplasia. We integrated multi-database science mapping with independent article-level clinical decision-domain classification to quantify how the research agenda evolved from mucosal recovery to residual-risk management and to identify decision-critical evidence gaps. METHODS: Web of Science, Scopus, and PubMed were searched through 27 July 2026. Records were deterministically deduplicated and independently screened. Bibliometric n

Case Report: Steroid-responsive recurrent iron-deficiency anemia in pediatric collagenous gastritis.

Collagenous gastritis (CG) is a rare inflammatory disorder of the stomach and an uncommon but important cause of iron-deficiency anemia in children. Currently, there are no accepted therapeutic guidelines for treating these patients. We report a case in an 11-year-old boy who presented with recurrent iron-deficiency anemia requiring blood transfusion with an ultimate diagnosis of CG on repeat endoscopy. While iron supplementation and proton pump inhibitors failed to achieve clinical remission, the addition of oral prednisone effectively corrected the anemia. Notably, endoscopy improved despite

Gastric cytomegalovirus disease mimicking locally advanced gastric adenocarcinoma in a patient with newly diagnosed AIDS: A case report and literature review.

INTRODUCTION: Gastric cytomegalovirus (CMV) disease is an uncommon manifestation of tissue-invasive CMV infection and, in rare instances, may mimic gastric malignancy. Recognition of this pseudomalignant presentation is important to prevent misdiagnosis and inappropriate oncologic management. CASE PRESENTATION: A 65-year-old woman presented with abdominal pain, diarrhea, recurrent post-prandial vomiting, poor oral tolerance, and a 20-kg weight loss over one year. Upper gastrointestinal endoscopy showed a large ulcerated antral mass with raised borders, highly suspicious for gastric adenocarci

The oral-gastric microbial axis in children: salivary signatures as non-invasive biomarkers for pediatric gastritis and Helicobacter pylori infection.

Helicobacter pylori is commonly acquired in childhood, with the mouth a presumed portal of entry, although the relative contributions of oral-oral, gastro-oral, and fecal-oral transmission remain unresolved. It is a principal cause of pediatric gastritis, yet most infected children are asymptomatic, and the pediatric mucosa mounts a tolerogenic response rather than the aggressive inflammation seen in adults. In the pathway defined by pediatric guidelines, diagnosis depends on endoscopy with multiple gastric biopsies, an invasive reference standard not suitable for serial assessment; non-invasi

A Non-Endoscopic Dynamic Nomogram for Atrophic Gastritis Risk Stratification Based on Serum Indicators and Clinical Factors.

BACKGROUND: Atrophic gastritis (AG) is a key precancerous lesion in gastric carcinogenesis, underscoring the need for non-endoscopic screening tools to improve early detection. This study aimed to develop a predictive model for AG based on serum indicators and clinical factors. METHODS: This single-center retrospective study enrolled 520 patients who underwent gastroscopy with biopsy at Shaoxing Second Hospital between January 2023 and December 2025. AG severity was classified via the Kimura-Takemoto system (no/mild: C0-C1, n=349; moderate/severe: C2-C3 and O-type, n=171). Multivariate analys

Complete Resolution of Unprovoked Left Renal Vein Thrombosis and Pulmonary Embolism Associated With Severe Iron Deficiency Anemia: A Five-Year Follow-Up Case Report and Dual-Energy CT Appraisal.

Combined renal vein thrombosis (RVT) and pulmonary embolism (PE) in young adults without nephrotic syndrome, malignancy, or an identified major thrombophilia is uncommon. Severe iron deficiency anemia (IDA) has been associated with increased thrombotic risk, although a causal relationship cannot be established from an individual case. A 21-year-old male patient presented with flank and chest pain. Diagnostic workup confirmed acute left RVT and an acute embolism of the posterior basal segmental artery of the right lower lobe with pulmonary infarction, concurrent with profound microcytic IDA (he

Gastrin and cholecystokinin 2 receptor, and gastrin-releasing peptide receptor in cancer: From pathogenesis to precision medicine.

Gastrin regulates gastric acid secretion and mucosal homeostasis via the cholecystokinin-2 receptor (CCK2R). Aberrant signaling via CCK2R (cholecystokinin family) and the distinct class A GPCR, gastrin-releasing peptide receptor (GRPR; bombesin family), drives oncogenesis across diverse GI and non-GI malignancies through calcium mobilization, MAPK/PI3K activation, EGFR transactivation, and constitutive splice variants like CCK2Ri4sv. Furthermore, precursor forms such as progastrin remodel the tumor microenvironment by interacting with non-CCK binding sites (p78, Annexin A2), converting fibrobl
